Objective To analysis the effect of using comprehensive rehabilitation treatment for osteoporotic bulging disc combine diabetes otients.Methods Ninety cases osteoporotic bulging disc combine diabetes patients, who treated in Guanganmen Hospital of Chinese Academy of Traditional Chinese Medicine from January 2011 to January 2011, were randomly divided them into observation group and control group, 45 cases in each group, control group was given conventional treatment, the observation group was given comprehensive rehabilitation treatment, compare visual analogue scale (VAS) score and Oswetry functional disability index (ODI) changes in two groups after treatment, and compared the efficient of treatment.Results VAS scores after treatment for one course and 2 courses in observation group were respectively (2.05± 1.31) and (1.26± 1.03) points,ODI score was respectively (26.37±5.10) and (21.82±4.57) points, VAS score in the control group was (5.85±1.38) and (3.29±1.94) points,ODI score was respectively (37.29±6.32) and (28.33±5.30), all obviously improving than before treatment, but compared with control group, the observation group improved more apparent,the difference between groups was statistically significant(P＜0.05).Treatment effective rate in observation group was 95.56％ (43/45), the control group was 80.00％ (36/45), the difference between groups was statistically significant(x2=5.075 ,P＜0.05).Conclusion Comprehensive rehabilitation therapy applied in treatment for osteoporotic bulging disc combine diabetes patients can obviously improve the patient's pain symptomsand and the lumbar spine, obtain better treatment effect, worthy of clinical popularization and applications.

Objective To study the chemical components from the pericarps of Juglans mandshurica with anti-tumor activity.Methods It showed that pericarps of J.mandshurica had a good performance in treating cancer and low toxicity from CHCl3 and EtOAc portions by a lot of pharmacological experiments and modern clinical application in the past.Compounds were isolated by chromatography on silica gel columns and AB-8 macroporous resin.On the basis of spectral evidences including 1D-NMR and 2D-NMR,the structures of these compounds were determined.Results These compounds from CHCl3 portion were identified as 20(S)-protopanaxadiol-3-one(1),dammara-20,24-dien-3?-ol(2),galeon(3),juglanin A(4),2?,3?,23-trihydroxy olean-12-en-28-oic acid(5),2?,3?,23-trihydroxy urs-12-en-28-oic acid(6),oleanolic acid(7),ursolic acid(8).Conclusion Compound 1 is a new natural product.Compounds 2 and 5-8 are isolated firstly from the plants of Juglans L.which are due to pentacyclic triterpane.Compound 3 is isolated from the pericarps of J.mandshurica for the first time.

OBJECTIVE To explore the canonical prevention and management method to improve the control quality safety of hospital infection in operating room,and the medical care quality.METHODS The key points included infection institution,nosocomial control,operating room management,effective infection disinfection,isolation and other measures.RESULTS According to canonical scientific management available to quality safety of infection prevention,it could prevent infection in operating room.CONCLUSIONS In order to prevent nosocomial infection the accomplishment of effective disinfection and isolation measures during contaminated operation in hospital operating room is an important guarantee to the medical care quality.

Objective To study the value of endoscopic management of thyroid disease through periareola approach. Methods From Nov. 2008 to Feb. 2010, 20 cases were treated by endoscopic throidectomy using bilateral areola three-hole method. The chest skin and platysma were separated and CO2 (6 - 8mmHg pressure)was inflowed to establish the operative space. Thyroid tissues were cut and separated by harmonic scaple. Recurrent laryngeal nerve, superior laryngeal nerve and parathyroid gland were preserved. Results Operations were successfully completed using endoscope, among which there were 1 case of thyroid cyst resection, 5 cases of thyroid adenoma resection, 8 cases of thyroid unilateral partial resection, 3 cases of thyroid bilateral partial resection and 3 cases of thyroid sub-total resection. The average operation time was (100. 0 ± 30. 5) min. Blood loss was (70. 0 ± 15. 0) ml. 1 case of neck skin burns and 3 cases of subcutaneous hydrops appeared. No convertion occured. Conclusion Endoscopic thyroidectomy through periareolar approach is a safe minimal invasive surgical procedure with excellent cosmetic results.

Objective To review the results of limb lengthening over an intramedullary nail in management of femoral defect and length discrepancy.Methods Femoral lengthening over an antegrade intramedullary nail was applied to treat length discrepancy of 4 to 7 cm for three cases,and femoral defects of 8 and 11 cm respectively for two cases from January 2001 to December 2003.The nail and an external fixator(Ilizarov external fixator for bone defect and Orthofix limb-reconstruction system for discrepancy)were applied concomitantly at the time of femoral osteotomy.After a mean fixation time of 16.2 days(13 to 24 days),gradual distraction began at a rate of 0.25 millimeter four times a day.When the managed femur reached the expected length,the nail was locked and the fixator was removed in patients with length discrepancy.The procedure was not finished in patients with bone defects until bony consolidation was partially seen.Results All the patients were followed up for a mean time of 36.4 months(range,22 to 47 months).The mean gain in length was 7.2 cm(range,4 to 10 cm)with good consoli- dation,the mean duration of external fixation was 5 months(range,3 to 8 months),and the mean external fixation index was 0.75 month per centimeter(range,0.57 to 1.00 month per centimeter).Neither axial deviation nor re-fracture occurred.At the last follow-up,the mean range of movement of knee was 107?(80?to 135?).Con- clusions In management of defect and length discrepancy of limbs,lengthening over an intramedullary nail has the advantages of short duration of external fixation,good protection against refracture,and earlier rehabilitation of knee joint,as well as disadvantages of long operation time,massive intraoperative bleeding,high cost and much post- operative scarring.

Objective To explore the expression and clinical significance of hypoxia-inducible factor 1α(HIF-1α)and glucose transporter-1(GLUT-1)in human lung adenocarcinoma cell line A549 and in elderly primary lung cancer tissue under hypoxia. Methods MTT assay was used to test the effect of CoCl2-induced hypoxia on the growth activity of lung adenocarcinoma cell line A549.Reverse transcription-polymerase chain reaction analysis(RT-PCR)and Western blot were used to determine the changes of HIF-1α and GLUT-1 in lung adenocarcinoma cell line A549 under hypoxia.The expressions of HIF-1α and GLUT-1 were detected by using immunohistochemical method and RT-PCR in 60 specimens of lung carcinoma and 26 benign lung tissues in elderly patients with pulmonary diseases.Their interrelations and their relationship with clinicopathologicaI characteristics of the tumors were analyzed. Results The growth activity of lung adenocarcinoma cell line A549 was increased under hypoxia.RT-PCR analysis indicated that the mRNA expressions of HIF-1α and GLUT-1 were 0.69±0.08,0.65±0.09 after hypoxia exposure for 24 hours,and 0.99±0.16,0.83±0.11 after hypoxia exposure for 48 hours,which were higher than those in control group (0.47±0.11,0.40±0.08,P＜0.01).The results from Western blot showed that the protein expressions of HIF-1α and GLUT-1 were 1.57±0.29,1.66±0.28 after hypoxia exposure for 24 hours,and 2.31±0.46,2.22±0.34 after hypoxia exposure for 48 hours,which were higher than those in control group(0.93±0.07,1.06±0.17,P＜0.01).In specimens of lung carcinoma,the positive expression rates of mRNA and proteins of HIF-1α and GLUT-1 were higher than those in benign lung tissues(all P＜0.01).Compared with the mRNA and protein levels of HIF-1a and GLUT-1 in lung carcinoma tissues of Ⅰ and Ⅱ stage,there were marked increases in Ⅲ and Ⅳ stage (all P＜0.05).The expressions of HIF-1α and GLUT-1 in lymph node metastasis group were higher than those in group without lymph node metastasis(all P＜0.05).There were positive correlations between the mRNA and protein expressions of HIF-1α and between the mRNA and protein expressions of GLUT-1(r1=0.527,r2=0.408,respectively,both P＜0.01). Conclusions CoCl2 can up-regulate the expressions of HIF-1α and GLUT-1 in lung adenocarcinoma cell line A549 by inducing hypoxia.The hypoxia-mediated effects on them have the potential to make lung cancer cells further grow and promote hypoxie tolerance.The overexpressions of HIF-1α and GLUT-1 are closely correlated with the progression of lung carcinoma.Combined detection of HIF-1α and GLUT-1 may be helpful for the diagnosis and prognosis prediction of primary lung cancer in elderly patients.

Objective To investigate the relationship between 5 ,10-methylenetetrahydrofolate reductase (MTHFR) gene C677T locus polymorphism and unexplained adverse pregnancy.Methods Three hundreds and twenty pregnant women with unknown ad-verse pregnancy ≥ 2 times in the infertility department ,and obstetrics and gynecology department of our hospital from June 2014 to May 2016 were selected as the case group and 388 healthy non-abortion parous women were taken as the control group.The MTH-FR gene C677T locus polymorphism in the two groups was analyzed by using gene chip method.The differences in genotype and al-lele frequency distribution were compared between the two groups ,and the relationship between unexplained adverse pregnancy and MTHFR C677T locus polymorphism was analyzed.Results The frequency distribution of MTHFR genotype C677T C/C had sta-tistically significant difference between the case group and control group (P<0.05 ,OR=0.284).The frequency distribution of gen-otype C/T had no statistical difference between the case group and control group (P=0.400 ,OR=1.140).The frequency distribu-tion of genotype T/T had statistical difference between the case group and control group (P<0.05 ,OR=7.672).The frequency distribution of allele C and T had statistical difference between the case group and control group ( P< 0.05 ,OR= 0.304 ). Conclusion The high expression of MTHFR C677T genotype T/T may be a risk factor of unknown adverse pregnancy in child-bearing age women.

AIM：To analyse sequences of p62dok amino acid and cDNA and to investigate p62dok tyrosine phosphorylation and its relation with p21ras GAP. METHODS：The purified p62dok was extracted from CHO/IR cells. The peptide sequence of p62dok was carried out on a high performance analyzer. PCR was performed with the primers designed from the sequence of p62dok amino acid. Western blot and immunoprecipitation were used to identify tyrosine phosphorylation of p62dok and the binding of p62dok with p21ras GAP. RESULTS：The p62dok cDNA is a 1863 bp sequence and code 481 amino acid with 15 tyrosine residues and a putative pleckstrin homology domain. The p62dok protein is rich in PxxP motif. The tyrosine-phosphorylated p62dok can bind p21ras GAP. CONCLUSION：Perhaps p62dok is a new signaling molecule and play an important role in insulin signaling networks through RAS/MAPK pathway.
