Objective To observe the therapeutic effect and safety of dexmedetomidine for sedation in ICU. Methods We selected 40 patients who transferred to the ICU with retained endotracheal intubation after abdominal surgery under general anesthesia in this study. 40 patients were randomly divided into the dexmedetomidine group (D group,20 cases) and midazolam group (M group,20 cases) . Patients in group D were treated with loading dose 1 μg/kg from central vein pump injection 20 min, 1h according to Ramsay score for each adjustment of drug dose is pumped into 0.2 ~ 0.7 μg/(kg.h). Patients in M group were given a loading dose of midazolam 0.05mg/kg intravenous injection 1 min, depending on the degree of sedation maintenance dose of 0.02 ~ 0.1 mg/(kg.h). Pain scores every 2 h (NRS),when the NRS score>4 sharing, plus fentanyl 1μg/kg. When the patients achieved the offline condition, the endotracheal tube was pulled out. The sedation efficiency, dosage of fentanyl, after discontinuation wake-up time, extubation time, duration of mechanical ventilation, the incidence of cardiovascular events and delirium were compared between two groups. Results The sedation efficacy in Group D sedation was significantly higher than in group M ( <0.05) . Compared with group M, the dosage of fentanyl in patients in D group was significantly reduced ( <0.05), the wake time after stopping, extubation time were significantly shorter ( <0.05) . The incidence of cardiovascular events has no significant difference between two groups during the treatment. The incidence of postoperative delirium in D group was significantly lower than in that in group M ( <0.05) . Conclusions Dexmedetomidine has good sedative effect, and can shorten the time of extubation with hemodynamic stability and low incidence of delirium for patients in the ICU. It is an ideal ICU sedative.

[Objective]To summarize clinical experience of Pemberton's ilium osteotomy in developing hip dislocation and emphasize the combined and individual principles in the clinical practice. [Methods]From February 1996 to April 2007,26 patients(aged 3 to 10 years) with 28 hips were operated on together with soft tissue solution,a pericapsular osteotomy of the ilium (Pemberton) and an optional femoral rotational, shortening or varus osteotomy of the proximal femur. Pre-and post-operative indexes including those of AI angle,CE angle and deep of acetabulum were compared. According to Severin's classification and McKay clinical curative effect evaluation critera, these indexes were evaluated.[Results]According to the Severin's classification, the results were rated as 64.3% excellent,28.6% good and 7.1% fair.On the McKay's critera ,the functional results were graded as 50.0% excellent,35.8% good,7.1% fair and 7.1% poor. On the whole, the radiographic evaluation was superior to that of the functional results.[Conclusion]The Pemberton's osteotomy is safe and effective. The combined surgical treatment can improve the success rate and decrease complications.

Objective To establish content determination of hydroxyl safflower yellow A in Biyangqing. Methods Hhgh-performance liquid chromatography(HPLC)was used in the determination.A C18 column was used for the separation flow rate Was set at 1.0mL/min,the temperature of the column was set at 30℃,and wavelength of diction was set at 403 nm.with 100.08%average recovery and 0.98%RSD.Conclusion This detrmination method is specific and reproducible and can be used to control the quality of Biyangqing.

Objective To estimate the prevalence of iron deficiency and its influencing factors in 6 to 12-month-old infants, then more effective community nursing intervention of prevention against iron deficiency can be made. Methods Three hundred and thirty-one 6 to 12-month-old infants had been enrolled by convenience sampling method to complete the investigation.Variables associated with iron deficiency in bivariate analysis were evaluated in Logistic regression models. Results The prevalence rates of iron deficiency in 6 to 12-month-old infants was 34.74%.Anemia,age,feeding patterns and zinc deficiency were independently associated with increased risk for iron deficiency. Conclusions Iron deficiency was common and associated with anemia, low age, feeding patterns and zinc deficiency in a certain community in Tianjin. So effective nursing measures must be made to reduce the prevalence rates of iron deficiency in 6 to 12-month-old infants.

To explore the measures of nursing management in the treatment of large number of infants with urinary calculi. The nursing management measures included launching the preparedness and response project for sudden public health events, formulating scientific and standardized nursing management system, optimizing work flow, strengthening nurse training, focusing on the details in nursing management, implementing disinfection and isolation system seriously, and paying close attention to health education for the parents of minority infants. Scientific nursing management can ensure the treatment effectiveness and nursing safety for the infants with urinary calculi.
