1.Effects of Dexmedetomidine on the Heart Rate Variability in Patients Undergoing Lower Limbs Operations with Application of Tourniquet

Objective To evaluate the effects of dexmedetomidine on the hemodynamics and the heart rate variability(HRV)in patients under-going lower limbs operations with application of tourniquet. Methods Forty patients undergoing lower limbs operations with application of tourni-quet were randomized assigned to dexmedetomidine group(group D,n=20)or control group(group C,n=20). After combined spinal-epidural anesthesia,group D received a continuous infusion of dexmedetomidine(0.5μg/kg for 10 min for loading dose and followed by 0.2μg·kg-1·h-1) until tourniquet deflation. The control group received normal saline instead. Mean arterial pressure(MAP),heart rate(HR),saturation of pulse ox-imetry(SpO2),low frequency power(LF),high frequency power(HF)and LF to HF ratio(LF/HF)were recorded at regular time points:imme-diately before loading dose(T0),before tourniquet inflation(T1),15 min after tourniquet inflation(T2),30 min after tourniquet inflation(T3),45 min after tourniquet inflation(T4),60 min after tourniquet inflation(T5),1 min after tourniquet deflation(T6),5 min after tourniquet deflation (T7)and 10 min after tourniquet deflation(T8). Results Compared with T0,the MAP of group D significantly decreased at T6-T8(P<0.05). Compared with T0 and T1,the MAP of group C increased at T2-T5(P>0.05). Compared with T2-T5,the MAP of group C significantly decreased at T6(P<0.05). Compared with group C,the MAP of group D significantly decreased at T6 and T7(P<0.05). Compared with T0,the HR of group D significantly decreased at T1-T8(P<0.05). Compared with T0,the HR of group C had no significant change at T1-T5(P<0.05). Compared with T1-T5,the HR of group D and group C significantly increased at T6(P<0.05). Compared with group C,the HR of group D significantly decreased at T1-T4 and T6(P<0.05). Compared with T0,the SpO2 of group D and group C significantly decreased at T6(P<0.05). Compared with group C,the SpO2 of group D significantly decreased at T1-T3(P<0.05). Compared with T6,LF of group D and group C significantly increased at T7(P<0.05). LF were comparable between groups D and C(P>0.05). Compared with T0,the HF of group D significantly increased and the LF/HF of group D significantly decreased at T1-T4(P<0.05). Compared with group C,the HF of group D significantly increased and the LF/HF of group D significantly decreased at T1-T4(P<0.05). Conclusion The appropriate dose of dexmedetomidine(loading dose 0.5μg/kg and maintenance dose 0.2μg · kg-1 · h-1)can significantly increase vagal tone and improve cardiac sympathetic and parasympathetic balance during tourniquet appli-cation.

Objective To investigate the feasibility of MR angiography of the dorsalis pedis artery (DPA),the first dorsal metatarsal artery (FDMA) and the branches at the first toe web by threedimensional time-resolved imaging of contrast kinetics (TRICKS) sequence.Methods Forty three patients with suspected or known soft tissue diseases of the ankle and foot were examined retrospectively by MR TRICKS sequence.Two experienced radiologists independently evaluated the visualization performance of DPA,FDMA and its branches with maximum intensity projection.Kappa analysis was performed for the image evaluation of the two radiologists.Consensus scores were obtained if the two radiologists had different scores.Clinical classification of FDMA was carried out for patients with scores equal to,or more than 2 points after imaging evaluation.FDMA was categorized according to its location (superficial,intramuscular,infra-muscular,absent),diameter (large,medium and small) and branching pattern at the toe web (ramifying type,main trunk type and fine small branch).Results The scoring results of the two radiologists indicated a high agreement (Kappa value =0.895,P < 0.05).TRICKS images can clearly show the arterial filling of DPA,FDMA and its branches.The final consensus scores were as follows:8 patients had 4 grade and 22 patients 3 grade,8 patients 2 grade and 5 patients 1 grade.Clinical classification of FDMA for 38 patients(arterial scales ≥2 point):(1) Location:superficial (8 patients),intramuscular (23 patients),intramuscular (7 patients) ; (2) Diameter at the midpoint of FDMA:large (2 patients),medium (25 patients),and small (11 patients); (3)Branching pattern at the toe web:ramifying type (11 patients),main trunk type (5 patients),fine branch (14 patients).Conclusion MR TRICKS sequences are valuable in the evaluation of the DPA and FDMA and its branches,which can provide useful anatomical information for classification of FDMA.

Objective To investigate the incidence and associated factors of pre-eclampsia in women with abnormal glucose metabolism in pregnancy. Methods A retrospective study was conducted on 1202 pregnant women with abnormal glucose metabolism who delivered their babies in our hospital between 1981-2003. All women were divided into 2 groups: group Ⅰ included 151 women with pre-eclampsia; group Ⅱ consisted of 1050 women without pre-eclampsia. The risk factors of pre-eclampsia were analyzed. 6. Antipyretic and anti-inflammatory effects of Mosla chinensis based on integration processing technology of origin

Objective To compare the effect of integration processing technology of origin with traditional cutting processing technology of Mosla chinensis on pyretic rats caused by LPS and inflammatory RAW264.7 cells induced by LPS, and provide the feasible basis for the implementation of integration processing technology of origin and traditional efficacy of the relieving exterior effects of M. chinensis. Methods The rats were intervened by M. chinensis (volatile oil, decoction), and the model of heat syndrome was established by ip of LPS. The temperature changes of the fever rats were observed. The levels of serum prostaglandin E2 (PGE2), tumor necrosis factor-α (TNF-α), adenosine cyclic phosphate (cAMP), interleukin-1β (IL-1β), and myeloperoxidase (MPO) in liver tissue were detected. The effects of integration processing of origin and traditional cutting processing of M. chinensis on heat syndrome rats were compared. The cell viability of LPS stimulated RAW264.7 macrophages cells after administration of different concentration of M. chinensis (decoction and volatile oil) was detected, and the NO secretion was determinated by ELISA for comparing the anti-inflammatory effect of two processing methods. Results The treatment of processed M. chinensis can decrease the anus temperature and the content of PGE2, TNF-α, CAMP, IL-1β, MPO, and enhance cellular immunity. Moreover the efficacy of integration processing of origin is better than traditional cutting processing, and the efficacy of M. chinensis decoction is better than volatile oil. Conclusion The integration processing technology of origin for M. chinensis is feasible.

The effect of vascular endothelial growth factor (VEGF) overexpression on matrix metalloproteinase-2 (MMP-2) in nasopharyngeal carcinoma (NPC) cells in vitro and the possible mechanism involved were investigated, and the correlation between the expression of VEGF and MMP-2 in NPC evaluated. The NPC cells were transfected with PAd-trackVEGF165 plasmid. The expression levels of VEGF and MMP-2 mRNA and protein in NPC cells were detected by semi-quantitative RT-PCR and Western blot respectively. It was found that the expression of VEGF and MMP-2 mRNA and protein was significantly increased in NPC cells after transfection of VEGF165. It was concluded that the expression of VEGF was correlated to the in vitro invasion of NPC cells, and the induction of MMP-2 by VEGF was a key process of NPC cell invasion.

10.Analysis of cognitive function and its related factors in patients with essential epilepsy

Objective To observe the cognitive function and its related factors in patients with essential epilepsy.Methods The cognitive function of 70 essential epilepsy patients (epilepsy group) and 40 healthy controls(control group) were evaluated by means of Wechsler Intelligence Scale for Adult-Chinese (WAIS-RC).The relation between the cognitive dysfunction and its related factors were analyzed.Results The scores of performance intelligence quotient,verbal intelligence quotient,full intelligence quotient in epilepsy group were significantly lower than those in control group [(98.06 ± 15.24) scores vs.(113.80 ± 12.14) scores,(98.09 ± 16.06) scores vs.(120.65 ± 11.28) scores,(98.06 ± 15.80) scores vs.(119.42 ± 11.85) scores] (P < 0.01).The scores of 11 numbers of rating scales were significantly lower than those in control group (P < 0.01).The factors related to the cognitive function were education level,age numbers,duration of the disease,frequency of seizures attack before medication,the duration of seizures and the quantity of antiepileptics.Conclusions Many of the essential epilepsy patients have cognitive function deficit.The cognitive condition of essential epilepsy patients should be pay more attention and reduce the dangerous factors in order to improve the life quality.
