2.Detecting plasma methylated Septin9 gene combined with fecal immunochemical test in screening colorectal cancer and adenoma in outpatients

Objective To evaluate the significance of detecting plasma methylated Septin9 (SEPT9) alone and combined with fecal immunochemical test (FIT) in screening colorectal cancer (CRC) and adenoma.Methods From October 2014 to April 2015,outpatients received CRC and adenoma screening were enrolled.Colonscopy examination and pathological diagnosis were taken as gold standard.The sensitivity and specificity of plasma methylated SEPT9 and FIT in CRC and adenoma were calculated.The diagnostic value of combined examination was evaluated.Receiver operating characteristic (ROC) curve of SEPT9 in CRC diagnosis was drawn.Results A total of 300 outpatients were enrolled.Among them,CRC was detected in 45 patients (15.0%) and adenoma was detected in 68(22.7%) patients,including 37(12.3%) cases of advanced adenomas.The sensitivity of SEPT9 and FIT for CRC diagnosis was 80.0% and 88.9%,and the specificity was 95.3 % and 54.1%,respectively.The area under concentration curve (AUC) of methylated SEPT9 in CRC diagnosis was 0.923.The sensitivity of SEPT9 and FIT in advanced adenoma diagnosis were 10.8% and 62.2%,and the specificity were 83.3% and 49.0%,respectively.The sensitivity of combined examination in CRC diagnosis was 97.8% and the specificity was 52.9%;the sensitivity in advanced adenoma diagnosis was 67.6% and the specificity was 47.4%.Conclusions Plasma methylated SEPT9 is helpful in CRC screening,moreover,when combined with FIT,it can increase detection rate of colorectal adenoma.
3.Efficacy of splenectomy plus selective pericardial devascularization under en-doscope in treatment of advanced schistosomiasis patients with portal hyper-tension and hypersplenism

Objective To evaluate the efficacy of splenectomy plus selective pericardial devascularization under endoscope in the treatment of advanced schistosomiasis patients with portal hypertension and hypersplenism so as to explore the minimally in-vasive and safer surgical treatment. Methods A secure splenectomy was performed with laparoscope and its supporting devices, and at the same time,the ligation of the left gastric vein and the ligation of esophageal vein perforating vertically into the esopha-gus were also performed in 14 advanced schistosomiasis patients with portal hypertension and hypersplenism. Results Among the 14 patients,the splenic artery was separated and clipped before the treatment of splenic pedicle. One patient was of conversion to open laparotomy for the splenic vein rupture bleeding in the separation. There was no death. Conclusion The operation of sple-nectomy plus selective pericardial devascularization under endoscope is effective,truly minimally invasive,and safe in the treat-ment of advanced schistosomiasis patients with portal hypertension and hypersplenism.
4.Expression and significance of vascular endothelial growth factor and transforming growth factor β1 in hepatic fibrosis

Objective To explore the expression and significance of vascular endothelial growth factor (VEGF) and transforming growth factor β1 (TGF-β1) in hepatic fibrosis.Methods Fifty-six cases of patients with hepatic fibrosis were selected as observation group and 50 healthy persons as control group.Immunohistochemistry were performed to detect VEGF and TGF-β1 in two groups.Results Serum VEGF and TGF-β1 in observation group were significantly higher than those in control group(VEGF:(110.87 ±32.64) μg/L vs (15.98 ±6.75) μg/L,t =20.166,P <0.001;TGF-β1:(15.08 ±4.27) ng/L vs (7.17 ±2.86) ng/L,t =11.066,P < 0.001) ;There were significant differences on VEGF and TGF-β1 level among S1,S2,S3 and S4 subgroups(VEGF:(84.25 ±16.86) μg/L vs (101.87 ±36.70) μg/L vs (118.04 ±40.75)μg/L vs (134.65 ± 45.73) μg/L,F =15.689,P =0.015 ; TGF-β1:(10.87 ± 2.64) ng/L vs (13.06 ± 2.74)ng/L vs (17.87 ± 3.28) ng/L vs (22.76 ± 4.75) ng/L,F =12.438,P =0.026).VEGF had positive correlation with TGF-β1 (r =0.532,P =0.013).Conclusion VEGF and TGF-β1 level have close relationship with the occurrence and development of hepatic fibrosis.Combined detection of VEGF and TGF-β1 can be serum index for diagnosis and evaluation disease condition.
5.Utilization of Antineoplastic Drugs in Our Hospital During the Period 2005~2007

OBJECTIVE:To investigate the utilization of antineoplastic drugs in our hospital. METHODS:A retrospective analysis was conducted on antineoplastics used during 2005-2007 in our hospital in respect of the consumption sum,DDDs and average daily cost etc. RESULTS:Over the 3 years,the proportion of antineoplastics showed a year-on-year increase in consumption sum,with the antitumor plant amedica topping the consumption sum list and Tamoxifen topping the DDDs list for three years. Domestic drugs,injections and drugs on the RDL (reimbursable drugs list for basic medical insurance) showed high DDDs in our hospital. CONCLUSION:The consumption of antineoplastics is basically rational in our hospital,yet its management remains to be further tightened.
6.TLR2McAb and TLR4McAb decrease inflammatory cytokines expression and increase counts of Lactobacilli and Bifidobacteria in dextran sulfate sodium-induced colitis in mice

Objective To evaluate the effect of TLR2McAb and TLR4McAb on intestinal flora of DSS-induced colitis in mice. Methods Fifty healthy male BALB/c mice (SPF level), were randomly assigned into five groups: the control group( group A), the UC model group( group B), TLR2McAb intervention group( group C), TLR4McAb intervention group( group D) and TLR2McAb + TLR4McAb intervention group(group E). Clinical symptoms were evaluated by the disease activity index(DAI), while tissue sam ples were evaluated by histological scoring(HS). The quantities of mRNA for IFN-γ, IL-4 and IL-17 were determined by real-time PCR. Meanwhile, fecal samples were obtained directly from the cecum for microbiological studies. Results After the treatment with TLR2McAb and TLR4McAb, DAI and HS were decreased significantly. Compared with group A, inflammatory cytokines such as IFN-γ, IL-4 and IL-17 in group B were higher. Compared with group B, expression of these three cytokines in group C to E was all markedly decreased. Group A showed a considerable predominance of Lactobacillus spp and Bifidobacterium spp,while the UC model group showed a conspicuous increase of Escherichia coli and decreases of Lactobacillus spp and Bifidobacterium spp. After treatment with TLR2McAb or/and TLR4McAb, Lactobacillus spp and Bifidobacterium spp increased to the normal level. But counts of E. Coli in the three intervention groups were not changed. Conclusion TLR2McAb and TLR4McAb suppressed the development of DSS-induced colitis and increase cecum counts of Lactobacilli and Bifidobacteria.

8.The in vitro dissolution of total composition of the tablet of rhizomes of Ligusticum chuanxiong components and in vitro-in vivo correlation by the method of area under the absorbance-wavelength curve.

To discuss the availability of evaluation on the dissolution studies of the multicomponents in traditional Chinese medicine, the in vitro dissolution of total composition of the tablet of rhizomes of Ligusticum chuanxiong components and its correlation with the in vivo were studied by the method of area under the absorbance-wavelength curve (AUAWC). Taken the tablet of rhizomes of Ligusticum chuanxiong components which is composed of sodium ferulate and ligustrazine hydrochloride as subject model, the dissolution tests were carried out with basket method. The plasma concentrations of tablets in different rats were determined by AUAWC at different interval times. The in vivo absorption percentage was calculated by Wagner-Nelson equation to evaluate the in vitro and in vivo correlation. According to the results, the cumulative dissolution in vitro of total composition of tablets of rhizomes of Ligusticum chuanxiong components at 60 min was 90.65% in water by AUAWC. The in vivo pharmacokinetics is fitted with an one-compartment model. The linear equation based on the cumulative dissolution rate (fr) and absorption percentage (fa) at 5, 10, 20, 30 and 60 min was fa = 0.819 7 fr+0.183 and the correlation coefficient was 0.959 5, which showed a good correlation between the in vitro dissolution and the in vivo absorption percentage. The method of AUAWC can be used accurately, feasibly and conveniently to evaluate the in vitro and in vivo correlation of total composition of tablets of rhizomes of Ligusticum chuanxiong components, which will provide better guidance to study the in vitro and in vivo correlation of sustained release preparation etc under complex system of traditional Chinese medicine in the future.

9.Effects of flavones extracted from Portulaca oleracea on ability of hypoxia tolerance in mice and its mechanism

OBJECTIVE: To identify anti-hypoxia ingredients extracted from Portulaca oleracea and to find out the possible mechanism of its anti-hypoxia actions. METHODS: Seventy mice were randomly divided into seven groups which were untreated (normal saline), ginsenosides-treated, polysaccharide-treated, acidic components-treated, basic components-treated, alkaloids-treated and flavones-treated groups, and the ingredients of polysaccharide, acidic components, basic components, alkaloids and flavones were extracted from Portulaca olerace. The mice in each group were fed with corresponding ingredients for one week respectively. Then the survival time of mice in hypoxic conditions was observed. Another 90 mice were divided into 3 groups: untreated (normal saline), ginsenosides-treated and flavones-treated groups. The mice in each of these 3 groups were divided into 3 subgroups according to 12-, 24- and 36-hour exposure to hypoxia (10% oxygen and 90% nitrogen), respectively. After exposure to hypoxia, the red blood cell count (RBC), hemoglobin (Hb) concentration and hematocrit (HCT) in mice were determined. The plasma erythropoietin (EPO) levels of mice were detected by enzyme-linked immunosorbent assay (ELISA) and the relative values of EPO mRNA in renal tissue and pallium of mice were determined by reverse transcriptase-polymerase chain reaction (RT-PCR). RESULTS: The survival time of mice in hypoxic conditions in flavones-treated group was significantly longer than that in the untreated group. The RBC, Hb concentration, HCT, plasma EPO level and the relative values of EPO mRNA in renal tissue and pallium of mice were significantly higher in the flavones-treated group than those in the untreated group. CONCLUSION: The anti-hypoxia ingredients extracted from Portulaca oleracea are flavones and the anti-hypoxia effects may be obtained by improving the expression level of EPO and accelerating the generations of erythrocyte and Hb.
10.The Influence of Hypnotherapy in Patients with Depression and Their Families

Objective To investigate the influence of hypnotherapy in patients with depression and their families. Methods One hundred and thirty-six cases of hospitalization or outpatient patients with depression were divided into the study group and the control group according to the order. The study group and the control group were treated with hypnosis therapy and venlafaxine, respectively. All patients were observed for 6 weeks. Hamilton Anxiety Scale (HAMA) and Hamilton Depression Scale (HAMD) were assessed for patients with depression and the Symptom Checklist (SCL-90) and family assessment table (FAD) were assessed for thir families in the 0 and 6 weeks. Results There were no significant differences between the two groups in the clinical efficacy (χ2=1.31,> 0.05) and the HAMD, HAMA scores in the 0 and 6 weeks ( >0.05) . There were significant differences between the 0 and 6 weeks within the two groups in the HAMD and HAMA scores ( < 0.05). There was no significant difference between the family members of two groups in the SCL-90 factor scores in the 0 and 6 weeks (>0.05) . There was no significant difference between the two groups in the FAD factor scores in the 0 and 6 weeks. There was significant difference between the 0 and 6 weeks within the two groups with emotional involvement, communication and FAD total scores ( <0.05). There were positiue correlations between the HAMA factor scores and FAD total scores ( <0.05) and between the HAMD factor scores and emotional involvement scores ( <0.05) . Conclusion There was equivalent efficacy between hypnotherapy and antidepressants in patients with depression. Hypnotherapy can effectively make the patient's depression and anxiety better and improve their family functions.
