1.Clinical Observation of Fludarabine、 Intermediate-dose Ara-C and G-CSF in Treatment for Refractory or Relapsed Acute Myeloid Leukemia(26 Cases)

Objective To observe the therapeutic effects and side effects of FLAG regimen(fludarabine, Ara-C, and granulocyte-colony stimulating factor( C-CSF) for refractory or relapsed acute myeloid leukemia( AML). Methods Twenty-six AML were treated with fludarabine plus intermediate-dose Ara-C and G-CSF,of whom 15 cases belonged to refractory and 11 cases belonged to relapsed. Results After two courses of treatment, 14 cases were completely relieved (53. 8% ) and 5 cases were partially relieved (19.2% ). The overall effective rates was 73.1%. The main side effects were severe myelosuppression and non-hematological toxicity was mild. Conclusion FLAG regimen was very effective for refractory or relapsed acute myeloid leukemia and was well tolerated. The treatment-related mortality rate was low,so it provided a treatment choice for these patients.
2.Effect of different doses of atorvastatin on chronic heart failure in patients with coronary heart disease

Objective To study the effect of different doses of atorvastatin in the treatment of coronary heart disease with chronic heart failure. Methods 100 patients with chronic heart failure who were admitted to Ankang Hospital from April 2013 to April 2016 were selected and randomly divided into the experimental group and the control group with a total of 50 patients in each group. Two groups of patients were treated with conventional treatment, the control group was given 20 mg of calcium a day, the experimental group was given a daily dose of 40 mg of calcium. The therapeutic effects of the two groups were compared. Results After treatment, the left ventricular end diastolic diameter in the control group was significantly higher than that in the experimental group, the left ventricular ejection fraction and E/A were significantly lower than those in the experimental group, and the difference was statistically significant (P<0.05). Patients in the control group were significantly higher than those in the experimental group in the incidence of cardiac death and recurrence of heart failure (P<0.05). Before treatment, there was no significant difference between the experimental group and the control group in serum NT-proBNP, hs-CRP levels and 6MWT. After treatment, the serum levels of NT-proBNP, hs-CRP in the control group were significantly higher than those in the experimental group, 6MWT was significantly shorter than that in the experimental group, and the difference was statistically significant (proBNP) (P<0.05). Conclusion Compared with 20mg/d, 40mg/d dose atorvastatin in the treatment of coronary heart disease with chronic heart failure curative effect, low recurrence rate, high safety, worthy of further promotion in clinical.
3.Preparation and in vitro Transdermal Studies of Dexketoprofen Trometamol Hydrogel Patches

Objective: To prepare dexketoprofen trometamol hydrogel patches, optimize the formula and evaluate in vitro transdermal properties.Methods: Dexketoprofen trometamol hydrogel patches were prepared with NP-800 as the hydrogel patch carrier, aluminum hydrochloride as the crosslinking agent, EDTA as the crosslinking modifier and glycerol as the moisturizing agent.The formula was screened by orthogonal design with the initial viscosity, holding force, peel strength and 12 h cumulative transdermal quantity as the evaluation indices to screen out the best formula.The transdermal absorption test was carried out with an improved Franz diffusion cells to compare the enhancement of Aznoe, oleic acid and menthanol on dexketoprofen trometamol hydrogel patches.Results: The best formula was as follows: the mass percentage of NP-800, glycerol, glycerol and EDTA was 5%, 0.3% , 25% and 0.15% , respectively.The transdermal enhancers had transdermal enhancement on dexketoprofen trometamol, and among them, 3% Azone had the most significant enhancement with the enhancing rate of 3.26.Conclusion: The preparation and formula of dextroxyprofen trometamol hydrogel patches are stable, reasonable and feasible.
4.Clinical significance analysis of radical operation for gastric cancer in elderly patients

Objective To analyze the therapeutic effect of the radical operation for gastric cancer in elderly patients and to investigate relevant factors affecting the efficacy Methods 216 cases of elderly patients with gastric cancer were divided into two groups:radical distal-and proximal-gastrectomy group (n=108,each).The therapeutic effect in the near future,intraoperative and postoperative recovery,numbers of lymph nodes resection as well as factors affecting the long-term survival rate were compared between two groups.Results The radical distal-versus proximal-gastrectomy group showed that the mean operation time was (241.8 ± 40.2) min vs.(244.7 ± 39.7)min,blood loss was(299.5 ± 114.9) ml vs.(273.8-± 121.4) ml,the number of lymph node dissection was(28.4± 9.4) vs.(27.7 ± 9.1) particles,the complication rate was[14/108 (13.0%)] vs.[15/108 (14.0 %)],3-year survival rate was (46.9 %) vs.(47.5 %),without statistically significant differences (all P > 0.05).Multiple regression analysis showed that TNM stage (OR =1.536),lymph node metastasis(OR =0.739),cancer infiltration depth (OR =1.534) and chemotherapy regimens (OR =1.337)are risk factors for the postoperative 3-year survival rate.Conclusions Surgical treatment of elderly patients with gastric cancer is significantly effective,TNM stage,lymph node metastasis,depth of invasion,tumor size and the chemotherapy regimens are closely associated with the life quality of patients.

7.Expression of Survivin Gene and Its Correlation with VEGF Expression in Colorectal Adenocarcinoma

Objective To investigate the role of Survivin gene in the pathogenesis of colorectal adenocarcinoma and its correlation with VEGF in tumor metastasis. Methods The expressions of Survivin mRNA and VEGF protein were detected by RT-PCR and immunohistochemical staining, respectively, in 42 colorectal adenocarcinoma samples. Results Survivin mRNA expression was positive in 23 cancer samples(54 8%), while no expression was detected in adjacent normal tissue. The expression of Survivin gene significantly correlated with histological differentiation and Dukes stage, but not with gender or lymph node metastasis. The expression of VEGF significantly correlated with lymph node metastasis. The expressions of Survivin and VEGF were coincident. The colorectal adenocarcinomas which expressed both the Survivin and VEGF were more inclined to occur lymph node metastasis. Conclusions Survivin gene expression significantly correlated with the clinico-pathologic characteristics and biologic behavior in colorectal adenocarcinoma. Detection of Survivin expression may be valuable for diagnosing colorectal adenocarcinoma, and evaluating malignancy extent and prognosis.

10.Clinical manifestations and esophageal motility characteristics of 180 patients with gastroesophageal reflux disease and extra-esophageal symptoms

Objective:To analyze the clinical manifestations and esophageal motility characteristics of patients with gastroesophageal reflux disease (GERD) and extra-esophageal symptoms.Methods:From January 1 to October 30, 2018, at PLA Rocket Force Characteristic Medical Center, 180 hospitalized patients diagnosed with GERD and extra-esophageal symptoms were retrospectively analyzed. The patients were divided into laryngopharyngeal symptom group (65 cases), airway symptom group (58 cases) and mixed symptom group (57 cases). General data, clinical symptoms, gastroscopic manitestations, the results of high-resolution esophageal manometry and 24-hour multichannel intraluminal impedance and pH monitoring of each group were analyzed and compared. Chi-square test and one-way analysis of variance were used for statistical analysis.Results:The patients aged <40, 40 to 60 and >60 years accounted for 12.8% (23/180), 53.3% (96/180) and 33.9% (61/180), respectively, and the difference was statistically significant ( χ2=12.030, P=0.017). There were 18.9%(34/180) of patients without typical reflux symptoms. There were statistically significant differences in the incidence of ectopic esophagogastric mucosa or Barrett esophagus under gastroscopy between laryngopharyngeal symptom group, airway symptom group and mixed symptom group (21.5%, 14/65; 5.2%, 3/58 and 8.8%, 5/57, respectively) ( χ2=8.578, P=0.014). There were no statistically significant differences in the lower esophageal sphincter pressure (LESP), upper esophageal sphincter pressure or distal contractile integral between laryngopharyngeal symptom group, airway symptom group and mixed symptom group ((8.57±0.76), (8.87±0.79), and (10.51±0.97) mmHg (1 mmHg=0.133 kPa); (44.75±2.86), (42.81±4.06), and (39.14±3.20) mmHg; (506.13±64.30), (432.59±78.10), and (682.99±82.28) mmHg·s·cm)(all P>0.05). The DeMeester score of laryngopharyngeal symptom group , mixed symptom group and airway symptom group was (14.33±2.09), (21.94±5.30) and (30.47±5.85) points, respectively, and the difference was statistically significant ( F=3.226, P=0.043). The results of multi-channel impedance monitoring showed that acid reflux and weak acid reflux were the main reflux in the patients, which accounted for 55.5% (76/137) and 34.3% (47/137), respectively. Among 87.6% (120/137) of the patients, reflux mainly occurred in the upright position while 12.4% (17/137) of the patients had reflux in the supine position. Conclusions:The extra-esophageal symptoms of GERD is associated with age. Ectopic esophagogastric mucosa or Barrett esophagus are more common in GERD patients with laryngopharyngeal symptoms. There are more acid exposure and pathologic acid reflux in GERD mainly with airway symptoms. Weak acid reflux at upright position plays an important role in the reflux mechanism of GERD with extra-esophageal symptoms.
