Objective To investigate reconstructive repair methods of a large scalp defect with the granulation tissue wounds and skull exposure caused by the trauma. Methods Skin and soft tissue expansion technique was used to repair eight patients with a large scalp defect with the granulation tissue wounds and skull exposure caused by the trauma. The skin and soft tissue expanders were embedded under normal epicranial aponeurosis after the formation of fresh granulation tissue wound.Strict aseptic technique as well as water injection was done in the expansion process and moderate expansion to maintain rich blood circulation in the expansive parts. Results 12 skin and soft tissue expanders were implanted in 8 patients and the scalp wounds were completely repaired. No infection was detected after surgery and injection expansion process. Conclusions The skin and soft tissue expansion can be used to reconstruct post-traumatic scalp defect with granulation tissue wound and skull exposure.

OBJECTIVETo investigate the clinical effect of steroids on reducing postoperative edema in rhinoplasty.
METHODSCochrane, Medline data, Pubmed date, were searched and updated on October 2013. Randomized controlled trials(RCTS) studies were included to assess the efficacy of steroids on decreasing postoperative edema after rhinoplasty. The methodological quality of the included studies was evaluated, and date analyses were performed using the Cochrane Collaboration's software RevMan 5.2.
RESULTSA total of 4 RCTS involved 172 patients with rhinoplasty, including 87 patients in the experimental group( steroid) and 85 paitents in control group (placebo). Meta analysis results showed the edema in experimental group was significantly less than that in the control group on postoperative day 1 and 3 (P < 0.01), while the difference was not significant on postoperative day 7 (P = 0.19).
CONCLUSIONSPerioperative application of steroid in rhinoplasty can significantly reduce periorbital edema in the first postoperative day. The edema can completely be relieved after application of steroid for 3 days. It is a safe and effective way to reduce the postoperative edema.

Objective To explore the clinical efficacy of early surgery combined with electron linac therapy on keloid. Methods The keloid patients with stable phase were selected;complete resection of keloid and relaxation suture were performed;after the surgery within 24 hours 6 MeV with Varian 2300CD radiotherapy was given, each measuring 4 Gy,total dose of 20 Gy.Results 860 cases of patients were colected for a period of 3 months to 36 months of regular follow-up,which recovered in 802 cases (cure rate was 93.26%),effective results were observed in 41 cases (effective rate of 4.77%),including 17 cases of recurrence (recurrence rate 1.98%),the total efficiency (cure plus effective) was 98.02%.Conclusions More accurately immediate radiotherapy after surgery can effectively reduce the recurrence rate, which is a safe and effective method in the treatment of keloids.

A systematic review was undertaken, including studies that evaluated the incidence of the blood system adverse events of Tripterygium wilfordii (TWP). Medline, Embase and the Cochrane library were searched for relevant studies, including RCT, cohort studies and case series, of patients treated with TWP published in English and Chinese from inception up until May 25th, 2013 with the keywords including "Tripterygium wilfordii", "toxicity", "reproductive", "side effect", "adverse", "safety" and "tolerability". Relevant information was extracted and the incidence of the blood system adverse events was pooled with MetaAnalyst software. Besides, subgroup and sensitivity analyses were performed based on age, mode of medicine, observation time and disease system. According to inclusion and exclusion criteria, a total of 49 articles were included in the meta-analysis, they were split into 54 researches incorporated in the analysis. There is a large degree of heterogeneity among the studies, so data was analyzed using random-effects model and the summary estimates of incidence of the blood system adverse events was 6.1%. The weighted combined incidence of three major blood system adverse events were white-blood cells decreasing 5.6% (95% CI, 4.3% - 7.3%), hemoglobin decreasing 1.7% (95% CI, 0.5% - 5.0%) and platelet decreasing 1.8% (95% CI, 1.0% - 3.1%), respectively . Sensitivity analyses based on 45 studies with high quality showed the combined value was close to the summary estimate of total 54 studies. The current evidence indicates that the incidence of the blood system adverse events induced by TWP was high; attentions should be paid on to the prevention and treatment of the blood system adverse events.

Objective To investigate the event-based prospective memory (EBPM) and time-based prospective memory (TBPM) in temporal lobe epilepsy patients, and test the hypothcsis that temporal lobe is involved in the prospective memory network. Methods Sixty-two patients with temporal lobe epilepsy (TLE) and 30 age- and education level-matched healthy adults were examed with the neuropsychological battery of tests including EBPM and TRPM tasks. Results In comparison with the controls, the TLE patients were impaired on MMSE,DS and VFT.Compared with the healthy adults ( EBPM:6.83± 1.34,TBPM:5.00 ± 1.70),the patients with temporal lobe epilepsy had a significant loss in EBPM (3.95 ±2.77,t =6.72,P <0.01 ) and TBPM (3.08 ±2.42,t =4.39,P <0.01).There was no difference in the performance of EBPM (3.82 ± 2.70,4.10 ± 2.90 ; t =- 0.40,P > 0.05 ) and TBPM (2.55 ± 2.20,3.69± 2.55; t =-1.90,P >0.05 ) between the patients with taking antiepileptic drugs and those without antiepileptic drugs. Conclusions These results suggest that patients with temporal lobe epilepsy may experience general difficulties with prospective memory. It is possible that temporal lobe is involved in the prospective memory network. The passages of TBPM need more self-initiated processes.

Objective To validate a new standardized training program for urological surgeons to improve their laparoscopic surgical skills. Methods The laparoscopic surgical training program was carried out by using the traditional mechanical simulators and animal models. Thirty-three trainees participated the urological laparoscopic surgical training program. Initially, the novices were assigned to practise basic laparoscopic skills step by step on the simulator with fixed trocar positions. After a period of basic training, they were allowed to practise on animal models for some particular proce-dures. Results All trainees (33/33, 100.0%) participated were able to perform all basic techniques skillfully and completed laparoscopic anastomosis accurately after the training. The time required for performing the partial nephrectomy, dismembered pyeloplasty and ureteral reimplantation on animal models declined from 64.0±18.4, 127.54±17.5 and 75.84±11.6 min at the beginning to 30.94±3.8, 65.2±7. 5 and 37.7±7.2 min after practicing these procedures 8 times (P<0.01). They could un-derstand the crucial procedures of the laparoseopic surgeries after 6 to 8 special trainings on animal models. Fifteen trainees (15/33, 45.5%) had started to carry out laparoscopic surgeries after-finish- ing the training program. Conclusions Our program enables the participants to improve their techniques in complicated laparoscopic surgeries. The challenging parts of reconstructive laparoscopic surgeries such as laparoscopic dismembered pyeloplasty can be taught by using animal models. This program could be incorpo-rated easily by all urological departments developing a laparoscopie surgical training program.
