Objective Rheological properties and gelation properties of agar were investigated. Methods The gelling point, melting point and the gel strength of agar were detected with MCR101 rheometer and TA texture testing instrument. Results and Conclusion Rheological properties of agar were affected by its concentration ,temperature and the addition of salt (such as NaCl ,CaCl2) and sucrose. Apparent viscosity exhibited shear thinning behavior following the power law model. Apparent viscosity increased with the increase of concentration, and decreased with the rise of temperature. The decrease in viscosity followed an Arrhenius temperature dependence. Agar solutions exhibited typical "weak gel" properties by small strain oscillatory measurements. The results indicated that the agar solution was characterized as a gel properties ,and which could form a kind of heat reversible gel. The gelling point of agar was lower than its melting point. The gel strength of agar could be affected by its gel time, and the addition of salt (such as NaCl,CaCl2) and sucrose.

Objective To explore the application of single-person operated mammotome minimally inva-sive surgery in treatment of benign breast lesions. Methods Clinical information of 403 patients were collected and retrospectively analyzed. 262 patients receiving ultrasound guided single-person operated mammotome mini-mally invasive surgery for benign breast masses were selected as the treatment group,141 patients undergoing open surgery for treatment of benign breast masses were selected as the control group.Operative time, introopera-tive blood loss, length of stay, postoperative healing time, as well as the occurrence of complications were com-pared between the two groups. Result Compared with the control group, the operative time was shorter, the in traoperative blood loss was less, the scar size and the postoperative healing time were shorter for the treatment group. The difference had statistical significance (P<0.05).The rate of postoperative pain, local ecchymosis, local infection and breast deformity for the treatment group was significantly lower than those of the control group (P<0.05). The difference had statistical significance (P<0.05). The recurrence rate between the two groups had no sta-tistical significance (P>0.05). Conclusions Single-person operated Mammotome minimally invasive surgery has the advantages of good operative time, good cosmetic effects, small surgical trauma, less blood loss, fast postoper-ative healing and few complications, which is worthy of clinical application.

BACKGROUND:Under co-culture conditions, mesenchymal stem cel s could regulate osteogenic differentiation and osteogenesis of osteoblasts. OBJECTIVE:To observe the osteogenic efficiency of osteoblastic precursor cel s co-cultured with undifferentiated bone marrow-derived mesenchymal stem cel s, umbilical cord-derived mesenchymal stem cel s, or placenta-derived mesenchymal stem cel s in mineralization medium. METHODS:Adipose-derived stem cel s were induced in osteogenic differentiation medium for 7 days before being indirectly co-cultured with undifferentiated mesenchymal stem cel s isolated from different tissues (bone marrow group, umbilical cord group and placenta group) in Transwel plates. Induced adipose-derived stem cel s cultured alone served as control group. At different experimental intervals, quantitative analysis of alkaline phosphatase activity and calcified matrix was preformed to observe the effects of mesenchymal stem cel s from different sources on the osteogenic efficiency of induced adipose-derived stem cel s. RESULTS AND CONCLUSION:Expression of alkaline phosphatase was significantly higher in different experimental groups than the control group (P<0.05), and it was also higher in the bone marrow group than the umbilical cord and placenta groups (P<0.05). Quantitative analysis of calcified matrix revealed that the experimental groups were significantly higher than the control group (P<0.05);and in experimental groups, the umbilical cord group was higher than bone marrow group and placenta group (P<0.05). These findings indicate that the osteogenic efficiency of induced adipose-derived stem cel s is improved dramatical y under co-culture conditions.

Objective To investigate the effect of endoscopic management of upper tract urothelial carcinoma (UTUC),and evaluate the indication and clinical value of postoperative prophylactic pelviureteric instillation chemotherapy. Methods Patients who met the inclusion criteria included elderly patients, patients who could not tolerate radical surgery, patients with renal insufficiency or needed dialysis after removal of the kidney, patients with bilateral UTUC,patients with tumor≥3 cm in diameter but could be completely resectted under ureteroscopy who required to preserve renal function.Surgical procedure was successful. The ureteral stents were placed, and prophylactic ureteral infusion chemotherapy via the retrograde transvesical ureteric catheterisation was conducted 1-2 times one week at 2 days after surgery.40 milligram of pirarubicin hydrochloride or epirubicin hydrochloride was dissolved in 40 milliliter sterile water for the prophylactic infusion chemotherapy. These instillations were completed within 40 minutes. Results Totally,9 patients (3 males and 6 females) aged 60-86 years, mean age of (69.7±6.4) years, were enrolled in this study. The carcinoma of the renal pelvis was found in 2 cases and ureteral tumors in 7 cases. In pathological results, ureteral polyps was found in 1 case, papillary ureteral neoplasm with low malignant potential in 2 cases, urinary tract epithelial carcinoma in 4 cases (1 case with poorlydifferentiation,1 case with well-differentiation,2 cases with urinary tract epithelial cancer),and 2 cases were lack of pathological reports because of too few pathologic specimens.4 patients received adjuvant chemotherapy by instillation successfully, and lumbago, fever or other complications were not found.Patients were followed up for a mean period of 15 moths (4-31 months).Only 1 patient had recurrence of bilateral ureteral tumor 15 months after surgery, and no tumor recurrence was found in other patients. Conclusions Endoscopic management is a safe and effective in treating UTUC,which can preserve renal function by avoiding nephrorectomy in some patients. The instillation chemotherapy after endoscopic management is safe, which has a satisfactory effect, but further validation in a large clinical sample is needed.Patients with tumor resection under ureteroscopy who cannot obtain the pathology results need to be closely followed up.

Objective To investigate the clinical features of connective tissue diseases (CTD) related organizing pneumonia (OP). Methods The clinical and radiological features, therapy and outcome of 57 CTD-OP patients diagnosed by biopsy were retrospectively analyzed. T test, x2 test, Pearson/Spearmanˊs correla-tion analysis were used for statistical analysis with software version 15.0. Results CTD-OP accounted for 44% of all the OP. There were 36 cases (63%) with the initial clinical manifestation of OP in CTD-OP patients. The underling primary disease of CTDs included Sj?grenˊs syndrome (22, 38%), polymyositis/dermatomyositis (13, 23%), RA (13, 23%) and undifferentiated connective tissue diseases (9, 16%). At baseline, the clinical symptoms of cough and malaise [54% vs 83%, x2=7.749, P=0.005; 49% vs 71%, x2=4.007, P=0.045] were significantly lower in the CTD-OP patients than in the COP patients. The signs of moist rales and crackles (54% vs 32%, x2=4.160, P=0.041; 49% vs 27%, x2=4.529, P=0.033) were more common in the CTD-OP patients than in the COP patients. The TLC%, FVC% of the lung function reduced significantly in CTD-OP patients than in COP patients [(72 ±18)% vs (97 ±29)%, t=-2.475, P=0.027;(75 ±26)% vs (96 ±23)%, t=-2.174, P=0.039). The dosage of corticosteroids [(44 ±14) mg/d vs (37 ±10) mg/d, t=2.541, P=0.013] were significantly higher in the CTD-OP patients than in the COP patients. The complete recovery rate was slightly lower in CTD-OP than in COP (22% vs 29%, x2=0.384, P=0.483) with a tendency towards higher recurrence rate in CTD-OP (33% vs 14%, x2=4.007, P=3.958). Conclusion CTD-OP is probably the common cause of OP. OP can be the initial manifestation of CTD. The primary disease of the CTD-OP includes Sjogrenˊs syndrome, polymyositis/dermatomyositis, rheumatoid arthritis and undifferentiated connective tissue diseases. Compared with COP, CTD-OP patients may have milder clinical symptoms, but the more severe damage of the lung, the higher the recurrence rate, therefore, CTD-OP is worthy of more attention from rheumatologists.
