1.Analysis of Antidiabetic Agents Consumed in18Hospitals of Chongqing During the Period2000~2002

OBJECTIVE:To survey and evaluate the present situation and trends of antidiabetic drugs used in18hospitals of Chongqing during the period2000~2002.METHODS:Data of antidiabetic agents used in18hospitals of Chongqing were collected and analysed concerning the consumptive sum and main kinds.RESULTS:The consumption sums of antidiabetic a?gents in the years2000,2001and2002were498,643and649tenthousand yuans(RMB),respectively,with an annual increase rate of15.03%.The cost for insulin injections accounted for38.56%of the total consumption sum of antidiabetic agents,and that for mixed insulin took59.02%of the consumption sum for insulin.CONCLUSION:Antidiabetic agent was an important kind of drug with developing potential.The consumption of mixed insulin injection and new kind of oral antidiabetic agents is increasing fast year by year.
2.Study of contrast enhanced ultrasound on muscle crush injury to extremity

Objective To explore imaging feature of muscle crush injury of extremity on contrast enhanced ultrasound (CEUS) by an animal experiment.Methods Twenty-eight New Zealand rabbits were anesthetized by intramuscular pentobarbital sodium (30 mg/kg).A special balloon cuff device was used to create left hind leg crush injury with a force of (18.6 kPa).23 of the 28 rabbits had crush injury of extremity and survived for 72 hours.CEUS evaluated infusion model and contrast intensity in crush region of hind leg at the 0.5,2,6,24 and 72 hour after the release of the crushing pressure.Peak intensity (PI) was analyzed by comparing with serum creatinekinase (CK) and myoglobin (MYO).Results Twenty-three rabbits presented that the area of crushed region in left hind leg was 60 cm2,accounting for 2.6%-3.0% of body surface area.In different time after decompression,the crushed regions showed rapid enhancement and the intensity was higher than that of no-crushed region.The time-intensity curves of crushed regions all appeared as fast wash-in and slow wash-out.PIs of rushed regions were higher obviously than those of nocrushed regions (P <0.001).ROC curves were employed in diagnosis value of PI for crushed region.And PIs showed positive correlation with CK and MYO and the correlation coefficient was 0.833-0.996.Conclusions CEUS presents that the micro-circulation perfusion of the crushed muscle increased obviously after the release of the crush pressure.PIs evaluated quantitatively the micro-circulation perfusion changes.It suggests a potential alternative for evaluating micro-circulation abnormality and ischemia reperfusion injury of the muscle crush injury to extremities.
3.Isoflurane induces expression of vascular endothelial growth factor through activating protein kinase C in myocardial cells.

OBJECTIVEVascular endothelial growth factor (VEGF) plays important roles in establishing collateral circulation of ischemic myocardium. This study aimed to investigate the effect of isoflurane on VEGF expression and the potential intracellular signal transduction pathway in cultured rat myocardial cells in order to further reveal the molecular mechanism of myocardial preservation of isoflurane.
METHODSPrimary myocardial cells of Sprague-Dawley rats were isolated and cultured. They were divided randomly into control group, isoflurane group, protein kinase C (PKC) inhibitor group and PKC inhibitor+isoflurane group where cells were respectively incubated without any treatment, treated by 0.5, 1.0 and 1.5 minimum alveolar concentration (MAC) of isoflurane for 6 hours, by PKC inhibitor calphostin C at a final concentration of 50 nmol/L and by 50 nmol/L calphostin C+1.0 MAC isoflurane for 6 hours. VEGF expression was detected by enzyme-linked immunosorbent assay (ELISA) and the expression levels of PKC isoforms were determined by Western immunoblotting method.
RESULTSIsoflurane increased the VEGF expression in myocardial cells in a dose-dependent way. VEGF levels were significantly higher in 1.0 and 1.5 MAC isoflurane groups than in the control group (both P < 0.01). The effect of isoflurane on upregulating VEGF expression was blocked by PKC inhibitor calphostin C (P < 0.01), but calphostin C did not alter VEGF expression (P > 0.05). Isoflurane induced the activation and translocation of PKCε. Immunoblotting analysis revealed that the immunoreactivity of PKC ε increased significantly in the membrane fractions and deceased significantly in the kytoplasm fractions for cells treated with 1.0 MAC isoflurane as compared with the untreated cells, but not of PKC-α, PKC-δ and PKC-ζ (P less than 0.01).
CONCLUSIONIsoflurane induces myocardial cells to release VEGF through activating PKC-epsilon from the endochylema to the cytomembrane, suggesting a possible novel mechanism of isoflurane protecting myocardial cells.

4.Molecular mechanism of wendan tang in prevention of lipid metabolism disorder in adult rats.

OBJECTIVETo elucidate the molecular mechanism of Wendan Tang in prevention of lipid metabolism disorder in adult rats.
METHODOn the basis of hyperlipidemia rat models, triglycerides (TG), total cholesterol (TC) in serum, activities of lipase (LA), lipoprotein lipase (LPL), hepatic lipase (HL) in liver, parts of hemogram and hepatic LDLR mRNA levels were investigated 21 days after the feeding of atherogenic diet.
RESULTWendan Tang significantly reduced the serum TG, TC and increased the activity of LPL and LA, but caused no chang in HL. The result of RT-PCR test showed that high fat and high cholesterol feeding could significantly induce the reduction of LDLR mRNA levels, while Wendan Tang could increase hepatic LDLR density.
CONCLUSIONWendan Tang can prevent disorder of lipid metabolism by regulating TC, TG, LDL-c through upregaulation of LDLR transcription level and improving antioxidant ability.

5.Flexible Graphene Platform-based Electrochemical Sensor for Sensitive Determination of Dopamine

The chemical vapor deposition grown graphene was transferred onto flexible polyethylene terephthalate surface to fabricate a graphene platform electrode ( GPE) , and gold nanoparticles ( AuNPs) were electrodeposited on GPE to form an AuNPs modified GPE ( AuNPs/GPE ) . The formation of AuNPs was confirmed by scanning electron microscopy ( SEM) , energy dispersive spectrometer ( EDS) , high-resolution transmission electron microscope ( HR-TEM) , X-ray diffraction ( XRD) and Raman spectra. On AuNPs/GPE, dopamine ( DA ) displayed a pair of well-defined redox peaks with highly enhanced peak currents compared with those on GPE. At detection potential of 0. 20 V, AuNPs/GPE sensor presented a wide linear range of 0. 1 μmol/L to 400. 0 μmol/L of DA with a detection limit of 3. 9 nmol/L (S/N=3). In addition, the proposed sensor allowed highly selective and sensitive, stable and fast amperometric sensing of DA.
6.Survey of current status and influencing factors analysis of attitude and willingness of ICU nurses towards organ donation in Chongqing

Objective To investigate the attitude and willingness of nurses from intensive care unit (ICU) in Chongqing towards organ donation and analyze the influencing factors. Methods In this study, 4 Grade 3A hospitals with organ transplantation qualifications in Chongqing were selected by cluster sampling, and 321 nurses in ICU from these 4 hospitals were chosen by convenient sampling method to complete the questionnaire survey. The scores of ICU nurses' attitude and willingness towards organ donation were calculated. Spearman's correlation analysis was adopted to analyze the correlation between the attitude and willingness to donate organs. Multiple linear regression analysis was performed to identify the influencing factors of attitude and willingness towards organ donation. Results The average score of ICU nurses' attitude toward organ donation was (3.5±0.9) and the median score of willingness to donate organs was 2 (2-3). 7.Creation of tissue engineered cartilage with internal support.

OBJECTIVETo test the hypothesis that tissue-engineered cartilage can be bioincorporated with a nonreactive, permanent endoskeletal scaffold.
METHODSChondrocytes obtained from swine articular were seeded onto polyglycolic acids(PGA) scaffold which was incorporated with high-density polyethylene (Medpor). After cultured in vitro for two weeks,the cell-scaffold construct was implanted into subcutaneous pockets on the back of nude mice. Six weeks later,the newly formed cartilage prosthesis was harvested, and a small part of sample was evaluated by gross view, histology, type II collagen immunohistochemistry and biochemistry. PGA scaffold seeded with cells as the control group.
RESULTSThe newly formed cartilage was very similar to normal cartilage in both gross view and histology, and jointed Medpor tightly. The center of control group was hollow.
CONCLUSIONThis pilot technique combining tissue engineering with a permanent success in creating cartilage without "hollow" phenomenon. biocompatible endoskeleton demonstrated

8.Effects of different liquid therapies on brain water content, intracranial pressure, and expressions of aquaporin-4 and N-methyl-D-aspartate receptor-1 in cerebral tissue.

OBJECTIVETo explore the effects of different liquid therapies on the intracranial pressure, brain water content, and expressions of aquaporin-4 and N-methyl-D-aspartate-1 in the brain tissue.
METHODSTwo intervention factors including the colloids (two levels: 4% gelofusine; 6% hydroxyethyl starch and sodium chloride injection) and the crystal/gel ratios (two levels: 0:1; 1:1) were set based on the results of the analysis of variance of factorial design. Thirty-two patient who had undergone epilepsy surgery were equally and randomly divided into four groups: group A (4% gelofusine, crystal/gel ratio 0:1); group B (6% hydroxyethyl starch and sodium chloride injection, crystal/gel ratio 0:1); group C (4% gelofusine, crystal/gel ratio 1:1); and group D (6% hydroxyethyl starch and sodium chloride injection, crystal/gel ratio 1:1). The intracranial pressure during operation was recorded. After the operation, the intracranial pressure and brain water content were measured and the expressions of aquaporin-4 and N-methyl-D-aspartate-1 in the brain tissue were determined with Western blot. Glasgow coma scores were obtained 2 hours after operation.
RESULTSThe intracranial pressure (F=55.714, P=0.000; F=142.432, P=0.000) and the brain water content (F=31.477, P=0.000; F=84.896, P=0.000) significantly increased after the application of the 6% hydroxyethyl starch and sodium chloride injection and crystal/gel ratio 1:1, and the expressions of aquaporin-4 (F=37.205, P=0.000; F=149.652, P=0.014) and N-methyl-D-aspartate-1(F=29.664, P=0.000; F=65.951, P=0.000) in the brain tissue significantly increased. There were additive effects between two of them (the intracranial pressure: F=11.056, P=0.002; the brain water content: F=8.007, P=0.008; the expression of aquaporin-4: F=9.845, P=0.004; and the expression of N-methyl-D-aspartate-1: F=5.020, P=0.033). However, the Glasgow coma score showed no significant difference after the administration (P>0.05).
CONCLUSIONThe liquid therapy with 4% gelofusine and crystal/gel ratio 0:1 can result in better control on the intracranial pressure, brain water content and expressions of aquaporin-4 and N-methyl-D-aspartate-1 in the brain tissue better than the liquid therapy with 6% hydroxyethyl starch and crystal/gel ratio 1:1 during neurosurgery, although it may not improve the coma status.

9.Bactericidal efficacy of five kinds of disinfectant on Staphylococcus aureus

Objective To study the bactericidal efficacy of hospital commonly used disinfectants such as ethanol,3" chlorine tablets,iodophor,glutaraldehyde,and avagard instant hand antiseptic on Staphylococcus aureus (S.a ureus) from different sources of hospital,and provide scientific basis for effective control of healthcare-associated infection(HAI).Methods A total of 48 strains of S.aureus from inpatients,hands of health care workers,and environment surface in the First Affiliated Hospital of Nanchang University were collected.Disinfectant was directly contacted with bacteria,in vitro killing efficacy of disinfectant on S.aureus from different sources at different diluted concentrations,and different contact time were studied.Results The killing rate of 5g/L iodophor,20g/L glutaraldehyde,and avagard instant hand antiseptic(0.5% chlorhexidine + 70% ethanol) to S.aureus with a 5-minute contact time was 100%;killing rates of 70% ethanol and 1g/L 3" chlorine tablets to S.aureus with a 5-minute contact time were 96.5 %-99.8 %;but highly diluted iodophor,glutaraldehyde,and avagard instant hand antiseptic still could not completely kill S.aureus even the contact time was extended.Conclusion The routine use of disinfectants in the hospital can meet the clinical bactericidal efficacy,it is necessary to monitor concentration routinely,avoid decreasing sterilization ability.
10.Clinical features and genetic screening of hereditary hemorrhagic telangiectasia.

OBJECTIVETo analyze clinical features of 4 families with hereditary hemorrhagic telangiectasia (HHT) and potential mutations of ENG, ACVRL1 and SMAD4 genes.
METHODSFour unrelated HHT patients and their affected family members were analyzed. All exons and flanking regions of ENG, ACVRL1 and SMAD4 genes were analyzed with PCR and direct sequencing and multiplex ligation-dependent probe amplification (MLPA) methods.
RESULTSEleven patients from the 4 families were enrolled in this study. Two ENG and 1 ACVRL1 mutations were identified, among which an ENG mutation (c.207G>A; p.L69L) and an ACVRL1 mutation (c.817C>T; p.L273L) have been previously reported. In addition, a novel ENG mutation (c.1004A>T; p.Q335L) has been found in 3 different families. Similar mutations were not detected in 200 healthy individuals. No mutations of ENG, ACVRL1 and SMAD4 were found in the fourth family.
CONCLUSIONA novel mutation c.1004A>T (p. Q335L) of ENG has been identified in patients with HHT. And there is significant phenotypic variability and genetic heterogeneity with the disease.
