1.Polyvesicular Vitelline Tumor of the Ovary: A case report .

Polyvesicular vitelline tumor of the ovary is an extremely rare variant of yolk sac tumor. We present a case of pure polyvesicular vitelline tumor in a 43-year-old woman. Light microscopy revealed a predominantly polyvesicular pattern embedded in mesoblastic stroma with the cysts showing two type of lining; tall columnar and cuboidal, or mesothelioid cells. The lining atypical cells showed occasional mitoses and intracytoplasmic PAS positive hyaline globules. In some areas, the cystic space contained a large amount of intraluminal hyaline material. Immunohistochemically, alpha- fetoprotein and alpha-1-antitrypsin were detected as a fine granular deposit in the cytoplasm of epithelial cells and hyaline globules. Electron microscopically, marked specialization of the vesicular lining cells suggested a differentiation toward gut structures and mature yolk sac.

3.Construction and Immuno - biochemical Studies of Chimeric Polioviruses Expressing Multivalent V3 / PND - concatamers of Human Immunodeficiency Virus Type 1.

Poliovirus Sabin 1 strain has its own special features that make it a particularly attractive live recombinant mucosal vaccine vehicle. Sabin 1 cDNA was manipulated to have multiple cloning site and viral specific 3C-protease cutting site at the N-terminal end of the polyprotein, named RPS-vax system HIV-1 V3- and principal neutralizing domain (PND)-concatamers were successfully cloned into the multiple cloning site of the vector system and produced expected chimeric viruses by transfection of their RNA transcripts into HeLa cells. These chimeric viruses have shown to express introduced HIV-1 subgenome concatamers efficiently during their replication in the infected HeLa cells. Expressed proteins were confirmed to retain the wild type structures at least in parts. Replication capacity of the chimeric viruses was slightly lower than that of wild type Sabin 1 likely to be due to delay in processing steps during their replication. Differing from the virulent Mahoney vectors, the rec-Sabin 1 chimeric viruses maintained the foreign gene stably during the serial passages. These chimeric viruses have also shown to be able to induce specific humoral immunity to the introduced vaccine proteins when inoculated into the poliovirus receptor-expressing transgenic (Tg-PVR) mice. Antiserum obtained from the immunized transgenic mice showed to have neutralizing capacity to HIV-1 in vitro. These results strongly suggest that the chimeric viruses expressing HIV-1 vaccine epitopes can be used as a good live mucosal vaccine candidate against AIDS.

5.Ultrastrural Observation in A Case of Palmar Fibromatosis.

We herein report a case of palmar fibromatosis, who was 78-year-old male and had had multiple variable-sized subcutaneous hard nodules on the left palm for 2 months. Histopathologic findings of the lesion revealed that the nodules were highly cellular, containing numerous spindle shaped cells. Ultrastructurally, intracytoplasmic micrcfilaments were present, and the nuclei were extremely indented by one or more transverse folds. These findings suggested the cells were myofibroblasts.

6.A Morphological Study of Stratum Corneum.

Stratum corneum was obtained from 10 healthy individuals aged 23 and 24 years, in order to observe morphological differences in a three-dimensionsl structure of stratum corneum from 3 anatomical sites, i. e., flexor surface of the forearm, abdomen, and palm. The stratum corneum obtained by the skin surface biopsy was ohserved under a scanning electron microscope(SME). The surfaces of stratum eorneum obtained by the replice, technique were observed under a stereomicrascope(SM). The results were as follows : 1) SEM observation of the posterior surfaces of the stratum corneum from the skin surface biopsies of the flexor surface of the forearm and abdomen showed divisions with folding lines which were elliptic in the forearm and squere in the abdomen. But there were no similar folding linea in the palm. Among the 3 anatomiral sites, overlapping corneocytes on the palm were most prominent. On a higher magnification, small pits and fine wrinklings in the forearm, fine wrinklings in the abdomen, and villous projections in the palm were observed respeeti vely. 2) SM observation of the stratum corneum from the negative replica of the flexor surface of the forearm, abdomen, and palm showed divisions with folding lines which were X-shape a.t the site of their crossinga in the forearm, *-shape in the abdomen, and linear shape in the palm, Among the 3 snatomical sites, tiny holes on the palm were most prominent. 8.Surgical treatment of atrial septal defect in adult patients.

The study consisted of all patients over 35yerars old undergoing surgical repair of atrial septal defect for the period from June 1985, to August 1992. The following results were observed. 1. ASD was closed with patch in 11 (73%) patients. 2. The relationship of pulmonary artery systolic pressure to Qp/Qs ratio was not significant. 3. Before operation 6 patients were in NYHA functional class II. 8 were in class III, After operation 8 patients were in class I, 6 were in class II. 4. Atrial fibrillation has persisted in 3 patients and returned regular rhythm in 1 patient after surgery. 5. There was no operative mortality and we had good surgical results regardless of patient's age.

9.Cutaneous Horn in Childhood: A Case Arising from Verrucaa Vulgaris.

"Cutaneous horn", the clinical term first defined by Moncorps in 1931, has been used to describe those markedly hyperkeratotic growths resembling the horns of animals in shape and hardness. Recently, we observed a typical case of cutaneous horn in a 13-year-old child. He had a finger-tip sized, yellow-brown, hard, protruding tumor with surrounding erythema on the right little finger for 2 years. On microscopic examination, the biopsy specimen from the squamous epithelium of tbe base showed a typical verruca vulgaris. Total excision and skin graft were performed under general anesthesia.

10.Negative Association between Schizophrenia and Rheumatoid Arthritis : Epidemiology and Prolactin and Estrogen as Protective Factors.

The purposes of this study were to test the negative association between schizophrenia and rheumatoid arthritis(RA) and to clarify the role of prolactin and estrogen as protective factors in this association. The author compared the prevalence rate of RA between 561 patients with schizophrenia and 222 patients with mood disorder. For investigating the role of estrogen and prolactin, the author checked the plasma prolactin and estradiol level in 80 patients with paranoid schizophrenia and 77 patients with RA. The results were as follows. 1) Epidemiological data The prevalence rate of RA in the schizophrenic group was 0/561 and that of RA in the mood disorder group was 2/222. To compare these results between two groups, the author applied the Binomial test using the average prevalence rate of RA(0.8%) in the general population as a reference rate. The prevalence rate of RA in the schizophrenic group was significantly lower than that of RA in the general population. However, the prevalence rate of RA in the mood disorder group was not significantly different to that of RA in the general population. 2) Comparison of plasma prolactin and estradiol level between two groups. The plasma level of prolactin in the schizophrenic group was significantly higher than that of prolactin in th RA group(p=0.000). However, the plasma level of estradiol in the schizophrenic group was significantly lower than that of estradiol in the RA group(p=0.017). These results were not consistent across gender. To contrast with the results in the female group, which were consistent with the results in the total subjects, for the male group, the plasma levels of prolactin and estradiol in the schizophrenic group were significantly higher than those of prolactin and estradiol in th RA group. These results support the results of previous studies which confirm the negative association between schizophrenia and RA. These results also suggest that the elevation of plasma prolactin level in the patient with schizophrenia has a antirheumatic effect while the elevation of plasma estradiol level in the patients with RA has a anti-schizophrenic effect, and that these effects act as a possible mechanism in the negative association between two disorders. However, these results suggest that this association is specific to female patients.
