Pulmonary artery rupture with a flow-directed balloon tipped pulmonary artery catheter (Swan-Ganz catheter)is a rare complication with high mortality. This report concerns a case of this complication with hemothorax leading to fatal exsanguination. Emphasis is placed on the safety guidelines to prevent this. Also, possible causes and managements are presented. A high index of suspicion is necessary whenever a patient with the catheter has hemoptysis or unexplained hemodynamic or respiratory changes.

5.The Intestinal Obstruction Due to Bezoar.

Bezoars are large conglomerates of vegetable fiber and hair, or concretions of various substances located in the stomach or the intestinal tract. They are classified as trichobezoars, phytobezoars, trichophytobezoars or concretions. The most common site for a bezoar is the stomach, but small-bowel bezoars are occasionally reported. A gastric bezoar can be treated by endoscopic removal or chemical dissolution. Small bowel bezoars are usually discovered due to intestinal obstruction, and surgical removal is the standard treatment method. We report the case of a patient who suffered from an intestinal obstruction due to a phytobezoar about 5 4 3 cm3 size.

6.Anticancer Effects of IP6 in a Human Colon Carcinoma Cell Line in Nude Mice Xenografts.

PURPOSE: Inositol hexaphosphate (IP6) is a naturally occurring polyphosphorylated carbohydrate that has been shown to suppress the growth of epithelial cancer. Because IP6 is a dietary phytochemical present in cereals, soy, legumens, and fiber-rich foods, we evaluated the efficacy of IP6 against colon cancer formation. METHODS: HT-29 cells were injected into nude mice. The animals were fed a normal diet (group 1), a low IP6 diet (group 2), and a high IP6 diet (group 3) for 4 wk. Body weight, tumor volume, tumor growth rate, growth inhibition rate, and therapeutic ratio were monitored after injection of HT-29 cells. RESULTS: HT-29-cell human-colon-carcinoma xenograft mice treated with IP6 showed a significant reduction in tumor growth rate, irrespective of the IP6 dose compared to normal diet group. Compared with the control group, group 3 showed a significant reduction (45%) in tumor volume. In the therapeutic ratio gain profiles, IP6 diet groups showed a significant alteration of therapeutic ratio when compared with the normal diet group (0% vs. 11%, P=0.014). In the body weight gain profiles, group 3 showed a significant reduction of body weight compared with the other two groups (20.25 g vs. 21.6 g, 21.7 g, P=0.009). Groups 1 and 2 showed similar changes in body weight. Tumor xenografts from IP6-fed mice showed significantly decreased cancer formation and growth, but increased toxicity was noted for high doses of IP6. CONCLUSION: These results indicate that in the future, IP6 could be an effective chemopreventive or chemotherapeutic agent for use in the treatment of colon cancer.

7.Analysis of Risk Factors on the G-induced Loss of Conscious in ROKAF Pilots.

Of all the aeromedical treats to flying safety, G-induced loss of consciousness (G-LOC) must be one of the most important. The present study is undertaken to acquire the physiological normative data (including lifestyle and physical fitness) of KAF pilots and to investigate the factors that will be related with G-LOC. From 15 May through 27 Nov 2000, 464 KAF pilots underwent high G training and flight performance tests at the Aeromedical Research and Training Center are checked CBC, total cholesterol, PFT, physical fitness battery tests-muscular strength, muscular endurance, power, and flexibility. High G training and 2 kinds (before and after the G-training) of questionnaire were tried. Pilot's attitude for health promotion was relatively passive and the proportions as of high and borderline risk for atherosclerosis in total cholesterol level were 4.98% and 22.1%, respectively. 17.6% of the pilots had G-LOC and three quarters of them fell into G-LOC within the first 5 seconds. Variables showing significant correlation (p<0.05) with G-LOC were as follows; height, age, total flying time, and high G-training times. Trunk extension had positive correlation and curls ups had negative correlation, but the reasons are needed more following study. Also, the judgments of pilots and a supervisor in skillfulness of L1 maneuver were significant with G-LOC.

8.The Effects of Thoracic Epidural Anesthesia and Vagotomy on the Enflurane-Epinephrine Induced Arrhythmias in Rabbits.

BACKGROUND: To evaluate the effects of thoracic epidural anesthesia, with or without bilateral vagotomy, epinephrine-induced arrhythmias were studied in 31 rabbits anesthetized with 1 MAC enflurane. METHODS: Logdose protocol was used for the infusion of epinephrine; its arrhythmic dose being defined as the smallest infusion rate produced four or more continuous or intermittent arrhythmias within 15 seconds. RESULTS: The values (geometric mean) of arrhythmic doses and the plasma concentration of epinephrine during arrhythmia were as follows: 10.21 g.kg 1.min 1 and 83.16 ng/ml in epidural control group; 118.90 g.kg 1.min 1 and 677.76 ng/ml in epidural lidocaine group (p<0.05); 6.34 g.kg-1.min 1 and 96.42 ng/ml in intravenous lidocaine group; 8.65 g.kg 1.min-1 and 44.64 ng/ml in vagotomized-epidural control group; and 12.03 g.kg 1.min 1 and 95.35 ng/ml in vagotomized- epidural lidocaine group. CONCLUSIONS: The data suggest that thoracic epidural anesthesia raises the threshold for enflurane- epinephrine arrhythmias in rabbits and that this effect is eliminated by bilateral vagotomy.

9.Congenital Urinary Tract Anomalies Associated with Urinary Tract Infection in Infants and Children.

We report 4 cases of malignant thymoma which were composed of 2 cases of invasive thymoma and 2 cases of thymic carcinoma. The cytologic findings of invasive thymoma were similar to those of benign thymoma. The distinctive cytologic features of thymic carcinoma were necrotic background, irregular clusters and individually scattered arrangement of anaplastic epithelial cells, and some scattered mature small lymphocytes. These findings may be found in the Hodgkin's lymphoma, seminoma, and metastatic squamous cell carcinoma, undifferentiated carcinoma, and large cell carcinoma of the lung. But, the feature of irregular clustering of anaplastic epithelial cell having scanty cytoplasm was different from Hodgkin's lymphoma and seminoma. Clinical and radiologic findings as well as cytologic finding were helpful in differential diagnosis of thymic carcinoma from metastatic carcinoma.

10.Hemolytic-Uremic Syndrome Associated with Bloody Diarrhea.

The authors analysed 2,653 cases of transthoracic fine needle aspiration cytology of the lung to evaluate the diagnostic accuracy and its limitation. A comparison was made between the original cytologic and the final histologic diagnoses on 1,149 cases from 1,074 patients. A diagnosis of malignancy was established in 38.3% benign in 48.1%, atypical lesion in 2.3%, and inadequate one in 11.9% of the cases. Statistical data on cytologic diagnoses were as follows; specificity 98.9%: sensitivity of procedure, 76.8%: sensitivity of diagnosis, 95.5%: false positive 5 cases: false negative 18 cases: predictive value for malignancy, 98.8%: predictive value for benign lesion, 79.5%: overall diagnostic efficiency, 87.5%: typing accuracy in malignant tumor, 80%.
