1.Thoughts and suggestions on personnel training assessment index in performance appraisal of tertiary public hospitals

Personnel training marks a key assurance for the healthy and sustainable development of healthcare sector and healthcare institutions in China, rendering it critical importance to include personnel training into performance appraisal. The authors analyzed the connotation and orientation of personnel training index assessment in the performance appraisal at tertiary public hospitals in the country, sorted out the assessment details to be optimized. On such basis, the authors recommended on optimal assessment methodology, combining the gradual improvement of medical personnel training assessment indexes and interval range assessment. These efforts aim at providing references for further improving the scientificity, standardization, homogeneity, as well as vertical and horizontal comparability and operability of personnel training index assessment.
2.The technique and effectiveness of potency preservation in robotic-assisted laparoscopic radical prostatectomy

Objective To evaluate the key technique and effectiveness of potency preservation in robotic-assisted laparoscopic radical prostatectomy (RALP).Methods The complete clinical and follow-up data of 30 cases underwent RALP between February and May of 2016 were reviewed retrospectively.The average age of the patients was 67.3 years (48-82 years).The peak PSA level before surgery ranged from 7.6 to 53.4 ng/ml with the average level of 21.1 ng/ml.There were 7,16,6 and 1 case with the Gleason score of 6,7,8,and 9 point,respectively.Preoperative erectile score (IIEF-5) of the 30 patients were list as below:3 cases (0-10 points),4 cases(11-15 points),17 cases(16-20 points),and 6 cases(21-25 points).The key techniques of potency preservation during RALRP includes deep dorsal vein ligation techniques,neurovascular bundles preservation techniques and drain tube placement techniques.Results All 30 cases underwent operation successfully with no transfer to open surgery.Average operative time was 150.7 min (98-240 min) with an estimated blood loss of 165.7ml (50-550 ml).The average drainage removal time was 5.1 d postoperatively.The average bowel recovery time was 2.7 d and average hospitalization time was 8.2 d,respectively.There were two cases with one positive margin on the bladder neck and one on the tip of prostate,respectively.Seventeen cases gained potency preservation six months after surgery.Conclusion It was safe and beneficial for the potency preservation in RALP,which could be considered as one of the best options for the prostate cancer surgery.

Objective To investigate the characteristics and clinicopathologic significance of microlymphatic vessel in breast cancer.Methods The microlymphatic density(MLD)and lymphatic vessel invasion(LVI)in 102 cases of breast cancer tissue were evaluated by immunohistochemical staining,using monoclonal antibody for podo- planin.The characteristic of microlymphatic vessel and the relationship between MLD,LVI and clinicopathological parameters were evaluated.And blood vessels were also detected with CD34 by double-labeling immunohistochemis- try for confirming the specificity of podoplanin for microlymphatic vascular.Results Podoplanin antibody was spe- cific for lymphatic vessel without intersection with blood vessel.The density and morphology of microlymphatic ves- sel in breast cancer had significant heterogeneity.The MLD in breast cancer tissues was significantly higher than that in normal breast tissues.The microlymphatic vessel that in breast cancer tissues indicated by a more irregular shape and a larger open lumen,and some cancer embolus entering the open microlymphatic vessel could be seen. 4.Clinical trial of esomeprazole enteric-coated tablets combined with Lizhong pill in the treatment of gastric ulcer

Objective To observe the clinical efficacy and safety of esomeprazole enteric-coated tablets combined with Lizhong pill in the treatment of gastric ulcer.Methods Fifty-four patients of gastric ulcer were randomly divided into control group (n =27 cases) and treatment group (n =27 cases).Control group was treated with esomeprazole 40 mg oral,tid.On the basis of control group,treatment group was given Lizhong pill 9 g oral,bid.Two groups were treated for 14 d.The clinical efficacy,levels of serum interleukin-8 (IL-8),tumor necrosis factor-α (TNF-α),vascular endothelial growth factor (VEGF),basic fibroblast growth factor (bFGF),and the occurrence of adverse drug reactions were compared between two group.Results After treatment,the total effective rates in treatment and control groups were 92.59% (25/27 cases) and 70.37% (19/27 cases),with significant difference (P < 0.05).After treatment,in treatment and control groups,the levels of serum IL-8 were (15.85 ±2.08),(19.37 ±2.11) pg · mL-1;TNF-α were (70.75-±10.05),(82.65 ±11.03)pg · mL-1,there was statistically significant difference (P < 0.05).After treatment,the levels of VEGF were (473.54 ± 50.78),(456.31 ± 51.57) pg · mL-1;bFGF were (567.43 ± 63.24),(540.86 ±58.67)pg · mL-1 in the treatment and control groups,respectively,with no significant difference (P > 0.05).The adverse drug reactions were abdominalgia,sicchasia and rashes in two groups.The incidences of adverse drug reactions in treatment and control groups were 11.11% and 18.52%,and there was no statistically significant difference (P > 0.05).Conclusion Esomeprazole enteric-coated tablets combined with Lizhong pill has a definitive clinical efficacy in gastric ulcer treatment,and without increasing the incidence of adverse drug reactions.
5.Protective effect of oxymatrine on chronic heart failure and ADMA metabolism pathway in isoproterenol-induced chronic heart failure in rats.

OBJECTIVETo investigate the protective effects of oxymatrine on chronic heart failure induced by isoproterenol (ISO) and to observe its effects on ADMA metabolism pathway in ISO-induced chronic heart failure in rats.
METHODMale Sprague-Dawley rats were given oxymatrine (100,50 mg kg-1) orally for 14 days. Heart failure was induced in rats by subcutaneous injection of isoproterenol (5 mg kg-1 d-1 ) at the 8th day for 1 week. Serum parameters, haemodynamic parameters, Heart weight, and histopathological variables were analysed. Expression of protein levels were measured by Western blot.
RESULTOxymatrine (100,50 mg kg-1) significantly attenuated serum content of cTn I, improved left ventricle systolic and diastolic function and left ventricular remodeling, reduced the ISO-induced myocardial pathological changes compared with ISO group. In addition, oxymatrine (100,50 mg kg-1) significantly reduced serum level of ADMA (P <0. 01), normalize the reduced dimethylarginine dimethylaminohydrolase 2 (DDAH2) expression (P <0. 01) , but had no effect on the isoproterenol-induced upregulated protein arginine methyltransferases 1 expression.
CONCLUSIONOxymatrine could ameliorate the experimental ventricular remodeling in ISO-induced chronic heart failure in rats and the mechanism involved in reducing serum content of ADMA and increased DDAH2 expression.

7.Cost analysis of Shenqi Fuzheng injection on reducing adverse effects during chemotherapy.

OBJECTIVEThe goals of this work was to analyse the cost of Shenqi Fuzheng injection-an extraction of a Chinese traditional herbs on reducing adverse effects in lung cancer patients during chemotherapy.
METHODSIn a randomized cross-over trial, each patient completed two identical cisplatin-based chemotherapy cycles, one with Shenqi Fuzheng injection, another without Shenqi Fuzheng injection. Adverse effects and change scores of quality of life (QOL) during chemotherapy were compared in tow cycles. The direct cost dealing with adverse effect and cost-effectiveness analysis were taken.
RESULTSOne hundred and thirty were enrolled with 123 of whom were evaluable. The patient characteristics were well balanced between the two groups. The chemotherapy cycles with Shenqi Fuzheng injection spent 220.5 more Chinese yuan, but the adverse effect of leukopenia, thrombocytopenia and vomiting were slight different and the change of score of several QOL domains showed significant better as compared to those in another cycle.
CONCLUSIONShenqi Fuzheng injection could reduce the severity of toxicity related to chemotherapy and improve the QOL of patients and had some benefits in terms of cost-effectiveness.

8.Expression of fibulin-5 in urothelial carcinoma of bladder and the effects of its overexpression on the mo tility and invasion of bladder cancer cells

Objective To study the role of fibulin-5 in urothelial carcinoma of bladder. Methods Fibulin-5 expression was detected in bladder cancer tissues (13 cases of G_1 and G_2, 7 cases of G_3) and normal bladder mucosa samples by Western blotting assay. Fibulin-5 cDNA was amplified by PCR and cloned into pMD-19T simple vector. The pMD-19T-Fibulin-5 vector was digested by restriction endonucleases XhoI and EcoRI to generate a XhoI-Fibulin5-EcoRI fragment that was then ligated into the identical sites in p-EGFP-Nl plasmid to synthesize p-EGFP-Fibulin-5 plasmid. The p-EGFP-Fibulin-5 plasmid was finally transfected into bladder cancer cell line 5637. The migration and invasion of untransfected, vector-transfected and fibulin-5-transfected bladder cancer cells were measured by Boyden chamber assay. Results Compared to 1. 16 ±0. 28 in the normal control, the expression of fibulin-5 protein in low grade and high grade tumors were 0. 57±0. 32 and 0. 44±0. 42(P<0. 01, respectively). However, the difference between low grade and high grade tumors was not statistically significant (P>0. 05). The successfully transfected bladder cancer cells demonstrated green fluorescent light. The migrated cell number of fibulin-5-transfected cells was 127. 6 ± 3. 1 compared with 139. 3±7. 7 for vector-transfected cells and 136. 9±5. 7 for untransfected cells (P>0. 05, respectively). In contrast, the invaded cell number of fibulin-5-transfected cells was 8. 0±3. 1 compared with 31. 5±4. 8 for vector-transfected cells and 31. 7±4. 7 for untransfected cells (P<0. 01, respectively). Conclusion Fibulin-5 is down-regulated in urothelial carcinoma of bladder and acts as a tumor suppressor gene by inhibiting the invasion of bladder cancer cells.
9.Effect of exogenous cytokine-stimulated decidual cells of early pregnancy on IgG secretion of B lymphocytes.

OBJECTIVETo study the effect of exogenous cytokine-stimulated decidual cells on IgG secretion of B lymphocytes and investigate the features of local immunological microenvironment of the decidua.
METHODSExogenous cytokines interferon-gamma (IFN-gamma), interleukin-2 (IL-2), IL-6 and epidermal growth factor (EGF) were added separately in cultured decidual cells, and the supernatant of the culture medium was prepared for stimulating human peripheral blood lymphocytes. IgG secretion of the B cells was measured by radio-immunological method.
RESULTThe decidual cells of normal early pregnancy stimulated B lymphocyte IgG secretion, and the supernatant of exogenous cytokine-stimulated decidual cells had the same effect, which, however, was depended not on the concentration and category of the cytokines, but only on the time of treatment.
CONCLUSIONThe exogenous cytokines can increase the humoral immunity in the decidual immunological microenvironment, but such effect might result from a self-regulatory mechanism of the local immunological microenvironment of the decidua, which can be fundamental for maintenance of normal pregnancy.

10.Necessity of splenectomy in radical resection of gastric cancer: a meta-analysis.

OBJECTIVETo evaluate the necessity of splenectomy in radical resection of gastric cancer.
METHODSTwelve studies comparing outcomes after radical resection of gastric cancer with or without splenectomy were identified. Both fixed effect model and random effect model were used.
RESULTSThere were 2628 patients in total. There were significant differences in complications between splenectomy group and spleen-preserving group(OR=1.91, 95% CI:1.28-2.87, P<0.05), while no significant difference in 5-year survival rate was noticed(HR=0.90, 95% CI:0.73-1.11, P>0.05).
CONCLUSIONRadical resection of gastric cancer combined with splenectomy is not associated with improved survival but increased postoperative complications.
