3.Influence of nursing intervention on self-esteem level and quality of life in patients with epilepsy

Objective To discuss the influence of nursing intervention on self-esteem level and quality of life in patients with epilepsy.Methods 60 cases of epilepsy patients were randomly divided into observation group (30 cases) and control group (30 cases). Both groups received in-hospital health education while the observation group received nursing intervention after discharge. Self-esteem Scale (SES), Feelings of Inadequacy Scale (FIS) and Quality of Life Scale in Patients with Epilepsy (QOLJE31) were used on discharge and one year later.Results There was no significant difference of SES or FIS scores on discharge (P<0.05) but there was one year later (P>0.05). The observation group obtained higher QOLJE31 scores on discharge than the control group. There was significant difference between the two groups (P<0.01). There was no significant difference of QOLJE31 scores on discharge compared with one year later of the control group (P>0.05).Conclusions Nursing intervention plays a positive role in eliminating inferiority feelings, enhancing self-esteem and promoting quality of life in patients with epilepsy.
4.Influence of health-behavior training on mind and serf-concept of children patients at school age having falling sickness

Objective To explore the influence of health-behavior training on mind and self-notion of children patients at school age having falling sickness. Methods Taking 112 school-aged children patients having falling sickness as the study subjects and dividing them into two groups: the study group and the control group. The control group was given a health education, and the study group was conducted a health-behavior training in addition to the health eduction. A comparison between the conditions before the training and after the training was conducted with Piers-Harris children self-concept scale. In addition, an evaluation on the effect was made after 1 year of discharge of the children from hospital. Results The six components, including behaviors, mind and conditions in school, physical appearance, anxiety, being gregarious, blessedness and satisfaction, as well as the total score of these' components, among the study group after being intervened, have statistically significant differences when compared to those of the control group (for all, P <0.01 ). The attitude to disease and to side effects of drugs, emotion, ability of adaptation to society, healthy condition, and satisfaction degree to lives, for the children patients in the study group is better than those for the children patients in the control group (P < 0. 01 ). Conclusions Taking health-behavior training as the intervening means may help the children patients eliminate their self-abasement mentality, and improve their self-concept level.
5.Prevalence and risk factors of retinopathy of prematurity.

OBJECTIVETo investigate the prevalence and the risk factors of retinopathy of prematurity (ROP).
METHODSTotally 172 premature infants who were less than 37 weeks postconceptional age, or more than 37 weeks but weighing < 2 500 g at birth, and born at PUMC hospital from May 1, 2003 to November 30, 2004, were enrolled in this study. Their fundus were routinely checked. Diagnosis and staging of ROP were performed according to the international guidelines. Another 20 mature infants were selected as the control group.
RESULTSTwelve infants quitted the treatment or died. The remaining 160 infants completed the follow up. The prevalence of ROP in the premature group was 19.4%, while no ROP was found in the control group. The prevalence of ROP in subgroup with body weight < or = 2 000 g (28.4%) was significantly higher than in subgroup with body weight > 2 000 g (8.3%, chi2 = 10.217, P = 0.001) at birth. The prevalence of ROP in subgroup with postconceptional age < or = 32 weeks (42.5%) was significantly higher than in subgroup with postconceptional age > 32 weeks (11.7%, chi2 = 18.258, P = 0.000). The postconceptional age (OR = 0.959, P = 0.036) and body weight (OR = 0.999, P = 0.026) were the most important risk factors of ROP. Furthermore, blood transfusion ( OR = 0.076, P = 0.029) and Apgar score ( OR = 23.62, P = 0.012) were inversely correlated with ROP. Correlation was not found between ROP prevalence and oxygen inhalation mode, surface active substance, administration of dopamine and dexamethasone, and mother conditions.
CONCLUSIONSThe prevalence of ROP is higher in premature infants than in mature infants. Shorter postconceptional age and lower body weight may result in higher ROP incidence. Routine screening of fundus in premature infants may be helpful for the early detection of ROP.

7.Analysis of influencing factors for pregnancyinducedhypertension retinopathy and its influence on pregnancy outcome of mothers and infants

AIM: To investigate the risk factors for pregnancy induced hypertension retinopathy, and analyze its influence on maternal and child pregnancy outcomes. METHODS: A total of 100 patients with gestational hypertension who met the requirements of screening were selected, they were divided into two groups, without retinal lesion group (n = 29) and retinopathy group (n=71). The age, course of disease, gestational age, blood pressure, proteinuria and hematocrit in two groups were investigated. The risk factors were detected by single factor analysis, and then the study on independent risk factors were analyzed by Logistic regression analysis. At the same time,their influence on maternal and child pregnancy outcomes were investigated. RESULTS: The single factor analysis showed that except for age, the differences in the course of disease, gestational age, blood pressure, proteinuria and hematocrit were statistically significant (P < 0. 01). Logistic regression analysis showed that the course of disease, blood pressure, proteinuria and hematocrit were the risk factors of retinopathy, gestational age was a protective factor. Compared with the group without retinopathy, the incidence of preterm birth, placental abruption, postpartum hemorrhage, perinatal death in the patients with retinopathy significantly increased, and the difference were statistically significant(P<0.01),and there was a certain correlation between the extent of increase and the grade of retinopathy. CONCLUSION: Course of disease, blood pressure, proteinuria and hematocrit of patients with the hypertension of pregnancy are the risk factors of retinopathy,retinopathy has a serious adverse effect on maternal and child pregnancy outcomes.
8.Association of ABCG2 gene C421A polymorphism and susceptibility of primary gout in Han Chinese males.

OBJECTIVETo assess the association between a C421A single nucleotide polymorphism (SNP) in exon 5 of ATP-binding cassette, sub-family G (WHITE), member 2 (ABCG2) gene and susceptibility of primary gout in Han Chinese males.
METHODSFor 200 male patients with primary gout and 235 controls, the genotype of C421A locus was analyzed by PCR and direct sequencing. Blood glucose, uric acid, total cholesterol, triglycerides, creatinine and urea nitrogen was measured by an automatic biochemical analyzer.
RESULTSCompared with the controls, there was a higher frequency for AA genotype and A allele of the rs2231142 SNP in gout patients (22.5% vs. 8.5% by genotype; 44.9% vs. 32.3% by allele). The association with gout reached significance (chi-square =15.91, P< 0.001, crude OR=3.02, 95% CI:1.36-4.90 and OR (adjusted by age)=1.80, 95% CI: 1.32-2.45 by dominant mode; chi-square=6.82, P=0.009, OR=1.67, 95% CI: 1.54-2.27 by recessive mode). Blood glucose, uric acid, triglycerides, creatinine and urea nitrogen levels in gout patients were significantly higher than those of controls (P< 0.001).
CONCLUSIONThe C421A SNP, in particular AA phenotype, may be associated with susceptibility of primary gout in Han Chinese males.

9.Clinical study of Astragalus combined with 1,6-diphosphate on child viral myo-carditis

Objective To evaluate clinical efficacy and safety of astraga-lus combined with 1,6 -diphosphate in the treatment of pediatric viral myocarditis( PVM) and its effect on cellular immune function of pediatric patients.Methods Eighty-one children with PVM were randomly di-vided into control group ( n=44 ) and experiment group ( n=37 ).Oral mulvital and 200 mg· kg -1 · d-1 of intravenous 1,6-diphosphate were given to children in the control group as the basic treatment.And chil-dren were given 100 mg · kg -1 of astragalus plus 10% glucose 100 mL intravenous as well as the basic treatment of control group.After 3 -week treatment , the total efficacy , side effect and cellular immune func-tion of children were recorded and compared between the two groups.Results The total efficacy of experiment group ( 97.3%) was much higher than that of control group (81.8%)(P<0.05).The cellular im-mune function ( CD3 +, CD4 +, CD8 +and NK cell percentage ) was sig-nificantly improved in children compared with prior -treatment condition ( P<0.05 ) , and a much higher improvement was detected in the experi-ment group ( P<0.05 );the myocardial enzymes ( creatine kinase , crea-tine kinase isoenzyme , and lactate dehydrogenase ) were decreased after treatment, and the experiment group showed a much higher improvement. ( P<0.05 ).Only one patient had rash in the experiment group during the treatment.Conclusion Astragalus com-bined with 1,6-diphosphate has significant clinical efficacy and few side effects in children with viral myocarditis .The cellular immune function of patients can be improved by the combination regimen.
10.Transgenic 4-1-BB ligand therapy induces tumor specific immune response in oral squamous cell carcinoma

Objective To examine the activation and cytotoxieity of human peripheral blood T lymphocyte induced in vitro by human 4-1-BB ligand(4-1-BBL) gene transfected into tumor Tca8113 cells.Methods The eukaryotic expression vector pEGFP-h4-1-BBL was transfected into human oral carcinoma cell line Tca8113 by Lipofectamine<'TM>2000. The transfected cells were then selected in medium containing G-418, cloned by limited dilution and named as Tca8113-4-1-BBL Human 4-1-BBL mRNA and protein expression of transfected cells was detected by reverse transcriptase-polymerase chain reaction (RT-PCR) and western blotting respectively. The tumor cell vaccines (TCV) were obtained by treatment with mitomycin (MMC). Human peripheral blood mononuclenr cells (PBMC) were prepared from lymphoprep, and then stimulated with anti-CD-3 mAb and incubated with non-transfected or transfected TCV-TcaSI13 cells,respectively. The proliferation of T cells was evaluated by trypan blue exclusion; the CCK-8 was used to detect the cytotoxic effect of T lymphecytes. Meanwhile, the secretion of intefferom-γ (IFN-γ) and interleukin(IL)-2 in culture supernatant was detected by enzyme-linked immunosorbnent assay (ELISA).Results The Tca8113 cells trasfected by pEGFP-h4-1-BBL could express human 4-I-BBL efficiently. As compared with wild type Tca8113 cells, the transfected Tca8113 cells could markedly promote proliferation,IL-2 and IFN-γ production and cytotoxic activity of lymphocytos. Condusions The transfection of human 4-1-BBL gene in Tca8113 cells is effctive in enhancing its immunogenicity and inducing antitumor immune response in vitro.
