1.Pathological changes of cerebral arterioles around hematoma at acute stage in patients with intracerebral hemorrhage

Objective To observe the pathological changes of cerebral arterioles around hematom in patients with acute period of intracerebral hemorrhage. Methods Brain tissue fragments were collected from the drainage effluents of 43 patients who received mini-invasive puncture drainage treatment of acute intracereal hemorrhage. Pathological changes of arterioles were analyzed under light microscope by routine HE and Congo red staining, respectively. Results The primary pathological changes of cerebral arteriole around hematom was arteriosclerosis and hyaline degeneration, which accounted for 28 cases (65.12%). Other pathological etiological factors included atherosclerosis in 3 cases (6.98%) and thrombosis in 5 cases (11.63%). Light microscopy also showed neutrophilic granulocytes sticking on the endothelial cells and exuding around vessels in 10 cases (23.26%) from 6h to 4d, and mini-focal hemorrhage in 8 cases (18.6%) from 24 to 48h.Conclusions Arteriosclerosis is the principal pathological etiology for spontaneous intracerebral hemorrhage. Neutrophilic granulocytes infiltrating was one of the most important pathological changes in acute stage of intracerebral hemorrhage.
2.Inflammatory reaction characteristics of perihematoma brain tissue in patients with acute hypertensive intracerebral hemorrhage

48 hours.The expressions of leukocyte common antigen(LCA) and intercellular adhesion molecule-1(ICAM-1) in the brain tissues collected during the surgery were detected by immunohistochemistry staining method.Results LCA and ICAM-1 immunopositive cells and microvessels were verified in all 29 cases of ICH groups,and the expressions were more than control group(all P48 hours group were higher than that in 6~24 h group(both P48 hours group.Conclusions Acute inflammatory reaction occurs in acute period of ICH.Expression of ICAM-1 reinforces gradually during early 48 hours since the onset of ICH.

4.A Meta analysis on NOS3 G894T polymorphism and cancer susceptibility

Objective To investigate the relation between polymorphism of NOS3 G894T and susceptibility of cancer .Methods Literatures under the inclusion and exclusion criteria about relation between polymorphism of NOS 3 G894T and susceptibility of cancer were collected by computer -based retrieval and manual retrieval .The pools ORs with 95% CI were calculated to assess the association strength between polymorphism of NOS 3 G894T and cancer risk by Meta methods .Sensitivity and publication bias were evaluated .Results 19 literatures with 7679 cases of cancer and 8180 cases as control group were included .The pooled result indica-ted that no significant association between its polymorphism and cancer (ORT versus G=1 .030 ,95% CI=0 .964 -1 .101 ;ORTT versus GG=1 .056 ,95% CI=0 .895-1 .245 ;ORTG versus GG=1 .045 ,95% CI=0 .977-1 .119 ;ORTT/TG versus GG=1 .047 , 95% CI=0 .981-1 .117 ;ORTT versus TG/GG=1 .005 ,95% CI=0 .900-1 .123) .No significant association was observed in sub-group analysis based on ethnicity ,cancer type ,and source of control group .Conclusion There was no relationship between NOS3 G894T polymorphism and cancer susceptibility .
5.Application of Different Routes of Administration of Tranexamic Acid in One Stage Posterior Surgery of Thoracic Tuberculosis

OBJECTIVE:To observe the effects of different routes of administration of tranexamic acid on coagulation function and amount of bleeding in patients with one stage posterior surgery of thoracic tuberculosis. METHODS:40 patients suffered from thoracic tuberculosis in our hospital from Jan. 2011 to Dec. 2013 were randomly divided into intravenous group(5% Glucose injec-tion 100 ml+tranexamic acid 10 mg/kg,through an intravenous drip at 30 min before closing the wound) and topical application group(5% Glucose injection 10 ml and tranexamic acid 10 mg/kg,through soaking the wound before closing the wound)with 20 cases in each group. Other 15 cases suffered from the thoracic tuberculosis in our hospital from Jan. 2009 to Dec. 2010 were includ-ed in control group. 3 groups received one stage posterior surgery of thoracic tuberculosis,interbody fusion and internal fixation. The difference of hemoglobin,coagulation function and the amount of suction drainage were observed before and after surgery, and followed up. Bone graft fusion and therapeutic condition of tuberculosis were observed in the study. RESULTS:There was no statistical significance in postoperative suction drainage between intravenous group and topical application group (P>0.05),but their decrease was more significant than control group,with statistical significance(P<0.05). There was no statistically difference in fiber protease,prothrombin time or activated partial thromboplastin time among 3 groups(P>0.05). The difference value of he-moglobin in control group before and after operation was significantly higher than in intravenous group and topical application group,with statistical significance (P<0.05);there was no statistical significance between intravenous group and topical applica-tion group(P>0.05). 55 patients were all followed up and bone graft of all cases were fused,and all patients were cured and no case recurred. CONCLUSIONS:Tranexamic acid by intravenous application or topical application can reduce hemorrhage and ane-mia after operation of thoracic tuberculosis,and has no effect on blood coagulative system.
6.Changes in NT-proBNP after physical training in patients with chronic heart failure

Objective To investigate the effect of physical training on plasma N-terminal pro-brain natri- uretic peptide(NT-proBNP)levels in patients with chronic heart failure(CHF).Methods Eighty NYHAⅡ-ⅢCHF patients were randomly divided into a training group(n=42)and a control group(n=38).A 6-minute walk- ing test was performed within 24 hours after the patients were admitted.The 6-minute walking distance and plasma NT-proBNP levels were determined before and after 8 weeks of programmed physical training.The patients of both groups were treated with routine drugs for heart failure.6-minute walk training was only performed in the training group twice a day for 8 weeks.Results Physical training could significantly reduce plasma NT-proBNP levels and improve performance on the 6-minute walking test.Conclusions Physical training could significantly reduce plas- ma NT-proBNP levels and improve the motor function of patients with CHF,and could be helpful in delaying the de- velopment of CHF.
7.Study on the Biological Characteristics of Permethrin-Resistant andSusceptible Strains of Culex pipiens pallens

Objective To compare the biological characteristics of Culex pipiens pallens between permethrin-resistant strain and the susceptible strain. Methods In laboratory, the biological characteristics about bloodsucking, reproduction and development of permethrin-resistant and susceptible strains were observed and recorded. Life table of the experimental populations was constituted. Results The bloodsucking rate of resistant strain was lower than susceptible strain and the difference was significant. The life-span of adults of resistant strain and the developmental duration of eggs, larvae and pupae were longer than susceptible strain and the difference was significant. The death rate of eggs of resistant strain was higher than that of susceptible strain and the difference was significant. The permethrin-resistant strain showed a fitness value of 0 ^58 relative to the susceptible strain. Conclusion The permethrin resistance of Culex pipiens pallens may result in a disadvantage to its reproduction and development.
8.Detection of chemokines in patients with Kawasaki disease and Henoch-Schonlein purpura and its clinical significance

Objective To investigate the changes and clinical significances of the chemokines of interferon-?-inducible protein-10(IP-10),monocyte chemoattractant protein-1(MCP-1) and growth-related oncogene-?(Gro-?) involved in pathagenesis of Kawasaki disease(KD) and HenochSchonlein purpura(HSP).Methods The chemokines production of IP-10,MCP-1 and Gro-? were assayed by ELISA in 15 patients with KD,12 patients with HSP and 10 healthy children.Results The plasma levels of IP-10 and MCP-1 were markedly elevated in KD group [(394.2?176.4)and(420.5?163.4)ng?L-1]compared with HSP group[(94.8?66.4)and(109.2?76.6)ng?L-1] and the control group [(76.4?46.5)and(87.7?47.8)ng?L-1](all P0.05),as well as Gro-? between the three groups.Conclusion Monocyte may enhance the immune damage in KD pathogenesis,and the levels of IP-10,MCP-1 may be important indexes for KD.Neutrophil may be not involved in pathogenesis of HSP and KD.
9.The multiple intervention study on type 2 diabetes in community comprehensive management based on clinical treatment

Objective To evaluate the necessity and practicability of optimizing the management of type 2 diabetes mellitus in community after acquiring the peer education and the music therapy to their physical and mental issues and sleep problems. Methods Totally 179 type 2 diabetes mellitus patients who were followed up in Ruijin 2nd community health service center, the random numbers table used to randomize the patients into 2 groups:control group ( 97 cases) and experiment group ( 96 cases), the conventional treatment was used in control group. Besides the conventional treatment measures, the peer support was used by patients in the experiment group. In the second step, 45 patients were met the inclusion and the exclusion criteria, the random numbers table used to randomize the patients into 2 groups:the multiple intervention group (22 cases) and the conventional treatment group (23 cases), the multiple intervention included the music therapy, the peer support and the sleep health education, the conventional treatment included the conventional treatment and the sleep health education. The t test was used to compare the patient's HbA1c and other quantitative data in two groups of patients after the intervention. Results In the first stage of research, compared with the control group patients, the patients 'HbA1c in intervention group was significantly improvement after 6 months(7.26%±1.37%vs.7.53%±1.63%,t=2.148, P<0.05),besides, the intervention group individuals achieved significant improvement in diabetic self-management behaviors and self-efficacy after 6 months, and the improvement in self-efficacy of peer support group was significant different compared with routinely educated patients(104.09±16.40 vs.110.96± 13.86,t=2.120,P<0.05), and the PHQ-9(5.95 ± 4.02 vs.2.55 ± 1.67,t=2.630,P<0.05)between the two group had significant difference, while no improvement was found in PSQI, BMI, and WHR between intervention group and control group. Conclusions Peer support could improve the blood glucose metabolism in patients with type 2 diabetes. With the effect of yoga music and physical exercises, peer support can improve the quality of sleep and decrease depression in T2DM patients, who also have sleep disorders and mild depression.
10.Effect of transparent cap-assisted endoscopic esophageal foreign body removal

Objective To compare the operation effect of esophageal foreign body removal with transparent cap-assisted endoscopic technique and traditional method. Methods Ninety-six patients with foreign body incarcerated in esophageal were randomly divided into two groups, and they were underwent transparent cap-assisted endoscopic esophageal foreign body removal or traditional endoscopic esophageal foreign body removal respectively. The clinical data of the two groups were collected, and the success rate, the operation time, the endoscopic vision clarity and the complication rate of the two groups were all recorded and analyzed. Results The success rate in transparent cap-assisted endoscopic technique group was 100% compared with 95.83% in traditional method group, and two cases of failure were transferred to transparent cap-assisted endoscopic technique and finally succeed. The mean operation time were (8.58±3.21) min in transparent cap-assisted endoscopic technique group, and (10.64 ± 5.45) min in traditional method group, and there was no significant difference in two groups (P > 0.05). Forty-five cases got A level and three got B in transparent cap-assisted endoscopic technique group in endoscopic vision clarity, in contrast to 35 A, 11 B and two C in traditional method group. The difference between the two groups was statistically significant (P = 0.020). There was no case with serious complications such as bleeding, perforation or death in two groups. Conclusions Transparent cap-assisted endoscopic technique for body removal is safety and efficacy, and this technique can provide better endoscopic operation field of vision and help improve the success rate of esophageal foreign body removal.
