1.MELD-Na score combined with serum cholesterol and endotoxin concentration on the evaluation of the decompensated liver cirrhosis prognosis

Objective To evaluate the prognosis of the decompensated liver cirrhosis by means of MELD-Na score combined with serum cholesterol and endotoxin concentration.MethodsData of 156 hospitalized patients with decompensated liver cirrhosis was retrospectively analyzed.Patients were divided into survival and death group according to follow-ups at 6 months,12 months and 24 months.MELD-Na score was made among 156 patients by detecting relevant indicators.Serum cholesterol and cholesterol levels were measured,too.The relationship between decompensated liver cirrhosis prognosis and MELD-Na score combined with serum cholesterol and endotoxin concentration was analyzed.ResultsNinteen patients died at the follow-up for 6 months.Fifty nine patients died at the followe-up for 12 months.Seventy seven patients died at the follow-up for 24 months.There was significant difference on MELD-Na score,endotoxin concentration and serum cholesterol between the survival group and the death group (t =-9.68,-9.22,11.4,-4.65,-19.60,16.20,-20.0,-18.7,17.3,respectively,P <0.05).The best critical value of MELD-Na score to predicate death in patients with decompensated liver cirrhosis was 32 points.The risk of death would rise if MELD-Na score increased.The best critical value of plasma endotoxin to predicate death in patients with decompensated liver cirrhosis was ≥ 12 ng/L.The best threshold value of serum cholesterol to predict death in patients with decompensated liver cirrhosis was ≤ 1.70 mmol/L.ConclusionMELD-Na score,serum cholesterol and serum endotoxin were of higher prognostic value to judge the prognosis of patients with decompensated liver cirrhosis.

Objective To investigate the possible mechanisms of teratogenesis caused by passive smoking, temporospatial changes in surface carbohydrates of the embryonic neural epithelium and the mesenchyme around it were explored. Methods The pregnant golden hamsters were randomly divided into the treated group and the control groups. In the chamber made by ourself for passive smoking experiment, the animals of the treated group were exposed to smoke from embryonic day 4 to 7, 3 times per day for 4 consecutive days. At twelve o'clock and eighteen o'clock of embryonic day 8, eight o'clock of embryonic day 9 and eight o'clock of embryonic day 10 respectively, the pregnant golden hamsters of both treated group and control group were sacrificed, the uteri were removed and embedded routinely with paraffin, sectioned transversely in series. The sections of each group were stained with immunohistochemical ABC method and concannavalin agglutinin (ConA) and ulex europaeus agglutinin (UEA) were used as molecule probes to conduct the qualitative and quantitative analysis of the fibronectin and the related surface carbohydrates of the embryonic neural epithelium and the mesenchyme around it. 5.Impact of resting heart rate on the progression to diabetes in impaired glucose regulation patients

Objective_ To investigate the effect of resting heart rate on the progression to diabetes in impaired glucose regulation patients. Methods A total of 638 patients with impaired glucose regulation, from January 2011 to December 2012 in our endocrinology clinic, were selected for the study. According to the resting heart rate, patients were divided into four groups:<66 beat/min group, 66 to 69 beat/min group, 70 to 75 beat/min group, and>75 beat/min groups. All patients'baseline data were collected. The incidences of diabetes in different resting heart rate groups were compared, and the relationship between resting heart rate and the progression to diabetes was estimated using multiple regression analysis. Results In 704 patients with impaired glucose regulation, 636 patients have been completed 2 years follow-up, or reached the end of follow-up ( diagnosed as diabetes ) , the follow-up rate was 90. 34%. During two years follow-up, the incidence of diabetes of<66 beat/min group, 66 to 69 beat/min group, 70 to 75 beat/min group, and>75 beat/min group were 16. 2%, 19. 4%, 25. 0%, and 33. 0%, respectivlely. And the Cochran Armitage trend test showed that χ2 =11. 109, P=0. 001, the difference was statistically significant ( P<0. 05). According to blood glucose monitoring, the 636 patients with impaired glucose regulation were divided into impaired fasting glucose group, impaired glucose tolerance group and impaired fasting glucose combined with impaired glucose tolerance group:the Cochran Armitage trend test showed that, with the resting heart rate accelerating, the incidence of diabetes increased. The incidence of diabetes in impaired fasting glucose combined with impaired glucose tolerance group was higher than that of impaired fasting glucose group and impaired glucose tolerance group ( P=0. 062, 0. 113). The average resting heart rate in 68 impaired glucose regulation patients progressed to diabetes was (79.8±8.3)beat/min,andin568non-diabetescases,itwas(74.5±7.2)beat/min(t=-5.043,P<0.01). With the use of patients progressing to diabetes as the dependent variable, different resting heart rate group as independent variables, and resting heart rate<66 beat/min group as a reference, the logistic regression analysis showed that the risk of the progression to diabetes increased with the resting heart rate levels. Conclusion Higher resting heart rate is linked to higher risk of diabetes in patients with impaired glucose regulation.
6.Research on the Status of Standardization of TCM Information

Standards of TCM information plays a fundamental and critical role in TCM informatization. In recent years, the standardization of TCM information has made a series of tremendous progress. This article introduced the major work progress of TCM terminological standardization and TCM data standardization in China, and status of TCM information standardization in International Organization for Standardization and World Health Organization. The existing problems in the construction of standardization of TCM information were discussed and suggestions were proposed to solve problems: to build the basic framework of standard system of TCM information; to cultivate professionals for the researches on the international standards of TCM information; to further explore the theories and methods about standardization of TCM information.
7.Research of TCM clinical terms and the suggestions for system improvement

This study analyzed the current clinical terminology standardization, systematization research status of traditional Chinese medicine(TCM). The previous version of the TCM clinical term system had some problems including imperfect classification structure, unclear relationship between concepts and so on, which made the TCM clinical terminology system(TCMCTS) difficult to support the clinical practice of TCM. The suggestions for system improvement were using ontology method to build TCMCTS concept model bases on the previous researches and data extracted from TCM clinical electronic medical record, and top-level-ontology accordance with ISO standards. The study also tried to summarize the ‘semantic network between classes’ through semantic relationships.
8.Therapeutic Effect of Acupoint Injection with Nucleotide Plus Conventional Western Medicine Treament for Moderate and Severe Chronic Obstructive Pulmonary Disease

Objective To observe the therapeutic effect of acupoint injection with nucleotide plus conventional western medicine treatment for moderate and severe chronic obstructive pulmonary disease (COPD).Methods Sixty patients confirmed as moderate and severe COPD were evenly randomized into treatment group and control group.The treatment group was given conventional western medicine treatment and injection with nucleotide on bilateral Dingchuan points,and the control group was given conventional western medicine treatment alone.The treatment lasted for one year.The frequency of COPD acute onset and hospitalization times within one year were recorded.Meanwhile,the severity of dyspnea was evaluated with Medical Research Council (MRC) dyspnea scale scoring.Results (1)The frequency of COPD acute onset and hospitalization times within one year in the treatment group were lowered,and the differences were significant compared with those in the control group (P < 0.05 or P < 0.01).(2) After treatment,MRC dyspnea scale scores in the treatment group were improved (P < 0.05 compared with those before treatment),but the improvement of the scores was insignificant in the control group(P > 0.05).The intergroup comparison showed that the effect on improving MRC dyspnea scale scores in the treatment group was superior to that in the control group (P < 0.05),indicating that the severity of dyspnea was much improved in the treatment group as compared with the control group.Conclusion Acupoint injection with nucleotide is effective on enhancing the therapeutic effect of conventional western medicine treatment for moderate and severe COPD.
9.Determination of emodin and chrysophanol in Fuyanling Effervescent Tablets by HPLC

AIM: To develop a method to determine the content of emodin and chrysophanol in Fuyanling Effervescent Tablets(Radix et Rhizoma Rhei, Flos Lonicerae, Radix Sophorae Tonkinensis, etc.). METHODS: HPLC was used to determine the content of emodin and chrysophanol in Fuyanling Effervescent Tablets. The separation was performed on YWG ODS column with methanol (0.1%) phosphoric acid(85∶15) mixture as a mobile phase and the wavelength of UV detector was at 254nm. RESULTS: The resolution and the linearity of this method was good. in the range of emodin was 50.2~401.6ng( r =0.9999); chrysophanol was 49.9~ 399.2 ng( r =0.9994); and with the average recovery of emodin: 99.4%( RSD =2.2%); chrysophanol:100%( RSD =1.1%). CONCLUSION: The method is simple, rapid and satisfactory and suitable for quality control of Fuyanling Effervescent Tablets.
10.Off-pump coronary artery bypass in the diabet ic patient-Perioperative clinical analysis

ObjectiveDiabetes is an independent risk factor for coronary artery bypasss grafting(CABG). Off pump coronary artery bypasss (OPCAB) experience in 251 cases was reviewed to determine whether diabetes wou ld be applicable in OPCAB procedures.MethodsConsecutive 251 patients underwent OPCAB over 12 month period. This study included 71 diebetic patients (DM group) and 180 nondiabetic patients (NDM group). Preoperative v ariables were compared between the two groups by univariate analysis.R esultsNo differences were found regarding the length of stay in cardio intensive care unit [DM group(2.4?0.3)d; NDM group (2.4?0.3) d;P=0. 386], and sternal complication (DM group: 5.7%;NDM group: 3.9%;P=0.511) . In hospital complications were as follows: death rate(DM group: 2.8%; NDM gr oup: 1.1%; P=0.680); stroke (DM group: 2 8%; NDM group: 1 7%; P=0 623 ); hemofiltratioin renal failure (DM group: 2.8%; NDM group: 0.5%; P=0.194); myocardial infarction(DM group: 0%; NDM group: 0.5%;P=1.000); blood using were more frequent in DM group comparied with NDM group (P=0.111). ConclusionOPCAB in diabetic patients is as safe as in non diabetic patients.
