1.The analysis of the results of colonoscopy in young patients with chronic constipation

Objective To explore the necessity of colonoscopy in young patients with chronic constipation.Methods Data of patients aged 18-50 underwent colonoscopy at Tianjin Medical University General Hospital with chronic constipation as the sole indication between April 2003 and May 2014 were analyzed.Endoscopic and pathologic reports were analyzed.Results During the study period,a total of 563 patients were included,who were aged 18-50 with chronic constipation as the sole indication,of which 260 patients were aged 18-35,and 303 patients were aged 36-50.No lesion was found during colonoscopy in 167 (29.7%) patients,whereas in other 396 (70.3 %) patients positive findings were reported,including polyps in 45 patients (of which 13 were with multiple polyps),adenomas in 20(17 in distal colon,3 in proximal colon).In patients aged 18-35,3 cases of adenomas(3/260,1.2%) were found,of which 1 patient (1/260,0.4%)had advanced adenoma.In patients aged 36-50,17 cases of adenomas(17/303,5.6%) were found,of which 4 (4/303,1.3%) were advanced ones.Colorectal cancers were found in 2 patients (0.7%,2/303),both in patients aged 36-50.The detection rate for colorectal neoplasms (including adenoma and cancer) in patients with chronic constipation aged 18-35 was significantly lower than that in patients aged 36-50[1.2%(3/260) VS 6.3%(19/303),P=0.002,95%CI:0.05-0.60].Conclusion The detection rate for colorectal neoplasms in patients aged 18-35 years with chronic constipation is relatively low,and colonoscopy is not recommended for them.
2.Anemia prevalence and its influencing factors for the infants aged 6-23 months in poor areas in Gansu Province

Objective To understand the prevalence of anemia and its influencing factors in infants aged 6 to 24 months in poor areas of Gansu Province, and to provide reference for improving the prevalence of anemia in local children. Methods A multi-stage sampling method was used to conduct a survey on infant and caregivers, in the 12 children nutrition improvement project counties in Gansu Province. T-test and analysis of variance were used to compare the hemoglobin content, the chi-square test was used to compare the prevalence of anemia, and the Logistic regression model was used to analyze the influencing factors. Results Among the 3 188 effective data, the detection rate of anemia was 25.69% (819). The detection rate of anemia among boys and girls was 24.54% and 26.90%, respectively. There was no significant difference in the detection rate of anemia among different sexes ( 2=2.326, P=0.127). The detection rate of anemia between different age groups were statistically significant ( 2=42.339, P<0.001); The results of multivariate analysis showed that children's age, children's ethnic groups, parents' awareness of feeding knowledge, the feeding method of 6 months after birth and the way of taking nutritional packs were associated with anemia (all P<0.05). Conclusions The prevalence of anemia in infants aged 6 to 24 months in poor areas in Gansu Province was relatively high. Strengthening the education of caretakers’ knowledge of guardian feeding and scientific child-rearing, and ensuring the nutrition packages intake can significantly reduce the prevalence of anemia prevalence in poor areas in Gansu Province.
3.Clinical trial of piperazine ferulate dispersed tablets joint clopidogrel in the treatment of coronary heart disease

Objective To research the effect of piperazine ferulate dispersed tablets joint clopidogrel in the treatment of coronary heart disease.Methods Patients with coronary heart disease was from January 2015 to December 2016 as the study subjects,according to random number table method they were divided into control group and treatment group,44cases in each group.Control group was treated with clopidogrel,preoperative acceptance of 75 mg clopidogrel,qd,and continuous treatment for 6 months.Treatment group was treated with piperazine ferulate dispersed tablets based on the control group,oral 100 mg of piperazine ferulate dispersed tablets,tid,continuous treatment for 1 month.Clinical effects,serum homocysteine (HCY),hypersensitive C-reactive protein (HsCRP),myeloperoxidase (MPO),ischemia modified albumin (IMA),and adverse drugs reactions(ADR) between two group was compared.Results Total effective rate of the treatment group and control group respectively were 90.90% (40 cases/44 cases) and 72.72% (32 cases/44 cases),the difference between groups was significant (P < 0.05).After treatment,the serum HCY in treatment group and control group respectively were (12.48 ± 1.91) and (16.36 ±2.02) μmol · L-1;HsCRP in the two groups respectively were (1.34 ±0.17) and (3.82 ±0.56)mg · L-1,MPO in the two groups respectively were (17.36 ±2.88) and (23.26 ±4.26)mmol · L-1,IMA in the two groups respectively were (20.10 ± 2.44) and (24.91 ± 3.53) μg · L-1,the difference of above factors between groups was significantly (all P < 0.05).Incidence of ADR in treatment group and control group respectively were 25.00% (11 cases/44 cases) and 20.45% (9 cases/44 cases),there was no significant difference (P >0.05).Conclusion Effect of piperazine ferulate dispersed tablets joint clopidogrel in the treatment of coronary heart disease is better than clopidogrel alone,which reduced serum HCY,HsCRP,MPO and IMA and slowed down the disease.
4.Development and study of three-dimensional CT scanning system for dental cast measurement and analysis.

OBJECTIVETo develop a computer assisted CT scanning system for 3D dental cast measurement and to study its reliability.
METHODS20 dental stone casts were scanned by helical CT and captured a series digital sliced radiographs of them. The program which was developed by Visual C 6.0 language and run under the Windows surroundings rebuilt the 3D graphics and measured the coordinate of the dental cast. These dental casts were scanned and measured to test the reliability of the system, contrasting with manual measurement.
RESULTSThere was no significant difference between 3D CT scanning and manual measurement (P > 0.05).
CONCLUSIONThe system is a valuable computer assisted system for clinical diagnosis and treatment of malocclusion.

5.Recombinase polymerase amplification combined with a lateral flow dipstick for rapid and visual detection of adenovirus

Objective To develop a rapid, accurate, visual, and portable detection method for adenovirus types B (AdvB) and E ( AdvE).Methods Universal primers were targeted on type-specific conserved regions to allow the simultaneous detection of both human Adv (HAdV) species.A detection method based on the combination of recombinase polymerase amplification ( RPA) and lateral flow dipstick ( LFD) was established the sensitivity and specificity evaluated , and throat swab specimens of 19 patients infected with AdvB and AdvE as well as 10 healthy volunteers were detected with this method.Results The detection limit of the method was 10 copies/μl Adv DNA, which was close to that of qPCR , and there were no cross-reactions with other species of Adv and unrelated virus .The detection could be finished within 15 to 20 min within the temperature range of 25 to 45℃.When applied to clinical samples , this method showed 100% sensitivity and specificity.Conclusion This detection assay is a sensitive , specific, rapid and simple method that eliminates the need for expensive equipment , trained personnel or laboratories .The characteristics of this system render it suitable for use in grass-roots healthcare departments , and the system is especially effective for field testing and on-site testing.
6.Association between Retinal Vascular Geometric Changes and Cognitive Impairment: A Systematic Review and Meta-Analysis

BACKGROUND:
AND PURPOSE: Previous studies have explored the association between retinal vascular changes and cognitive impairment. The retinal vasculature shares some characteristics with the cerebral vasculature, and quantitative changes in it could indicate cognitive impairment. Hence, a comprehensive meta-analysis was performed to clarify the potential relationship between retinal vascular geometric changes and cognitive impairment.
METHODS:
Relevant databases were scrupulously and systematically searched for retinal vascular geometric changes including caliber, tortuosity, and fractal dimension (FD), and for cognitive impairment. The Newcastle-Ottawa Scale was used to evaluate the methodological quality of included studies. RevMan was used to perform the meta-analysis and detect publication bias. Sensitivity analyses were also performed.
RESULTS:
Five studies that involved 2,343 subjects were finally included in the meta-analysis. The results showed that there was no significant association between central retinal artery equivalents (Z=1.17) or central retinal venular equivalents (Z=1.74) and cognitive impairment (both p>0.05). Similarly, no significant difference was detected in retinal arteriolar tortuosity (Z=0.91) and venular tortuosity (Z=1.31) (both p>0.05). However, the retinal arteriolar FD (mean difference: −0.03, 95% CI: −0.05, −0.01) and venular FD (mean difference: −0.03, 95% CI: −0.05, −0.02) were associated with cognitive impairment.
CONCLUSIONS
A smaller retinal microvascular FD might be associated with cognitive impairment. Further large-sample and well-controlled original studies are required to confirm the present findings.
7.Natural killer-like T-cell lymphoma/leukemia--a case report and literature review.

OBJECTIVETo identify the clinical and pathological features of natural killer-like T-cell lymphoma/leukemia.
METHODSThe characteristics of natural killer-like T-cell lymphoma/leukemia was discussed with report a new case and review of literatures.
RESULTSA 16-year-old girl was referred to our hospital because of fever and disseminated cutaneous herpes and ulcer. Atypical lymphoid cells surrounded the dermal vessels with a CD3(+), CD8(+), CD4(-), CD5(-), CD10(-), CD19(-), CD57(-), CD56(+), perforin(+), granzyme B(+) immunophenotype and rearranged T-cell receptor-gamma gene implicated natural killer-like T cell origin. She was treated with prednisone and for several months. Then the patient developed progressive spleen enlargement with overt leukemia, which led to her eventual death.
CONCLUSIONSNatural killer-like T-cell lymphoma/leukemia is a rare disease with distinctive clinical, histopathologic, and immuno phenotypic characteristics. Current treatment modalities are ineffective for most of the patients.

8.Laparoscopic adhesiolysis versus open surgery for adhesive small bowel obstruction: A meta-analysis

Objective To systematically evaluate the efficacy and safety of laparoscopic adhesiolysis versus open surgery for adhesive small bowel obstruction.Methods The databases including PubMed,EMbase,The Cochrane Library (Issue 3,2017),ICTRP,CNKI,VIP,CBM and Wanfang Data were electronically searched to collect randomized controlled trials (RCTs) about laparoscopic adhesiolysis versus open surgery for adhesive small bowel obstruction patients from inception to March 2017.Two reviewers independently screened literature,extracted data and assessed the risk of bias of included studies.Then meta-analysis was conducted by RevMan 5.3 software.Results A total of 31 studies involving 3293 patients were included.The results of meta-analysis showed that,compared with the open surgery group,the laparoscopic adhesiolysis group could significantly reduce the recurrence rate (OR=0.18,95%CI 0.12-0.25,P<0.000 01),overall complications (OR=0.17,95%CI 0.13-0.23,P<0.000 01),wound infection (OR=0.21,95%CI 0.13-0.35,P<0.000 01),pneumonia (OR=0.35,95%CI 0.15-0.82,P=0.02) and intestinal fistula (OR=0.32,95%CI 0.15-0.70,P=0.005).Conclusions Current evidence shows that laparoscopic adhesiolysis can reduce the recurrence rate and complications in adhesive small bowel obstruction.Due to the limited quality of included studies,more high quality studies are needed to verify the above conclusion.
9.Risk Assessment of Echocardiography Parameter Scoring System for 1 Year Re-admission in Patients With Left Ventricular Systolic Dysfunction

Objective: To establish an echocardiography parameter scoring system for assessing the risk of 1 year re-admission in patients with left ventricular systolic dysfunction (LVSD). Methods: A total of 412 chronic LVSD patients treated in our hospital from 2007-01 to 2016-01 were studied and the end point event was 1 year re-admission. The data included in 280 patients from 2007-01 to 2014-12 for establishing the scoring system and 132 patients from 2015-01 to 2016-01 for verifying the system. Based on 7 echocardiography parameters, the patients were divided into 7 sets of groups: ① Left ventricular diameter (LVD): Group0, n=290 and Group1, n=122;② Mitrial regurgitation (MR): Group0, n=203, Group1, n=138 and Group2, n=71; ③ Tricuspid regurgitation (TR): Group0, n=302, Group1, n=90 and Group2, n=20; ④ LVEF: Group0, n=272 and Group1, n=140; ⑤ Pulmonary artery systolic pressure: Group0, n=282 and Group1, n=130; ⑥ Hydropericardium: Group0, n=347 and Group1, n=65; ⑦ Hydrothorax:Group 0, n=261, Group1, n=86 and Group2, n=65. The parameters were identified by COX regression analysis, weighted value of scoring system was calculate by hazard ratio (HR), predictive value for1 year re-admission was assess by ROC curve and finally, scoring integration was verified by validation data group. Results: The integration score was calculated as follows: LVD>60mm=1 point; TR: Group1=1 point and Group2=3 points; MR: Group1=2 points and Group2=4 points; Hydrothorax: Group1=2 points and Group2=3 points;Hydropericardium=1 point. COX regression analysis indicated that for 1 year re-admission: HR=1.552 in Group1 vs Group0, HR=3.374 in Group2 vs Group0 and HR=4.562 in Group3 vs Group0, all P<0.05. The AUC of ROC for establishing the data was 70.0% (95% CI 0.640-0.761) and for verifying the data was 70.4% (95% CI 0.616-0.792); the best integration score was 4 points. Conclusion: Echocardiography parameter scoring system may better predict the risk of 1 year re-admission in LVSD patients which is superior to single echocardiography parameter.
10.Protective effect of lytic cocktail (inhibition of stress) on lung injury in severe burn rat.

OBJECTIVETo investigate the protective effect of inhibition of stress (lytic cocktail) on lung injury in severe burn rats at early stage.
METHODSSprague-Dawley rats inflicted with 30% TBSA full-thickness burn were randomly divided into A group (n = 36, fluid resuscitation with administration of lytic cocktail), B group (n = 36, fluid resuscitation only). Lung function was evaluated by partial pressure of oxygen (PaO2) in arterial blood and histopathologic changes on 3, 5, 7, 10 post burn day (PBD). The levels of malonyldialdehyde (MDA), myeloperoxidase (MPO), tumor necrosis factor-alpha(TNF-alpha) and interferon-gamma (IFN-gamma) in lung tissue were measured at the same time points.
RESULTSThe PaO2 level in A group on 3 PBD (12.58 +/- 0.41 kPa) was significantly higher than that in B group (8.86 +/- 0.23 kPa, P < 0.01). Compared with those in B group, the levels of MDA and MPO were significantly decreased in A group at each time point (P < 0.05 or 0.01), the levels of TNF-alpha on 3, 5, 7 PBD (P < 0.05 or 0.01) and IFN-gamma on 5, 7, 14 PBD (P < 0.01) were also decreased in A group. Swollen lung mesenchyme was alleviated, infiltration of inflammatory cell was lessened in A group.
CONCLUSIONLytic cocktail combined with immediate fluid resuscitation can inhibit stess response, downregulate the expression of inflammatory factor, ameliorate lung function in severe burn rat at early stage.
