Urodynamics*

Hemoglobin(Hb), zinc protoporphyrin(ZPP) and blood lead(PbB) levels were determined for 1,851 blood samples collected from healthy urban population to establish reliable baselines for Hb, ZPP and PbB levels by age and sex. ZPP values were analyzed with a Hmatofluorometer and PbB determinations were concurrently carried out using flameless atomic absorption spectrophotometry. The blood sampling period was about 6 months from May, 1991 and the summarized results were as follows; 1. The mean value of Hb in male female were 14.55+/-1.81 g/dl and 12.61+/-1.81 g/dl respectively and there was statistically significant difference(p<0.05). 2. The mean value of ZPP in pre-schoolchildren was 37.49+/-13.31 microgram/dl for male, 35.77+/-11.85 microgram/dl for female and that of ZPP in after 7 years groups was 31.91+/-8.23 microgram/dl for male, 30.11+/-9.11 microgram/dl for female and there was statistically significant difference(p<0.05). 3. The mean value of PbB in pre-schoolchildren was 25.10+/-5.21 microgram/dl for male, 24.45+/-4.18 microgram/dl for female and that of PbB in after 7 years groups was 24.28+/-3.00 microgram/dl for male, 21.99+/-5.05 microgram/dl for female and there was statistically significant difference(p<0.05).

Activation of T cells for an immune response requires the participation of antigen presenting cells (APC) that express class II major histocompatibility complex gene products on their surface. Until recently, the macrophages have been considered to be the prime candidates for this role, but it is now recognized that other cells, including dendritic cells, B cells, activated T cells and endothelial cells, can present antigen effectively. Particularly, among them, dendritic cells (DC) are considered to be very efficient APC for various T -cell dependent immune responses in comparison with other types of APC. Nonlymphoid dendritic cells including Langerhans cells and interstitial dendritic cells strongly express the MHC class II products and have characteristic dendritic morphology. As far as we know, there is no study on the ontogeny of MHC class II -immunoreactive dendritic cells in the rat tongue. The aim of the present study is to investigate the ontogeny and morphological characterization of dendritic cells in the tongue of growing and developing rats. The distribution and morphology of the dendritic cells in the rat tongue were studied from the fetal 15 -day until 180 days after birth by means of immunocytochemical methods using anti -rat MHC class II monoclonal antibodies. The results were as follows: 1. MHC class II -immunoreactive dendritic cells were first found in the muscle layer of 17 -day fetus, and in the epithelium and lamina propria of the tongue at birth. 2. The number of MHC class II -immunoreactive dendritic cells was gradually increased with age, particularly, in the epithelium at 14th day, in the lamina propria at 14th day, and in the muscle layer at 21st day after birth. 3. Numbers of MHC class II -immunoreactive dendritic cells were higher in the dorsal part than in ventral part of the tongue at all developmental stages. Especially, dendritic cells were twice higher numbers in the epithelium, a little higher numbers in the lamina propria and almost same numbers in the muscle layer. 4. With age, shapes of MHC class II -immunoreactive dendritic cells were changed from round to dendritic and aggregated together. In conclusion, the above results suggest that the increases of the number and the changes of the morphology in MHC class II -immunoreactive dendritic cells, with age may influence on effects of cell -mediated immune responses.

Myoclonic-astatic epilepsy is an epileptic syndrome characterized bv the presence of myoclonic and astatic seizures. It is a rare generalized epilepsy of childhood and represents 1 to 2% of epilepsis in children up to the age of 9 years. In addition to myoclonic and astatic seizures, generalized tonic-clonic seizure, tonic seizure and absence status (non-convulsive status) can be combined. Non-convulsive status can produce apathetic and stuporous appearance. We represent 4-year-old boy who showed nonconvulsive status of myoclonic-astatic epilepsy with typical clinical and eleckoencephalographic characteristics.

We studied the left ventricular function with the use of M-mode and 2-D Echocardiography to 32 patients with congestive heart failure before and 2 weeks after medication of prazosin 2 mg tid during July 1979 and Dec. 1980 in Heart Center, Paik Hospital, In-Je Medical College, Busan, Korea. The results were following; 1) LVDeD before and after vasodilaor therapy was each 5.64+/-0.76 and 5.04+/-0.57.cm, LVSeD before and after therapy each 4.52+/-0.51 and 3.70+/-0.57cm, and F.S. before and after therapy 19.87+/-3.66 and 26.59+/-6.75%. The difference of LVSeD, LVDeD and F.S. was each statistically significant. 2) LVDeV before and after therapy was each 182.2+/-58.07 and 132.80+/-47.01 ml LVSeV before and after therapy each 92.35+/-23.60 and 50.65+/-27.21ml, and E.F. before and after therapy each 49.31+/-7.07 and 61.86+/-11.74%. The difference of LVDeV, LVSeV and E.F. was each statisitically significant. 3) LVPWTh before and after therapy was each 0.89+/-0.11 and 0.77+/-0.08cm, and Vcf before and after therapy 0.83+/-0.11 and 1.03+/-0.27 cir./sec. The difference of LVPWTh and vcf before and after therapy was each statistically significant. 4) The difference of VSTh, RVD, LAD, and Aorta Dimension before and after therapy was each statisitically significant.

The majority of hypospadias repairs can now be accomplished in one-stage, providing a better result with reduced morbidity and hospitalization costs. One-stage repair of hypospadias using Hodgson`s type III urethroplasty was performed on 22 of 47 cases with hypospadias treated at the Departrnent of Urology, Chonnam University Hospital during 10 years between 1976 and 1985. The following results were obtained. 1. The age of all 22 patients ranged from 4 to 30 years at the time of operation and about a half of the patients were in the age group between 6 and 10 years. 2. The most common type of hypospadias was penoscrotal one (55%) 3. Associated anomalies were bifid scrotum in 5 cases (23%) and cryptorchidism in 3 (14%). 4. Urinary diversion was performed on all cases with suprapubic Troca Cystocath in 16 cases (73%) and indwelling urethral silastic catheter in 6(27 %). 5. Postoperative complications were partial skin necrosis in 1 cases (4%), urethrocutaneous fistula in 6 (27%) and urethral stricture in 3 (14%). 6. Opening of the external urethral meatus after surgery was unsatisfactory in 5 cases, of which 3 underwent meatal advancement later. Tumors of the epididymis are rare. The most common one is adenomatoid tumor. At least more than two hundred cases have been reported in the literature but only one was reported by us last year in this country. We add another one which is involving bilateral epididymis simultaneously and review literature briefly.
