1.Changes in the expressions of CD63 on basophils in patients with penicillin allergy

Aim To evaluate the value of expressions of CD63 on basophils in diagnosing allergy to penicillins.Methods The expressions of CD63 on basophils activated with 9 kinds of antigens(PG:penicillin G,PV:phenoxomethylpenicillin,AMP:ampicillin,AX:amoxicilloyl,6-APA:6-aminopenicillanic acid,PHA:phenylacetic acid,PHOA:phenoxacetic acid,PHPG:N-(?-hydroxyphenyl)glycine,NPG:N-phenylglycine) were detected by FAST(Flow cytometric allergen stimulation test) in 43 patients with penicillins allergy.Eight types of specific IgE and IgG antibodies to penicillins were determinated by RAST and ELISA,respectively.Results The sensitivity and specificity of CD63 used as a marker in patients with penicillins allergy were 65.12% and 93.33%,respectively.Moreover,the sensitivities of CD63 and specific IgE were higher than those of specific IgG in patients with positive skin test(P
2.New drug delivery systems and the function of camptothecin

Camptothecin is known to possess a broad spectrum of anticancer activities, however due to its high toxicity and low solubility in water, camptothecin is severely limited in clinic. To optimize the effects of camptothecin in the treatment of malignancies, besides the structural modification and reconstruction of camptothecin, the establishment of new drug delivery systems becomes a hot topic in the camptothecin research. This paper aims to summarize the advanced progress in the delivery system and function of camptothecin at recent years. The advances in improving the poor solubility, lactone ring instability, controlled release systems and targeting delivery systems of camptothecin are also reviewed. As the related methods and technologies are improved and developed, camptothecin will be a group of prospective anticancer drugs.
3.IDENTIFICATION OF PRE-X PROMOTER SEQUENCE IN HEPATITIS B VIRUS GENOME AND APPRAISAL OF ITS TRANSCRIPTION ACTIVITY

It was found that pre-X region was present in five clones of hepatitis B virus (HBV) genome from 2 patients with chronic HBV infection. The open reading frame (ORF) consisted of 168 base pair (bp), and could be transslated with the X gene in frame. To investigate the activity of pre-X gene promoter, promoter DNA sequence was amplified from HBV-DNA by polymerase chain reaction (PCR), The amplified product was cloned into pCAT3 vector, The HepG2 cells were transfected by pCAT3-Pre-X-p. The CAT activity was detected by an enzyme-linked immunosorbent assay (ELISA) kit. It was found that pCAT3-Pre-X-p had higher activity of CAT, which was 3.5 fold over that of pCAT3-promoter. This result implicated that pCAT3-Pre-X-p possessed promoter activity.
4.DEFINITION OF PRE-PRE-S PROMOTER SEQUENCE FROM HEPATITIS B VIRUS GENOME AND IDENTIFICATION OF ITS TRANSCRIPTION ACTIVITY

To investigate the promoter activity of pre-pre-S gene, which was found in five clones of hepatitis B virus (HBV) genome from 2 patients with chronic HBV infection, promoter DNA sequence was amplified from HBV-DNA by polymerase chain reaction (PCR).The amplified product was cloned into pCAT3 reporter vector. The HepG2 cells were transfected by pCAT3-pre-pre-S-p. The CAT activity was detected by an enzyme-linked immunosorbent assay (ELISA) kit. It was found that pCAT3-pre-pre-S-p had higher activity of CAT which was 10 fold over that of pCAT3-basic. This result implicated that pCAT3-pre-pre-S-p had promoter activity.
5.The study on perfusion flow of cardiopulmonary bypass during acute Stanford type A aoritc dissection procedure

Objective To research the reasonable perfusion flow of cardiopulmonary bypass during aortic arch procedure of patients with acute type A aortic dissection.Methods Forty patients suffered from acute Stanford type A aortic dissection had been divided into two groups randomly.Group A named traditional perfusion flow group,group B named modified perfusion flow group.Monitoring cerebral blood flow and cerebral tissue oxygen during deep hyperthermia circulatory arrest and antegrade aelective cerebral perfusion procedure by transcranial doppler(TCD) and near-infrared spectroscopy(NIRS).The concentration of S100 protein and lactic acid was measured at six time point.Results Statistical difference of mean blood flow velocity of MCA had been found between two group 3 min after total flow reperfusion.TOI was more tban 60% during study in both of groups.S100 protein in group A was significantly higher than group B at T6,T7 and T8.Statistical difference of blood lactic concentration had been found between two groups,(4.88± 1.62) mmol/L in group A,(3.83± 1.48) mmol/L in group B,P < 0.05.Safe consciousness time between two groups was difference,(7.36± 2.86) h in group A and (5.27± 3.11) h in group B,P < 0.05.Conclusion Compared with the traditional perfusion flow,modified perfusion flow can provide sufficient cerebral perfusion and prevent the luxury perfusion.
6.Achillis Tendon-Sliding Procedure and Subtalar Joint Capsula Release by the Same Cut and Individualization in Treatment of Congenital Clubfoot

Objective To explore achillis tendon-sliding procedure and subtalar joint capsula release by the same cut and individualization treatment used in the correction of congenital clubfoot.Methods Forty eight cases (76 feet) of congenital clubfoot who were first visit were treated by achillis tendon-sliding procedure and subtalar joint capsula release by the same cut,meantime,footplate fascia release and transfer of the anterior tibial tenden were made by individualization.Results All cases were followed-up,the average time follow-up was 1 year and 6 months,excellent were 71% and good were 15.7%.Five cases were recurrence and its rate was 6.94%.Conclusions Achillis tendon-sliding procedure and subtalar joint capsula release by the same cut can solve rotation of displacement between calcaneus and talus and individualization treatment can also solve high arch deformity and adduction of foot.This technique is a simple,safe and effective treatment for congenital clubfoot.
8.Retrospective analysis of 91 patients with T cell non-Hodgkin's lymphoma

ObjectiveTo analyse treatments and prognostic factors of T cell non-Hodgkin lymphoma (T-NHL). MethodsNinety-one patients with T-NHL were retrospectively analyzed, and clinical features,histopathology, laboratory data were included in Kaplan-Meier and prognostic analysis. Results Median age was 38 years,58 (63.7 %) had high-intermediate and high risk by IPI,72 (79.1%) presented with advanced stage disease,extranodal disease was present in 64.8 % of patients.The overall response rate (ORR) for the whole group was 63.8 %,and the estimated 3,5-year ORR were 55.5 %,41.3 % respectively.Compared with CHOP-like regimen, the addition of etoposide could improve the survival of patients, meanwhile radiation therapy could improve the outcome of patients with NK/T cell lymphoma and mediastinal bulky disease, and consolidation chemotherapy with HSCT could improve the survival and reduce the recurrence of patients.Clinical stage,B symptoms,ECOG score,the level of LDH,extranodal involvment,anemia,initial treatment outcome, IPI score, the level of serum albumin and fibrinogen were predictive to overall survival. Cox multivariate analysis showed initial treatment outcome and B symptoms were independent prognostic factors.IPI and m-PIT were useful for stratified patients into different prognostic risk groups. Conclusion T-NHL is a heterogeneous group of malignancies with an inferior long term outcome. New treatment modality needs to be explored for these patients,and new drugs and HSCT may be good choices.
10.Drug therapy for advanced hepatocellular carcinoma: approach, controversy and progress

For unresectable advanced hepatocellular carcinoma (HCC),besides sorafenib,alternative drugs and treatment modalities are required.Clinical studies of hepatic arterial infusion chemotherapy (HAIC),transcatheter arterial chemoembolization (TACE),and system chemotherapy have shown favorable efficacy and tolerance in advanced HCC patients.In addition,the potential efficacy of sorafenib combined with focal treatment is also an interesting issue.As more therapies become available,decision-making for treating advanced HCC becomes increasingly complex.In our opinion,diverse treatment modalities should be utilized for the best interest of patients.Based on predictive biomarkers,we should develop a precise patient stratification system to select suitable candidates for each treatment modality in future studies,as is useful for improving prognosis of patients with advanced HCC.
