2.CT Findings of Solitary Tuberculoma with a Cavity.

PURPOSE: Differential diagnosis of solitary pulmonary nodule with cavity includes lung abscess, tuberculoma, bronchogenic carcinoma, metastasis and trauma, etc. We analyzed the CT appearance of tubercuioma presenting as a solitary pulmonary nodule with cavity and describe the findings which suggest tuberculoma in the differential dignosis of soliary pulmonary nodule with cavity. MATERIALS AND METHODS: 25 patients with solitary pulmonary nodule(diameter less than 4 cm) without surrounding parenchymal consolidation on chest radiograph, who had a cavity within the nodule on CT, were included in our study. Density of the nodule, maximal wall thickness, the character of inner and outer wall margin, location of cavity within the nodule, location of the nodule, presence or absence of satellite lesions and calcification were analyzed. RESULTS: Solitary tuberculoma with cavity showed maximal wall thickness more than 15 mm in 40%(10/25) and 5-14 mm in 56%(14/25), eccentric cavitation in 84%(21/25) and concentric cavitation in 16%(4/25), spiculated outer wall margin in 56%(14/15) and Iobulated margin in 32%(8/25), smooth inner wall margin in 60%(15/25) and nodular margin in 40%(10/25). CT density of the cavity wall compared with the chest wall muscle was low in 84%(21/25) and isodense in 16%(4/25). Accompanying satellite lesions were seen in 84% (21/25) and calcification was visible in 28%(7/25). CONCLUSION: The CT findings of solitary tuberculoma with cavity are relative peripheral location, eccentric cavitation, finely spiculated outer wall margin, and mean maximal wall thickness of 13.2 mm, which are also the common features of malignant nodule. However, relative low density of the nodule compared to the chest wall muscle and surrounding satellite lesions can be additional clues favoring solitary tuberculoma with cavity on CT.

3.The Effect of Regional Sympathetic Block in the Treatment of Herpes Zoster .

We must be assured that sympathetic ganglion block interrupts a vicious cycle of nerve impulses. Therefore, it prevents vasospasm and improves local circulation. The sympathetic ganglion block is effective in acute stage of herpes zoster and reduces the incidence of post-herpetic neuralgia. Herpes zoster is more often involved in an immunosuppressive imbalance. Three cases of herpes zoster were treated by repeated sympathetic ganglion block with 1% proved. Duration from onset of pain was 1 week, 1 month and 2 months, respectively. The block decreased the degree of pain markedly and dried up the vesicle in the early stage. Two of these three cases had suffering from diabetes melitus.

4.Hypercarbia Due to Mistaken Supply of Carbon Dioxide Originating from Nitrous Oxide Gas Tank: A case report.

A 49 year-old male was scheduled for a cholecystectomy, thereafter a 37 year-old female scheduled for removal of a epidural hematoma in the same operating room. Both of them had no specific medical problems and past medical histories for anesthesia. For those reasons, anesthesia was induced with thiopental sodium and succinylcholine with endotracheal intubation. After induction, vital signs including body temperatures were stable. But moisture dew in the unidirectional valves and corrugated tubes, and color changes of soda lime were discovered. At that time, severe hypercarbia was recognized by arterial blood gas analysis in both cases. In both cases, there were no malfunctions in unidirectional valves, expiratory valves, corrugation tubes, soda lime, ventilators and there connection parts in the anesthetic machines. Also there were no abnormalities of blood pressures, electrocardiograms, pulse oxymeters, temperatures and the pulse in the patient monitoring systems except capnography. At first, we thought that medical signs revealed malignant hyperthermias. But vital signs, air way pressures and functions of all kinds of anesthetic machine components including ventilators were normal. After discontinuing N2O gas deliveries in the operation room, hypercarbias disappeared. Thus, anesthetic gas delivery systems via central piping systems were checked and it was discovered that CO2 gas was in the N2O gas tank instead of N2O.

5.p53 Expression and Ki-67 Labeling Index in Brain Tumor with Special Reference to Tumor and Histologic Grade.

Mutation in the p53 suppressor gene is the most common genetic alteration found in human cancers including primary brain tumors. Ki-67 labeling index(LI) is known to be a marker of proliferating activity. The purpose of this study was to verify whether an immunohistochemical expression of p53 antibody and Ki-67 LI could be related to different clinicopathologic parameters including histologic grade, size, invasiveness and recurrence of the brain tumors. Materials were based on the 147 surgically resected brain tumors during the last two years. Of the 147 brain tumors, there were 35 astrocytic tumors, 35 meningiomas, 10 oligodendrogliomas, 7 craniopharyngiomas, 5 dysembryoplastic neuroepithelial tumors, 4 medulloblastomas, 5 ependymomas, 23 pituitary adenomas, 9 schwannomas, and 14 other brain tumors. The p53 expression and Ki-67 LI were higher in malignant brain tumors including astrocytic tumors, medulloblastoma, PNET and gliosarcoma. The p53 positivity was correlated with histologic grades and tumor recurrence. The brain tumors with a high Ki-67 LI(>6%) also showed a close relationship to a higher histologic grading, radiological invasiveness and recurrence. There was no evident correlation with the age and tumor size with p53 expression and Ki-67 LI. These results suggest that p53 overexpression and high proliferation potential of the tumor cells are associated with the higher histologic grade and aggressive clinical course in the central nervous system tumors.

8.Intraneural Ganglion of the Peroneal Nerve: A Case of Report

Intrsneural ganglion cysts of the peroneal nerve are rare. The precise etiology and pathological changes remain obscure. We have experienced recently a case of intraneural ganglion cyst developed in the sheath of the peroneal nerve. A 28 year old male had suffered from lump producing pain over the posterolateral aspect of the upper portion of the fibula, weakness of dorsiflexion of the ankle and of extension of the toes and diminution of sensation on the dorsum of the foot. He underwent excision of the ganglion and the prognosis for recovery of function is good.

10.An Experimental Study about the Effects of Parathyroid Hormone on Strength of Vertebra and Bone Density of Proximal Tibia in Ovariectomized Rats.

Parathyroid hormone (PTH) has not been studied as extensively as therapeutic agents of osteoporosis, but the anabolic effect of parathyroid hormone on the skeleton has been demonstrated in several studies. The current study was undertaken to assess the effect of parathyroid hormone on bone mass and its biomechanical competence using ovariectomized rat model. Fifty female Sprague Dawley rats of 3 months old were randomized into five groups, 10 in number for each groups. Group I: sham-operation, Group II: ovariectomy (OVX) plus saline injection, Group III: OVX plus PTH 20ug/kg, Group IV: OVX plus PTH 40ug/kg, Group V: OVX plus PTH 80ug/kg. Treatment regimens were initiated 8 weeks after ovariectomy and continued for 4 weeks thereafter. Bone mineral density was measured in proximal one-third level of tibia by dual energy X-ray absorptiometry (QDR-2000, Hologic, U.S.A). Biomechanical competence was assessed in lumbar vertebral body by universal testing machine (Instron-4467, U.S.A). The average of bone mineral density in sham operated group was 0.088+/-0.0084g/cm (2), and in control group (OVX +/- saline)was 0.065+/-0.0095g/cm (2). Bone mineral density decreased in control group significantly. The average of bone mineral densities in Group III, IV, V (OVX + PTH) were 0.071+/- 0.0089, 0.081+/-0.0086 and 0.084+/-0.0093g/cm (2), respectively. This showed that parathyroid hormone injected groups had significantly higher bone mass than control group (p<0.0#5), and the increment was in proportion to the amount of parathyroid hormone in the range (p<0.05). The average of biomechanical competence of lumbar vertebral body in sham operated group was 30.036.24 MPa and was 25.11+/-5.69 MPa in control group. In Group III, IV,V they were 25.74+/-6.77, 27.96+/-5.17 and 29.83+/-5.79 MPa, respectively. Above results seem supportive of the phenomenon that parathyroid hormone exert anabolic effect on osteoporotic bones in certain experimental condition.
