1.Navelbine in the Treatment of Recurrent and Metastatic Breast Cancer.

Objective To evaluate the efficacy and adverse reactions of Navelbine(NVB)in the treatment of recurrent and metastatic breast cancer.Methods 34 patients with recurrent and metastatic breast cancer were treated with Navelbine in this clinical study,and 16 patients were treated by the NP(DDP+NVB)regimen,12 patients by the NA(ADM+NVB)regimen,6 patient by single-agent NVB.34 patients received above regimens for two cycles(3~4 week a cycle).Result The overall response rate(CR+PR)was 35.3%,clinical benefit rate(CR+PR+SD≥6 months)44.1%,disease contrrol rate(CP+PR+SD)58.8%,the median time to failure 3.4 months(range:0.7~11 months),the median time to progression(TTP)3 months(range:0.8~11 months),and the median duration of response(CP+PR)5 months(range:2~11 months).The main side effect of the therapies is bone marrow depression,and the grade Ⅲ~Ⅳ leucopenia is 35.3%.Conclusion Single Navelbine and Navelbine plus DDP(or ADM)are effective and well tolerated treatments in patients with recurrent and metastatic breast cancer.
3.Recent advances in diabetic cataract advanced glycation end products and its inhibitors

Diabetic cataract is an metabolic-type cataract. Its pathogenesis mechanism is not completely clear. Researches demonstrated that glycosylation plays an increasingly important role in the formation of diabetic cataract,and increased blood glucose level promote the pathogenesis and progression of diabetic disease by accelerate the glycosylation response. Therefore, more researches on advanced glycation end products(AGEs) and its inhibitors is being concerned. The advance in the study of AGEs concept, its effect on diabetic cataract formation, the influence of AGEs inhibitor on diabetic cataract are summarized in this article.
4.Advance in studies on anti-diabetic mechanism of Gardeniae Fructus and its active ingredient geniposide.

The diabetes is mainly treated by the oral administration of western medicines at present. Despite their rapid curative effect, there have been still many reports for the western medicines about their clinical adverse reactions, failure of effective prevention and treatment of complications and drug resistance. Hence, they are not suitable for long-term administration. Traditional Chinese medicines have a long history in treating diabetes mellitus (DM) , which is commonly known as Xiaokezheng in the theory of traditional Chinese medicines. In recent years, many scholars have taken extracts from traditional Chinese medicines or separated active constituents as the study objects in the expectation of developing new-type drugs for treating and preventing diabetes. Therefore, a large number of study reports have been emerged in this field. Due to their significant glucose-reducing effect and specific effect in treating complications of diabetes, traditional Chinese medicine Gardeniae Fructus and its iridoid component geniposide shall be given full attention. This paper summarized the advance in studies on the curative effect and action mechanism of Gardeniae Fructus and geniposide in preventing and treating diabetes.

5.Combination of arterial perfusion chemotherapy and embolization for the treatment of giant carcinoma of kidney in 21 cases

Objective To explore the efficiency of arterial perfusion chemotherapy combined with embolization for the treatment of giant carcinoma of the kidney. Methods Arterial perfusion chemotherapy combined with embolization was performed through the renal artery in 21 cases of giant carcinoma of the kidney from April 1992 to April 2006. The chemotherapeutic agents contained carboplatin(300 mg), mitomycin(20 mg) and cyclophosphamide(800 mg). The embolization was conducted with anhydrous alcohol, sodium morrhuate, and lipiodol plus gelatin sponge. Results The arterial perfusion with embolization was successfully obtained in all 21 cases. Surgical resection was accomplished in 15 cases, 1 - 8 weeks after the embolization; revealing severely less or no blood supply to the tumor. Pathological findings showed marked necrosis of tumor cells with peripheral inflammatory infiltration, fibrous proliferation as well as capillary embolization. The survival rates at 1-, 2-, and 3-, year were 80%(12/15), 53.33%(8/15) and 40%(6/15) respectively. Conclusions Combination of perfusion chemotherapy and embolization through the renal artery for the treatment of giant carcinoma of the kidney offers promising clinical effects.
6. Hypoglycemic effect of geniposide and its relative mechanism

Objective: To investigate the hypoglycemic effect of geniposide and to explore the mechanism. Methods: The diabetic mice were induced by a single dose of 90 mg/kg streptozotocin (STZ) injection followed by four-week high-fat diets and randomly divided into three groups: normal control (con), diabetic model (diab), and geniposide (gen, 100 mg/kg) groups. Body weight and fasting blood glucose were measured during the treatment. Thirty days later, the oral glucose tolerance test (OGTT) was performed, blood samples were collected to measure insulin concentration in plasma. The morphological changes of pancreas pathology and β-cell proliferation were examined by immuno-fluorescent staining of insulin and Ki67. The phosphorylations of AKT and GSK-3β (p-AKT and p-GSK-3β) in liver tissues were detected by Western blotting assay. Results: Compared with the diab group, geniposide showed significantly hypoglycemic effect, together with lowering body weight, increasing the insulin content in plasma, and improving OGTT. The immuno-fluorescent staining showed the islets destruction caused by hyperglycemia was recovered by geniposide. The β-cell proliferation presented by Ki67 staining increased as compared with that in the diab group. Moreover, both p-Akt and p-GSK-3β levels in the liver tissue were upregulated by geniposide remarkably. Conclusion: The present study demonstrates that geniposide could ameliorate the hyperglycemia in diabetic mice by enhancing the pancreatic β-cell proliferation and activation of AKT signaling pathway in liver.

10.The therapy and prognosis of DHS, PFNA and Intertan nail on intertrochanteric fractures of elderly patients

Objective To investigate the therapy of DHS,PFNA and Intertan nail for the treatment in elderly patients with intertrochanteric femoral fractures.Methods One hundred and one elderly patients with intertrochanteric fractures were collected.Thirty-four cases of them were undertaken DHS (DHS group),33 cases for PFNA (PFNA group),and 34 cases for Intertan nail (INTERTAN group).Operative time,blood loss and complications were observed.Results The Operative time of patients in DHS,PFNA and Intertan groups were (85.1 ± 8.9) min,(63.1 ± 8.2) min,(57.9 ± 9.2) min respectively and the difference was significant (F=12.761,P<0.001).The blood loss amount in DHS group were (350.1 ±80.2)ml,lower that those in PFNA and Intertan groups ((137.5 ± 35.4) ml and (125.2 ± 38.2) ml,F =20.462,P < 0.001).Meanwhile,the postoperative drainage in DHS group was (125.3 ± 20.4) ml,significantly higher than PFNA group and INTERTAN group ((69.4 ± 9.2) ml and (74.6 ± 10.4) ml; F =15.871,P <0.001).Operative time,blood loss amount in INTERTAN group were less than that in PFNA group,but no significant difference (P >0.05).Nonunion in DHS group was significantly higher than the other two groups (8.82% vs 0% and 0%,x2 =6.092,P =0.047).Excellent rate in DHS group,PFNA group and Intertan group were 82.3% (28/ 34),87.8% (29/33),85.3% (29/34) respectively,and there was no significant difference (x2 =0.591,P > 0.05).Conclusion Compared with DHS,Intertan nail and PFNA have advantage of shorter operative time,stronger anti-rotation capability and less various postoperative complication rates,thus they are more suitable for elderly osteoporotic intertrochanteric fracture.
