2.Laboratory findings in symptomless clonorchiasis.

The duration of infection, frequency of eating raw fresh water fishes, and a series of liver function tests were studied in a group of 23 Korean symptomless cases with Clonorchis sinensis, and compared with those in a control group of uninfected persons. Of 23 lightly infected cases, 11 cases who had been infected for 10 to 20 years was found the highest in percentage(47.8), and 2 had been infected for over 30 years. The frequency of eating raw fresh water fishes ranged from 2 to 35 times. The range of Stoll egg-counts in 23 cases varied from 600 to 8,600 eggs per gram of feces and the average value was approximately 2,800 eggs. A series of liver function tests examined showed normal value in all cases. These results showed no difference in either infected group and its contol group.

3.A Case of Distal Renal Tubular Acidosis.

Distal renal tubular acidosis (RTA) is a biochemical syndrome as a deficiency of hydrogen in secretion by the distal tubule and collecting duct. Owing to the nature of the defect, hyperchloremic non-anion gap metabolic acidosis and high urine pH despite severe systemic acidosis is characterized. Typical manifestations of distal RTA are growth retardation, rickets, polyuria and nephrocalcinosis. We experienced a case of distal renal tubular acidosis in a 4 years old female child who complained of growth retardation and gait disturbance. She showed typical hyperchloremic non-anion gap metabolic acidosis and persistent high urine pH. In radiographic examination. we found delayed bone age and severe rachitic changes of wrist, elbow, and knee as well as nephrocalcinosis. In ammonium chloride loading test, high urine pH persisted despite the lowering blood HCO3- concentration. After alkali therapy, she is in well condition with excellent growth velocity and absence of rachitic changes.

5.Predicting Autism Spectrum Disorder Using Blood-based Gene Expression Signatures and Machine Learning.

OBJECTIVE: The aim of this study was to identify a transcriptomic signature that could be used to classify subjects with autism spectrum disorder (ASD) compared to controls on the basis of blood gene expression profiles. The gene expression profiles could ultimately be used as diagnostic biomarkers for ASD. METHODS: We used the published microarray data (GSE26415) from the Gene Expression Omnibus database, which included 21 young adults with ASD and 21 age- and sex-matched unaffected controls. Nineteen differentially expressed probes were identified from a training dataset (n=26, 13 ASD cases and 13 controls) using the limma package in R language (adjusted p value <0.05) and were further analyzed in a test dataset (n=16, 8 ASD cases and 8 controls) using machine learning algorithms. RESULTS: Hierarchical cluster analysis showed that subjects with ASD were relatively well-discriminated from controls. Based on the support vector machine and K-nearest neighbors analysis, validation of 19-DE probes with a test dataset resulted in an overall class prediction accuracy of 93.8% as well as a sensitivity and specificity of 100% and 87.5%, respectively. CONCLUSION: The results of our exploratory study suggest that the gene expression profiles identified from the peripheral blood samples of young adults with ASD can be used to identify a biological signature for ASD. Further study using a larger cohort and more homogeneous datasets is required to improve the diagnostic accuracy.

8.Clicically improved venous malformation by sclerotherapy.

There are two major types of vascular birthmarks, hemangiomas, those demonstrating endothelial hyperplasia, and malformation, those with normal endothelial turnover. Venous malformations have previously been treated by surgical excision, where possible. Although not a panacea for all such tumors, the use of sclerosing agents is decidedly preferable in some cases to extirpation with a scapel. The method involves the direct injection of absolute alcohol into the lesion on the fluoroscopic guide. The effect of treatment is satisfactory and there appears tobe no long term complication. We report 2 cases of surgically irresectable and deep-seated venous malformation which were treated sclerotherapy using ethanol.

9.Clinical Analysis of 8 Cases of Chronic Carbon Disulfide Poisoning in Workers Engaged in the Viscose Rayon Industry.

The authors have experinced 8 cases of chronic carbon disulfide poisoning in workers engaged in the viacase rayon industry during the period between March 1987 and September 1988. Cases of carbon disulfide poisoning has not been reported in Korea till then. Carbon disulfide is primarily a neurotoxic poison, therefore those symptoms indicating central and peripheral nervous damage are very important. And it has toxic effects on the various organs such as skin, eye, ear, and respiratory, cardiovascular, gastrointestinal, hepatic, genitourinary, hematological, and endocrine system. Occupational histories were obtained, and clinical findings and laboratory findings were analysed on all cases. 1. Seven (7) cases had been exposed to carbon disulfide for more than 10 years, and 1 case has been exposed for 6 years. 2. Most common symptoms were neurological symptoms such as gait disturbance, extremity weakness, paresthesia, and speech disturbance. 3. Involvement of central nervous system was noted in 7 cases through abnormal findings of electroencephalography, brain computerized tomography, neurological signs, and psychological test. Needle electromyography and nerve conduction study showed polyneuropathy in all cases. Ophthalmological abnormalities were found in 7 cases, mainly retinal microaneurysms and peripheral constriction of the visual field. Renal damage appeared in 4 cases, increase of serum cholesterol and/or triglyceride level appeared in 3 cases, and dercrease of erythrocyte and hemoglobin appeared in 2 cases. And hypertenison appeared in 2 cases, and diabetes appeared in 1 case. One(1) case who has been exposed tn carbon disulfide for 6 years showed polyneuropathy and peripheral constriction of the visual field.

10.Feasibility, Safety and Prognostic Factors for Computed Tomography Guided Aspiration and Thrombolysis of Intracerebral Hematoma - Clinical Analysis -.

OBJECTIVE: The authors reviewed experience with patients harboring intracerebral hematoma (ICH) treated by stereotactic computed tomography (CT) guided thrombolysis and aspiration and evaluated feasibility, safety and prognostic factors of this procedure. METHODS: One hundred and ten patients with supratentorial ICH >25 ml without underlying structural etiology or coagulopathy were recruited. The patients with Glasgow Coma Scale (GCS) <5 were excluded. A catheter was directed stereotactically into the ICH under CT guidance. Hematoma aspiration was followed by instillation of urokinase. This was repeated every 6 hours until less than half of its initial volume remained. For analysis of prognostic factors, we classified them into two groups;good (Glasgow Outcome Scale (GOS) > or =4) and bad (GOS<4) prognosis group, and performed comparative analysis between two groups. RESULTS: Mean age was 59.8 years. The baseline hematoma size ranged from 15 to 72 mL. ICH volume reduced by an average of 74.2%. At 6 months after the procedure, 56 patients had achieved a good recovery, 29 patients were dependent, and 10 remained vegetative. Fifteen patients died in hospital. The main good prognostic factors were young age, small ICH volume, high GCS, absence of rebleeding, underlying disease and complications. CONCLUSION: CT-guided thrombolysis and aspiration appears safe and effective in the reduction of ICH volume. Patients of ICH presenting with bad prognostic factors should require frequent radiological investigation and more meticulous procedure. Further studies are needed to assess optimal thrombolytic dosage and must include controlled comparisons of mortality, and disability outcome.
