4.Hurthle Cell Tumor of the Thyroid Gland in an Infant: A case report.

Hurthle cell tumors are an infrequent neoplasm of the thyroid gland in adults. Hurthle cell tumors represent 4. 5% to 10% of all primary thyroid epithelial neoplasms in the foreign literature. It has been known as Hurthle cell tumor since Ewing was the first to use this term in 1928. Tumor occurring in an infant was described by Symmers(1941) and Morrow(1945). The authors experienced a case of congenital Hurthle cell tumor of the thyroid gland in a 2 months old boy. He was admitted to the pediatric surgical department because of a growing mass in the neck since birth. Ultrasonogram showed a huge lobulated homogenous solid mass with medium level echogenicity in the region of the thyroid gland. Subtotal thyroidectomy of right lobe and total thyroidectomy of left lobe were done. The specimen measures 2x3x1.5 cm and 7x3x3 cm, respectively. It was an encapsulated, yellow gray firm and solid mass. The cut surface was smooth, glistening and homogenous. Microscopicully, the tumor was composed of tightly packed regular follicles lined by polyhedral, cuboidal, large cells with a granular acidophilic cytoplasms. The nuclei are vesicular, usually only a little bit larger than those of normal thyroid cells.

5.Brain MRI findings of complex partial seizure in children.

Twenty-two children(4 months to 17 years old)with a clincial diagnosis of complex partial seizure(CPS) were examined with a 0.5T MRI scanner using spin-echo sequences. Eleven patients showed abnormal brain MRI findings; two had focal lesions with corresponding seizure foci on the EEG, one arising from temporal lobe(Hippocampal Formation atrophy) and the other from the frontal lobe. Nine patients showed diffuse lesions with inconsistent seizure foci on EEG. The remaining 11 patients were normal on bran MRI; two of them had normal EEG findings and the others either focal or diffuse abnormalities on EEG.

6.Pericardial lymphangioma: Case report.

Cardiac lymphangioma is one of the rarest, primary, benign tumor of the heart. We report a case of cardiac lymphangioma, which was diagnosed with CT and MRI in a 50 years old female. Plain chest film showed minimal enlargement and globular shape of the heart. On CT scan, abnormal fluid density mass lesion was noted within pericardial sac. The signal intensity was lower on T1-weighted image and hgher on T2-weighted image than that of the myocardium and located along the left atrioventricuar groove. Several small low signal spots representing hemorrhage were seen within this lesion.

10.Endourologic Management of Caliceal Diverticula Containing Calculi.

Caliceal diverticula containing calculi had been managed conventionally by deroofing of the diverticulum, closure of the communication with the collecting system, or partial nephrectomy. To minimize the loss of the renal parenchyme and to treat caliceal diverticulum containing calculi, endoscopic procedure was performed successfully in 6 cases. but failed in 1. Repeated endoscopic procedure was performed in failed case 6 months after first operation and succeeded also. The diverticular neck was dilated with the Amplatz dilator upto 24-30Fr. A 20Fr. nephrostomy catheter was placed for 10-14days, and there was no complications. We evaluated the result of diverticular obliteration with excretory urography 6 months after the operation. In conclusion, endourologic management of caliceal diverticulum containing calculi is a safe. less invasive. successful and parenchyme preserving procedure.
