1.Heavy Metals of the Suspended Particulate in Atmosphere of Seoul City.

In order to investigate air pollution by heavy metals in Seoul city, the suspended particulates in the atmosphere were sampled with high volume air sampler in industrial area(Ku Ro Dong), commercial(Kwang Hwa Moon) and residential(Shin Chon Dong) from January to November, 1977. The sampled suspended particulates were digested and extracted from suspended particulates with the acidic solution by reflux-extraction technique, and were measured by atomic absorption spectrophotometry. And Mercury was measured by mercury analyzer applying the reducing sublimation technique. Among heavy metals analyzed, the iron was identified at th highest level in the suspended particulates and the chromium was the least. Through the surveyed area, the concentration of heavy metals of the industrial area was comparatively high among others and the commercial was the second. It was detected that lead was the most concentrated in the suspended particulate of the commercial area, that might be caused of the traffic emissions. The seasonal variations were analyzed and the correlations among heavy metals and total suspended particulate were also calculated. Especially, the iron was highly correlated with total suspended particulate in all the surveyed areas.

2.Studies on Benzo(a)pyrene of the Suspended Particulate in Atmosphere of Seoul City.

This study was carried out to investigate air pollution by total suspended particulate(T.S.P.), benzene soluble matter and benzo(a)pyrene in Seoul city. The sampling areas were divided into commercial(Kwang Hwa Moon), industrial(Ku Ro Dong) and residential area(Shin Chon). Sampling was undertaken by High Volume Air Sampler for four seasons from January 1977 to November 1977. The T.S.P. was extracted with Soxhlet apparatus by benzene and benzo(a)pyrene was separated by column chromatography and thin layer chromatography. The concentrations of benzo(a)pyrene were measured by means of fluorophotometer, and following results were obtained. 1. Arithmetic average concentration for 1-day averaging time of total suspended particulate were 275.6 microgram/m3 in Kwang Hwa moon, 325.9 microgram/m3 in Ku Ro Dong and 193.0 microgram/m3 in Shin Chon. 2. The seasonal variance of total suspended particulate at Ku Ro Dong and Shin Chon were 102.7 microgram/m3 99.6 microgram/m3 respectively and at Kwang Hwa Moon 39.9 microgram/m3. And the concentration of autumn is higher than of that spring at Ku Ro Dong and at Shin Chon, but at Kwang Hwa Moon, the seasonal variance is very little. 3. The concentrations of 50% frequency from geometric mean for 1-day averaging time were 264 microgram/m3 and 178 microgram/m3 at Kwang Hwa Moon, Ku Ro Dong and Shin Chon. And geometric standard deviation were 1.27, 1.38 and 1.41 respectively. 4. The concentrations of benzene soluble mater were 26.9 microgram/m3 Kwang Hwa Moon, 22.7 microgram/m3 at Ku Ro Dong and 15.5 microgram/m3 at Shin Chon, and the ratios to the T.S.P. were 9.8%(range 5.6-14.8%), 7.0%(range 2.4-14.4%) and 8.0%(range 5.5-22.1%) respectively. 5. The concentrations of benzo(a)pyrene were 8.5 microgram/m3 (range 0.8-29.9 microgram/m3 ) at Kwang Hwa Moon 10.9 microgram/m3 (range 1.1-52.0 microgram/m3 ) at Ku Ro Dong and 5.8 microgram/m3 (range 1.5-11.4 microgram/m3) at Shin Chon. 6. The results of this investigation were relatively high in compared with the recommended standards of suspended particulate in air of U.S. Environmental Protection Agency and observed levels of Benzo(a)-pyrene in U.S. city.

3.Measurements of pulmonary vasculatures in normal adult postero-anterior chest x-ray

In spite of recent introduction of such highly sophisticated diagnostic modalities as computerized tomography,cardioangiography and polytomography the simple PA chest remaines as an essential and important method of chestdiagnosis especially in terms of vascular diseases of the lung and heart. Until recently, however, theinterpretation of chest x ray has been largely dependent on much subjective observation of roentgen anatomy of thechest. Several years ago Simon proposed a quantitative approach to interpretation of the chest in an attempt tomake the judgement more objective. In the present study we have carried out a clinical study on normalities of thestandard PA chest of Korean adults. The materials consisted of 216 normal chest films. Age ranged from 20 to 60years and sex distribution 103 males and 113 females, We counted the number of vessels in both lung fields,measured the diameter of the basal artery and investigated the level of hilum points as well as hilar distance.There were 9 to 11 vessels in the upper zone and 9 to 12 in the lower zone. There were no significant sexdifference. The number of vessels was not affected by aging. The average diameter of the basal artery was11.7±2.4mm in the right lower zone and 10±1.8mm in the left lower zone in female. There was significant sexdifference (p 0.01). Aging seemed not to be affected the size of the basal artery. The majority of hilum point waslocated at the level of 5-6th rib at axillary portion and the left hilar point is 0.3-2.0cm higher in locationthan the right. The hilar distance was 4.1±0.4cm on the right 5.0±1.1cm on the left in male and 4.0±0.7cm onthe right and 4.7±0.5cm on the left in female. There was significant sex difference (p 0.01). There wassignificant difference in size of the basal arteries and hilar distance between the Koreans and Englishmen but thenumbers of vessels were not significantly different.

4.The diagnostic value of barium enema in acute appendicitis

Acute appendicitis is the most common acute surgical condition of the abdomen. when the clinical presentationis atypical, barium enema has proven to be safe and useful in confirming the diagnosis and reducing the negativesurgical exploration. However, the performance of barium enema in acute appendicitis has known contraindicationprimarily because of fear of leakage by perforation of the inflammed appendix. This study using barium enema as adiagnostic aid in acute appendicitis with atypical clinical presentation was performed to further support thepreviously noted efficacy and safety of this procedure. The results were as followings: 1. In case of acuteappendicitis with atypical clinical presentation, the use of barium enema as a diagnostic aid increased theaccuracy of diagnosis and decreased the negative surgical exploration. In women between 11 to 50 years old age,especially, it played important role differentiating appendicitis from nonsurgical acute abdomen. 2. The resultsof the study were 92.31% in sensitivity, 7.69% in false positive, 6.9% in false negative, and 10.26% in negativeappendectomy. 4. A simple partial or nonfilling of appendix without other associated possitive finding could notexclude appendicitis, therefore, close clinial observation was necessary. 5. The positive findings of barium enemaand their sensitivity were as followings: 1. Nonfilling of appendix: 9% 2. Partial filling of appendix: 91.7% 3.Displacement or a local impression on temrinal ileum: 100% *, In all cases, combined with a local impression oncecum and/or irritability of cecum or terminal ileum.

5.Glossopharyngeal Neuralgia - A case report .

Glossopharyngeal neuralgia, first discribed by Labat in 1928, is a very rare disease of unknown cause. This neuralgia is associated with a characteristic sharp pain of the posterior pharynx, tonsils and larynx, and especially triggered by swallowing action. It is said that this pain is more severe than that of trigerminal neuralgia and the pain may last several up to 30 minutes and the attack repeats intermittently. Diagnosis of glosspharyngeal neuralgia is made by the symptoms and by the elongation of the right side of the styloid process in this case. The glossopharyngeal nerve block by the deposition of local anesthetic solution is useful in the accurate diagnosis of the douleureux or neuralgia in which this nerve is involved and in providing anesthesia for operative intervention upon the posterior third of the tongue. This is a report of a case of glossopharyngeal neuralgia, which did not respond to Tegretol and other analgesic drugs and treated by glossopharyngeal nerve block with 0.5% bupivacaine l.5-2.0 ml. The block was performed every day for 15 days and the neuralgia disappeared without complication.

6.A Case of Malignant Transformation of Gastric Tubular Adenoma Proven by 9-year Follow-Up.

The association of gastric tubular adenoma and adenocarcinoma is already well known. Then, are those two pathologic lesions merely incidentally coexisting or does the one evolve to the other? That is a longstanding controversy. Nowadays, as is the case with the colonic tubular adenoma, the hypothesis that gastric tubular adenoma may be a precancerous lesion is generally accepted. However the direct evidences are rare. We report a clear case proving the potential of direct malignant transformation of gastric tubular adenorna through the 9-year close endoscopic follow-up. The developed cancer is well differentiated, surrounded with background tubular adenoma and limited to the submucosal layer.

7.Reconstruction of Large Bone Defect after Bone and Soft Tissue Tumor Resection , using Jeat

To fill the large bone defect after bone and soft tissue tumor resection, there are several options such as tumor prosthesis, bone cement with intramedullary nail, autogenous bone graft and allograft. We had used isotrophic autogenous bone graft by using the heat-treated bone removed from tumor site. We analyzed the periods for junctional union and regeneration of autoclaved or low-heat treated groups, and compared these two methods to know which method is better for reconstruction of the bone defect after tumor resection. From Jan. 1987 to Sept. 1993, twelve patients took heat-treated autogenous bone graft: 6 auto- claved, and 6 low heat-treated. Each group had 10 places of junction sites between host and grafted bone. The tumors were 2 cases of osteosarcoma, 3 parosteal osteosarcoma, 2 Ewing's sarcoma, 2 malignant soft tissue tumors, 1 giant cell tumor, and 2 metastases from thyroid cancer and synovial sarcoma. The graft sites were 4 in humerus, 4 pelvis and 4 femur. Two cases showed marginal surgical margin and others wide surgical margin. Here we compare4 the difference between autoclaved group(120℃, 2 atm., 20 min) and low heat-treated group(65℃, 30 min. in water) on the aspect of complications and period to achieve junctional union to host bone. Average follow-up period was 25.3(11 to 88) months. Graft related complications in autoclaved group were bone resorption(2 sites), fracture of grafted bone(2). For low heat-treated group there was no such complication. Nonunion occurred in 3 sites for autoclaved group and 1 for low heat-treated group. Average period for junctional union was 7.3 months(5 to 10 months) for autoclaved group and 6.1 months(5 to 9 months) for low heat-treated group. With these results, heat treated bone autograft may have several advantages such as easy accessi- bility, low cost and anatomical reconstruction of the bone defect. The low heat-treated autogenous bone graft may have more advantages than that of the autoclaved one, and this method may be ratio- nalized to fill the large bone defect made by tumor resection.

8.Postoperative Airway Obstruction in a Patient With Huge Mediastinal Hemangioam.

Anesthesia for the patients with mediastinal mass may be associated with significant respiratory and cardiovaacular complications due to compression of traeheobronchial trees, the pulmonary artery, heart and superior vena cava The authors present a case of a 6-year-old girl with a large mediastinal tumor located in anterior and superior mediaetinum. Preoperatively, the patient was asymptomatic and anesthesia was induced and maintained uneventfully. Operative finding was that 8 X 14 cm sized hemangioma was extended superiorly over thoracic outlet, posteriorly aorta and SVC, medially pericardium and right hilum and inferiorly right upper lung and pleura. Because difficulties in removal of the hemangioma were expected, exploratory thoracotomy was discontinued. On emergence, as soon as the patient was extubated due to irritability, the patient became cyanotic and was noticed bulging mass on right supraclavicular area, Intubation was attempted and during direct laryngoscopy, anesthesiologist also noticed a bulging mass in right oral cavity. Postoperatively, the patients airway was kept with endotracheal tube, but cyanosis appeared on the face and skin over the chest, but not below the abdomen. Bulging mass on right supraclavicular area, distension of neck vein and conjunctival edema were noticed only when the patient was crying or suctioned. Symptoms improved by deep sedation. We assumed that enlargement of hemangioma due to increased central blood volume occasionally compressed tracheobronchial tree and sup. vena cava when patient was irritable, crying and coughing. On 11th postoperative day, patient was transferred to the another hospital for the better treatment with endotracheal intubation.

10.A Case of Myxoid Malignant Fibrous Histiocytoma.

Malignant fibrous histiocytoma is the most common soft tissue sarcoma in adult life and shows a mixed proliferation of both fibroblsstic & histiocytic cells. We report a case of myxoid malignant fibrous histiocytoma involving right lower leg in a 68-year-old male patient, who presented with a childs palm-sized nodular mass of 3 years duration. Histoapthologically, the tumor had varying proportion of myxoid cellular components. The myxoid areas showed spindle and pleomorphic cells arranged in a vague storiform pattern, abrupt transition between a solidly cellular area and a myxoid area, and lipoblast-like cells. The cellular areas showed spindle cells arranged in a storiform pattern characteritic of the typical pleomorphic malignant fibrous histiocytorna.
