Fibulin protein family has widely existed in the extracellular matrix, and plays an important role in maintain the stable of basement membrane, elastic fibers and loose connective tissue. Fibulin-5 (FBLN-5) is one of the members of the Fibulin protein family, which is essential to the structure function and cell mediated action. The lack of FBLN-5 will cause many diseases such as internal organ deformity. Current research suggests that changes of expression level of FBLN-5 are associated with the occurrence of tumors such as lung cancer, and also can influence the tumor proliferation, invasion and prognosis, thus may become a new molecular marker for cancer diagnosis. Therefore, in this review, we summarize and update biological characteristics and functions of FBLN-5, and its role in the development of tumor formation mechanism.

OBJECTIVE:To study the protective effect of the injection of puerarin combined with salvianoli acid B(Sal B)on rats with myocardial ischemia reperfusion injury (MIRI). METHODS:62 rats were randomly divided into sham operation group, model group, puerarin group(20 mg/kg)and puerarin(20 mg/kg)-Sal B group(mass ratio of 1:0.5,1:1,1:2,respectively),10 in each group. Except for sham operation group, rats in other groups were reduced for MIRI model. After 180 min of reperfusion, kinase(CK),lactate dehydrogenase(LDH),superoxide dismutase(SOD),malondialdehyde(MDA)in serum and percentage of myocardial infarction size of rats were detected. RESULTS:Compared with sham operation group,CK,LDH,MDA levels in se-rum of rats in model group were obviously increased (P<0.01),SOD level was obviously decreased (P<0.01);and percentage of myocardial infarction size was obviously increased (P<0.01). Compared with model group,CK,LDH,MDA levels in serum of rats in each administration group were decreased(P<0.05 or P<0.01),SOD levels were obviously increased(P<0.05 or P<0.01),and indexes changed the most obviously in puerarin-Sal B group(1:1);percentage of myocardial infarction size was obvi-ously decreased(P<0.01),and the percentage of myocardial infarction sizes in puerarin-Sal B group(1:1)and group(1:2)were less than Puerarin injection group (P<0.01). CONCLUSIONS:Compared with Puerarin injection alone, puerarin combined with Sal B by injection can more effectively inhibit the cardiomyocyte injury and decrease myocardial infarction size after MIRI,with best efficacy when quality ratio is 1:1.

ObjectiveTo compare the sedative effect and safety of dexmedetomidine and midazolam in the intensive care unit (ICU) patients undergoing ventilator bundle treatment.MethodsA prospective single-blind randomized controlled trial (RCT) was conducted. Ninety patients receiving ICU ventilator-assisted therapy and ventilator bundle treatments for more than 3 days in the First Department of Critical Care Medicine of the Second Hospital of Lanzhou University from January 2013 to December 2014 were enrolled. The patients were randomly divided into two groups for sedative treatment. The patients in dexmedetomidine group (n = 42) were given dexmedetomidine 0.2-0.7μg·kg-1·h-1 to achieve a goal of satisfactory sedation [Richmond agitation-sedation scale (RASS) score 0 to - 2 during the day, and -1 to -3 at night). The patients in midazolam group (n = 48) were given midazolam 2-3 mg intravenously first, and then 0.05 mg·kg-1·h-1 for maintenance. The drug dose was adjusted according to RASS every 4 hours to maintain the appropriate sedation depth. The patients in both groups received continuous intravenous infusion of fentanyl for analgesia. Ventilator bundle treatments included the head of a bed up 30°to 45°, awaken and extubation appraisal, daily use of proton pump inhibitors for peptic ulcer prevention, prevention of deep vein thrombosis (DVT), chlorhexidine mouth nursing, and removal of sputum by suction from subglottic area. When the patients in both groups obtained satisfactory target sedation, daily awakening was conducted, and spontaneous breathing test (SBT) was carried out to determine optional weaning time. When the condition was optimal, weaning was conducted, otherwise ventilator bundle treatments were continued. The systolic blood pressure (SBP), diastolic blood pressure (DBP), mean arterial pressure (MAP), heart rate (HR), respiratory frequency (RR) were monitored before and 15, 30, 60, 120, 180 minutes after the treatment, and at the moment of extubation and 30 minutes after extubation. The duration of mechanical ventilation, extubation time, length of ICU stay, and the incidence of adverse events were also recorded. Results Both dexmedetomidine and midazolam could give rise to sedation with same score of analgesia in patients in both groups, and similar effect of sedation and analgesia could be achieved. Compared with midazolam, dexmedetomidine could significantly reduce the duration of mechanical ventilation (hours: 108.33±21.96 vs. 119.85±20.29,t = -2.586, P = 0.011), earlier extubation time (hours: 112.95±22.20 vs. 128.58±26.18,t = -3.031,P = 0.003), length of ICU stay (hours: 149.21±20.47 vs. 163.88±33.59,t = -2.457,P = 0.016), the incidence of delirium [9.5% (4/42) vs. 31.2% (15/48),χ2 = 6.349,P = 0.012], but it would elevate the incidence of severe hypotension [28.6% (12/42) vs. 8.3% (4/48),χ2 = 6.277,P = 0.012] and severe bradycardia [19.0% (8/42) vs. 8.3% (4/48),χ2 = 2.225,P = 0.136]. Both drugs could lower SBP, DBP, MAP, and HR, and the effect in dexmedetomidine group was more significant from 60 minutes after treatment [SBP (mmHg, 1 mmHg = 0.133 kPa): 113.12±14.42 vs. 124.40±15.79, DBP (mmHg): 69.02±9.62 vs. 76.94±10.41, MAP (mmHg): 83.76±10.50 vs. 92.77±11.87, HR (bpm): 79.19±12.28 vs. 87.42±17.77,P< 0.05 orP< 0.01]. Both sedatives could significantly lower the rate of spontaneous breathing, and the effect of midazolam group was more significant from 60 minutes after treatment compared with dexmedetomidine group (times/min: 18.27±4.29 vs. 20.07±4.11,P< 0.05).Conclusions The sedative effects of dexmedetomidine in the ICU patients treated with ventilator bundle treatment are satisfactory, and it can shorten the duration of mechanical ventilation, extubation time and length of ICU stay, reduce the incidence of delirium. However, monitoring should be strengthened in order to prevent and control the adverse effects such as severe hypotension and severe bradycardia.

Aim To investigate the effects of K + channel blockers on arsenic trioxide-induced HeLa cell death. Methods Viability of HeLa cells was assessed by mitochondrial dehydrogenase activity using colorimetric MTT assay and the voltage-dependent K+ currents were recorded by using patch-clamp rest living cells after As2 O3 24 h-incubation showed significant increase of K + currents densities. At + 80mV, the densities of K+ currents (61 ± 18) pA/10 pF (n = 8) in As2O3 24 h-incubation group were significantly more than that in the control group (38 ± 10) pA/10 pF (n = 8, P < 0. 05 ). The HeLa cells were prevented partially from As2 O3-induced cell death by co-application for 24 h with typical voltageeffects on HeLa cells. Conclusion Chronic treatment with As2 O3 increased voltage-dependent K+currents in HeLa cells and the cell death induced by As2O3 was reduced partially by voltage-dependent K +channel blockers, 4-aminopyridine or tetraethylammonium.

Methods Seventy-seven hemiplegia convalescence patients with depression were divided into general treatment group (38 patients, received general back-up psychology therapy) and combined treatment group (39 patients, received general back-up psychology therapy and Taoist cognitive psychotherapy) by random digits table. All patients were treated for 8 weeks and followed up for 6 months. Two groups were evaluated with HAMD and SS-QOL before treatment and at the end of the 2 weeks,4 weeks, 8 weeks and 6 months after treatment. The results were analyzed with statistics. Results In general treatment group, the HAMD scores were gradually decreased, and the HAMD scores of patients after 8 weeks' treatment were significantly lower than those before treatment[(22.35 ± 4.69)scores vs. (29.62 ± 5.95 ) scores,t = 6.425 ,P < 0.01]. At the end of 6 months after treatment, the scores increased [(24.48 ± 4.12 ) scores vs. (22.35 ± 4.69 ) scores, t = 2.014, P < 0.05], but they were lower than those before treatment(t = 4.836, P < 0.01 ). At the end of 6 months after treatment, the SS-QOL scores were lower than those after 8 weeks' treatment (t =2.543,P <0.05),but they were higher than those before treatment (t = 2.790,P < 0.05 ). In combined treatment group, the HAMD scores decreased continuously, and the scores after 8 weeks' treatment [(20.08 ± 4.60) scores] and 6 months' treatment [( 15.21 ± 3.42)scores] were significantly lower than those before treatment [( 30.14 ± 4.92 ) scores] (t = 8.341,15.443, P <0.01). Meanwhile, the HAMD scores after 6 months'treatment were significantly Iower than those after 8 weeks' treatment (t =4.724,P < 0.01 ). The SS-QOL scores after 8 weeks' treatment [( 117.56 ± 26.22)scores] and 6 months' treatment [(126.57 ±21.82) scores] were significantly higher than those before treatment[(86.54 ± 23.90) scores] (t = 6.716,8.916,P < 0.01 ) ,and there was significantly difference(t=2.378,P < 0.05). The HAMD scores of combined treatment group after 8 weeks' and 6 months' treatment were significantly lower than those of general treatment group at the same time(t = 2.118, P < 0.05 ;t = 8.405,P< 0.01 ) ,and SS-QOL scores were significantly higher than those of general treatment group at the same time (t = 3.123,P < 0.05 ;t = 6.580,P < 0.01 ). Conclusions General back-up psychology therapy combined with Taoist cognitive psychotherapy can improve depression and life quality of cerebral stroke hemiplegia convalescence in aged patients. The effects of Taoist cognitive psychotherapy is slower, but it is more beneficial in the long time.

Objective To investigate the effect of electroacupuncture at different points on auditory brainstem response (ABR) in a rat model of sodium salicylate-induced tinnitus.Method Forty-one male Wistar rats were randomly allocated to saline control (saline), sodium salicylate model (SA), electroacupuncture at Tinggong+ Yifeng (EA), electroacupuncture at Waiguan+ Zhongzhu (AA) and electroacupuncture at Zusanli+ Sanyinjiao (LA) groups. The saline group consisted of five rats and each of the other groups, nine rats. The model was made by intraperitoneal injection of sodium salicylate 275 mg/kg. The saline control group was injected with a corresponding volume of saline. Various acupoint electroacupuncture groups were given electroacupuncture at bilateral Tinggong+ Yifeng, Waiguan+ Zhongzhu and Zusanli+ Sanyinjiao, respectively, at 30 min after model making. Electroacupuncture lasted 30 min. The ABRs were recorded before model making and once every one hour for five consecutive hours after model making. The stimulus sounds were short clicks and tone bursts of frequencies of 4, 8, 16 and 32 kHz. The ABR threshold was used as an assessment index. Results Under the condition of clicks, there was a statistically significant difference in the ABR threshold at one to five hours after model making between the SA, EA, AA or LA group and the saline group (P<0.05), at two to five hours after model making between the EA or AA group and the SA group (P<0.05) and at one hour after model making between the LA and SA groups (P<0.05). Under the conditions of 4, 8 and 16 kHz, there was a statistically significant difference in the ABR threshold at one to five hours after model making between the SA, EA, AA or LA group and the saline group (P<0.05). Under the condition of 32 kHz, there was a statistically significant difference in the ABR threshold at one to five hours after model making between the SA, AA or LA group and the saline group (P<0.05). Under the condition of 4 kHz, there was a statistically significant difference in the ABR threshold at two to five hours after model making between the EA and SA groups (P<0.05) and at four to five hours after model making between the AA and SA groups (P<0.05). Under the condition of 8 kHz, there was a statistically significant difference in the ABR threshold at two to five hours after model making between the EA or AA group and the SA group (P<0.05). Under the condition of 16 kHz, there was a statistically significant difference in the ABR threshold at two, four and five hours after model making between the EA and SA groups (P<0.05). Under the condition of 32 kHz, there was a statistically significant difference in the ABR threshold at one to five hours after model making between the EA and SA groups (P<0.05).Conclusion Electroacupuncture at both periauricular and forelimb points can improve the ABR threshold in sodium salicylate-treated rats. The effect of electroacupuncture at periauricular points is superior to that at forelimb points.

Chinese Journal of Medical Imaging 2015;(7):489-493
PurposeAs a main method to detect thyroid nodules, ultrasonography seems to have a rather low accuracy in differentiating benign and malignant ones. The present study aims to explore the diagnostic value of thyroid imaging reporting and data system (TI-RADS) in identifying benign and malignant thyroid nodules.Materials and Methods A total of 168 patients with thyroid nodules conifrmed pathologically (with 251 suspicious nodules) underwent ultrasonography and were further grouped into category 3-5 according to TI-RADS classiifcation standard. The results were retrospectively compared with histopathological ifndings. The sensitivity, speciifcity, accuracy, positive predictive value and negative predictive value of TI-RADS classiifcation in the diagnosis of benign and malignant thyroid nodules were calculated, and the differences in ultrasonic features between benign and malignant thyroid nodules were also compared. Results The surgical and pathological findings showed that 94 nodules were benign and 157 were sinister; TI-RADS regarded 93 nodules were benign and 185 were malignant. The difference of the two means of identification was significant (χ2=149.6,P<0.01). The sensitivity, speciifcity, accuracy, positive predictive value and negative predictive value of TI-RADS were 91.2% (144/157), 85.1% (80/94), 89.3% (224/251), 91.2% (144/158) and 86.0% (80/93), respectively. The ultrasonic manifestations of benign and malignant nodules were signiifcantly different with regard to aspect ratio, echogenicity, shape, calciifcation inside the nodule (χ2=8.7-121.4,P<0.01).Conclusion TI-RADS classiifcation standards have a high accuracy in the differentiation of benign and malignant thyroid nodules as a means of ultrasound examination, and thus can serve as an important guiding method in clinical diagnosis and treatment.
