2.A Case of Ancient Schwannoma of the Lingual Nerve.

Schwannomas are neurogenic tumors that arise from Schwann cells of the neural sheath. They are most often benign and solitary. Ancient schwannoma is a rare variant of schwannoma with a typical characteristics of a slow growing benign tumor. A case of ancient schwannoma which originated from the lingual nerve has not been reported in the literature yet. The clinical and histological aspects of this tumor are discussed and the literature regarding this rare entity is reviewed.

3.A Case of spontaneous hemorrhagic pseudocyst of adrenal gland mimicking cystic pheochromocytoma.

Cysts of the adrenal glands are uncommon and present difficult problems in differential diagnosis. In autopsy studies, the incidence of adrenal cysts is ranges from 0.064% to 0.18%. Recently, we have experienced a 22-year-old female patient with spontaneous hemorrhagic pseudocyst of adrenal gland without known cause, presenting symptoms of nausea, epigastric discomfort and intermittent pain of right upper abdomen. In clinical presentation, abdominal ultrasonogram and computerized tomogram suggest cystic degeneration of malignant pheochromocytoma, but screening hormonal evaluation was normal. Selective adrenal venous sampling, adrenal scintigram and pathologic examination were not compatible with the functioning adrenal cortical or medullary adenoma/carcinoma, the mass results in spantaneous hemorrhagic necrosis and cystic degeneratio#n of adrenal gland. The authors reported a case of spontaneous hemorrhagic pseudocyst of adrenal gland, which was successfully resected by laparoscopic excision with reviews of the literatures.

8.A Study of Infuential Factors on Glucose Levels in Blood and Cerebrospinal Fluid of Children with Convulsions.

BACKGROUND: In febrile convulsions, glucose concentrations are known to increase both in the blood and cerebrospinal fluid(CSF). The reason behind this increase is, however, not completely understood. We have studied the effects of convulsion and fever on the CSF and blood concentrations in four different groups of children: convulsive and non- convulsive children, with and without fever. METHODS: This study was retrospectively undertaken to review medical records of 167 children who had lumbar puncture at Chosun University Hospital during 2 years from January 1994 to December 1996. The blood samples for glucose determinations were taken just before the lumbar puncture. The effects of several variables (body temperature, duration of fever, convulsions) were analysed on glucose concentrations in the CSF and blood between the groups. RESULTS: 1) The concentration of glucose in the CSF was significantly higher in convulsive children with(90.9+/-24.Smg/dl) and without fever(105+/-29.7mg/dl) than in non-convulsive children with(68.4+/-12.3mg/dl) and without fever(67.5+/-13.3mg/dl). 2) The concentration of glucose in the blood was not significant in the different groups. 3) There was no correlation between the body temperature and the concentration of glucose in the CSF(r=0.09, p>0.05).4) A negative correlation between the duration of the fever and the concentration of glucose in the CSF was found in febrile chi1dren (r=-0.26, p<0.05). CONCLUSIONS: Our results show that an increase in the CSF glucose concentration in convulsive children may be influenced more by a stress reaction, evoked by the seizure than increased body temperature.

Duodenal intramural hematoma is mostly caused by blunt abdominal trauma. It is also less commonly reported as a complication of anticoagulation therapy or as a blood dyscrasia, and as a complication of diagnostic/ therapeutic endoscopy. The presentation of these patients is abdominal pain, vomiting, fever and hematochezia, and this is rarely accompanied with intestinal obstruction, severe pancreatitis and acute peritonitis as its complications. The diagnosis is made clear by performing abdominal ultrasonography and abdominal computed tomography. We reported here on one case of intramural duodenal hematoma and hemoperitoneum after performing endoscopic hemostasis in a chronic renal failure patient who was on maintenance hemodialysis.
