1.Demonstration of Metagonimus yokogawai metacercariae from Plecoglossus altivelis in river Ahnseong, Kyungpook province, Korea.

There are a number of eating-houses in the basin of river Ahnseong that sell rawsweetfish, Plecoglossus altivelis, to local residents and visitors. These facts called attention to the possibility of wide spread of metagonimiasis in that area. In an attempt to determine whether the metacercaria of Metagonimus yokogawai does present in the sweetfish caught in the river Ahnseong, a total of 77 fish were examined in the period from April to September 1978. All the fish were infected with the encysted larvae. The average number of larvae persweetfish was 1,815, of which the metacercaria rate for the fluke in the flesh was 56.9 per cent and in the scale, 30.5 per cent. These results indicate that all the sweetfish in the river Ahnseong harbour the metacercaria of Metagonimus yokogawai.

3.A Case of Right Ovarian Vein Syndrome.

A case of right ovarian vein syndrome is reported with a review of literature. The patient presented was a 29 year-old multipara and eighth month of pregnancy. The extirpation of entire right ovarian vein, including it's all tributaries produced a good result and has not caused interruption of pregnancy. The clinical situation due to aberrant right ovarian vein is probably more frequent than heretofore suspected, by reason of a frown of X-rays during pregnancy and also an unawareness of the condition. Therefore, if clinical observations and X-ray studies are performed, more cases of such a condition. will be expected.

4.A Clinical Analysis of Intussusception in Infants and Children.

This article consists of a clinical analysis of 171 cases of intussusception treated at the Department of General Surgery and Pediatrics of Pusan Adventist Hospital, during a five year period from Jan. 1991 to Dec. 1995. The results were as follows ; 1) On the age and sex distribution, the age incidence under 1 year old was the most common(73.1%), and male to female ratio was 1.48 : 1. 2) The seasonal variation was Autumn(35.1%), Spring(32.1%), Summer(19.9%), and Winter(12.9%). 3) On the hematologic laboratory finding, 68% were revealed leukocytosis above 10,000/mm3 in serum. 4) In etiology, idiopathic type was common(98.5%), and Meckel's diverticulum was rare(1.5%). 5) The frequent Symptoms and signs were abdominal pain and irritability(84.2%), bloody stool(83.6%), vomiting(80.7%), palpable abdominal mass(48.5%). 6) On comparison of barium reduction to methods of the operation in relation to duration of symptoms, within 12 hours was 81.4% : 15.7% and above 24 hours was 49% : 51% 7) The common anatomical type of intussusception was ileo-colic type (36.9%), and the common types of bowel resection were ileoileo type and ileoileocolic type(80%). 8) The methods of the operation were manual reduction(53.8%), bowel resection(30.8%), and spontaneous reduction(15.4%). 9) Postoperative complications occurred in 10 cases ; febrile convulsion 3 cases, wound infection 2 cases, and wound dehiscence, intestinal obstruction, pulmonary complication each 1 case. 10) The overall recurrence rate was 5.3% (8 cases); 6.6% (7 cases) were after barium reduction, and 1.5% (1 case) was after manual reduction.

6.Overcorrection, Undercorrection and Astigmatic Change after Excimer Laser Photorefractive Keratectomy of Myopia.

We have studied the overcorrection, undercorrection, astigmatic change and its incidence on time sequence after the excimer laser photorefractive keratectomy, and its relation with the degree of preoperative myopia, sex, and age. The study has been done with 429 eyes which we could assess for 6 months or more after excimer laser photorefractive keratectomy of myopia from July 1993 to march 1997. Overcorrection of more than -1D was common during the early postoperative periods and then gradually decreased with time. Overcorrection has been seen in 261 eyes(60.84%) after 1 month, but only 33 eyes(13.47%) after 1 year, and 13 eyes(11.02%) after 2 years. Undercorrection has been seen in 2 eyes(0.47%) after 1 month, but 25 eyes(10.20%) after 1 year, and 20 eyes(16.95%) after 2 years. Surgically induced astigmatism increased to 0.54+/-0.84D after 10 days and then decreased 0.19+/-0.75D after 12 months. Also, surgical induced astigmatism developed 1.13+/-1.10D in 247 eyes (57.58%) after 1 month, but decreased 0.89+/-0.45D in 107 eyes(43.67%) after 12 months. The occurrence of overcorrection and undercorrection was related to the severity of preoperative myopia(p<0.05), but with sex and age, it had no statistical correlation(p>0.05). The amount of postoperative astigmatism was related to preoperative astigmatism(p<0.05), but it had no statistical correlation with the degree of preoperative myopia, age, sex, and development and increase of surgically induced astigmatism(p>0.05).

7.Clinical Observations of Inguinal hernia in Pediatric Patients.

This article consists of a clinical analysis of 221 cases of the Pediatric Inguinal Hernia treated in General Surgery of Pusan Adventist Hospital, from January 1990 to June 1996. The results were as follows ; 1) Patients with inguinal hernia were 37.5% of total operative patients in the division of pediatric surgery 2) Males predominated over females by the ratio of 5.5:1 3) Of 221 cases, 101(54%) were on the Rt side, 80(43.1%) on the Lt side and the remainder (6.4%) on both side in case of Male. 20(58.8%) were on the Rt side, 13(38.2%) on the Lt sid and the remainder (3.0%) on both side in case of Female. 4) The chief Complaints were inguinal protrusion in 131 cases(59.3%) and scrotal enlargement in 77 cases(34.9%) 5) Peak incidence age (32 Months) & Duration of history (20.2 Months) 6) Associated diseases were found in 10 cases(4.9%) and they were cryptorchidism (1.47%), hydroclele (0.98%), Umbilical hermia (0.49%), Heart anomaly (0.49%), cleft palate (0.49%), Teratoma (0.49%), and Jejunal atresia (0.49%). Family History was denied. 7) All were indirect hernia. 8) Incarcerated cases were 6(2.7%) and the ratio of male to female was 2:1, and the location of incarcerated hernia was Rt (66.6%), and Lt (33.3%) 9) There were 70 cases of strangulation. 10) Intraperitoneal organ in hernial sac was noted in 5 cases (2.26%), and they were ovary and fallopian tube in 3 cases (1.35%), small intestine in 1 case (0.45%), cecum and appendix in 1 case (0.45%). 11) 215 (97%) of all hernia were repaired only by highligation of sac and 6(3%) were inforced with Bassini's posterior wall repair. 12) Mean postoperative hospitalization were 5.2 days. 13) Postoperative complications occured in only 2 cases : hematoma and edema 1, wound infection 1.

9.An Experimental Study about the Effects of Parathyroid Hormone on Strength of Vertebra and Bone Density of Proximal Tibia in Ovariectomized Rats.

Parathyroid hormone (PTH) has not been studied as extensively as therapeutic agents of osteoporosis, but the anabolic effect of parathyroid hormone on the skeleton has been demonstrated in several studies. The current study was undertaken to assess the effect of parathyroid hormone on bone mass and its biomechanical competence using ovariectomized rat model. Fifty female Sprague Dawley rats of 3 months old were randomized into five groups, 10 in number for each groups. Group I: sham-operation, Group II: ovariectomy (OVX) plus saline injection, Group III: OVX plus PTH 20ug/kg, Group IV: OVX plus PTH 40ug/kg, Group V: OVX plus PTH 80ug/kg. Treatment regimens were initiated 8 weeks after ovariectomy and continued for 4 weeks thereafter. Bone mineral density was measured in proximal one-third level of tibia by dual energy X-ray absorptiometry (QDR-2000, Hologic, U.S.A). Biomechanical competence was assessed in lumbar vertebral body by universal testing machine (Instron-4467, U.S.A). The average of bone mineral density in sham operated group was 0.088+/-0.0084g/cm (2), and in control group (OVX +/- saline)was 0.065+/-0.0095g/cm (2). Bone mineral density decreased in control group significantly. The average of bone mineral densities in Group III, IV, V (OVX + PTH) were 0.071+/- 0.0089, 0.081+/-0.0086 and 0.084+/-0.0093g/cm (2), respectively. This showed that parathyroid hormone injected groups had significantly higher bone mass than control group (p<0.0#5), and the increment was in proportion to the amount of parathyroid hormone in the range (p<0.05). The average of biomechanical competence of lumbar vertebral body in sham operated group was 30.036.24 MPa and was 25.11+/-5.69 MPa in control group. In Group III, IV,V they were 25.74+/-6.77, 27.96+/-5.17 and 29.83+/-5.79 MPa, respectively. Above results seem supportive of the phenomenon that parathyroid hormone exert anabolic effect on osteoporotic bones in certain experimental condition.

10.Arthroscopic Reduction and Internal Fixation of distal Femoral Intraarticular Unicondylar Fractures: Report of Three Cases.

Intraarticular distal femoral unicondylar fractures are uncommon. Anatomical reduction is the point of the good result of the treatment. Non displaced or minimally displaced fractures can be treated nonoperatively, but must be followed closely for loss of reduction. Displaced unicondylar fractures require surgical fixation to prevent the complications of axial malalignment, post-traumatic arthritis, knee stiffness and instability frequently reported after nonoperative treatment. We present a case report discribing an alternative treatment method, namely, arthroscopic reduction and internal fixation (ARIF), of displaced intraarticular distal femoral unicondylar fractures. There are several potential benefits of less pain, fewer complications (bleeding, arthrofibrosis), quick recovery, better cosmesis and shorter hospital days.
