1.Electroretinographic Changes Experimentally Induced by the Intraocular Copper.

Copper fragment was introduced into the vitreous body of fourteen albino rabbits through the scleral incision at equatorial region. Both photopic and scotopic ERGs were recorded for 30 days thereafter stimulated by the electronic photoflash of 2 msec duration.The changes in scotopic b-potential were roughly classified into 3 types. The first type (5 eyes) showed acute, severe reduction, extinguishing from 6 to 24 days. The second type (3 eyes) was characterized by the absence of any remarakble changes during the whole observation period, the final b'potential being 82,4% of the original value. In the third type (6 eyes), the b-potential decreased gradually and 24.3% of the pre' operative value was attained after 30 days. The latent time and the implicit time of the b-potential increased proportionally to the reduction of the b-wave. The scotopic a-wave behaved parallel to the b-wave, and in later stage, negative (-) type of ERGs were frequently encountered. The photopic a- and b-waves changed in the similar way as the scotopic a- and b-waves, respectively. The slitlamp biomicroscopy and ophthalmoscopy revealed the fibrous tissue proliferation around the copper fragment in relatively early stage. When the proliferation progressed and extended into the vitreous body, ERG showed reduction in b-potential, and sometimes increase in b-potential was noted In cases in which connective tissue encapsulated the copper and did not extend into the vitreous body, ERG changes were much milder and slower. In histological sesctions performed 2 weeks and 3 weeks after the insertion of copper fragment, the degeneration of the retina began both in the visual cell layer and ganglion cell layer almost simultaneously. When the ERG was extinguished, the retinal tissue showed complete destruction.

2.A Case of Thanatophoric Dwarfism.

A case of thanatophoric dwarfism is presented. The previous gestation was polyhydramniotic and aborted spontaneously at 19th week. The aborted fetus showed marked micromelia. Present gestation was also polyhydramniotic and terminated by therapeutic abortion at 31th weeks due to skeletal deformity representing thanatophoric dwarfism. The fetus shows typical characteristics of thanatophoric dwarfism. Grossly the fetus shows marked micromelia, narrowed thorax with relatively normal length of trunk, and enlarged head. In the radiograph the femur shows characteristic ‘ telephone receiver’-like form. And the histological observations shows marked disturbances of the endochondral ossification.

3.Multiple Chorioangiomas of Placenta: A case report associated with repeated abortions and intrauterine fetal death.

Chorioangioma is the most common primary tumor of the placenta. It is widely accepted to be formed from hamartomatous proliferation of angioblastic cells in early placenta. According to groups who systematically examined the placentas with the greatest scrutiny, the incidence of chorioangioma is 1~1.2% of all placentas examined. The clinical implication of chorioangioma has been emphasized due to its high association with hydramnios, antepartum and postpartum hemorrhage, premature onset of labor, or intrauterine fetal distress or death. The authors experienced a case of a 38-year old multigravida who had undergone 4 consecutive spontaneous abortions. The fifth pregnancy was carried to 35 weeks whereupon a dead fetus was delivered. The placenta was enlarged(950 gm) with multiple protruding nodules into the fetal surface. Microscopically, the nodule was composed of many vilous structures lined by chorionic epithelium. Within these structures was proliferation of endothelial lined capillaries some of which demonstrated ectatic change.

7.Percutaneous K-wire Fixation of Colles' Fracture A Prospective Study of Twenty Seven Cases

27 Colles' fracture were treated with closed reduction and percutaneous k-wire fixation under the C-arm fields, from July 1980 to September 1984 at the Department of Orthopedic Surgery of Eul Ji Hospital. The average duration of the follow up was 4.9 Months. A prospective study was made and evaluated under the subjective and objective criteria of Gartland and Werley, and the objective criteria of Scheck. The results of this study were as follows: 1. The incidence was high in 4th and 5th decade as 66. 5%, not significant between females and males, 45% of the cause of injury was falling accident. 2. In 27 cases, showed the satisfactory result about 92.6%, but, in the eomminuted intraarticular fracture group, 2(15.4%) out of 13 cases showed unsatisfactory result. 3. Better result was obtained with more secure reduction and skillful technique, in certain case, considered that the open reduction and internal fixation with k-wires would be better than percutaneous k-wire fixation. 4. In the aspect of hand motion and the concept of improved anatomical restoration and maintenance would lead to improved function, ideal method was percutaneous k-wire fixation and casting after closed reduction under the C-arm fields.

8.Acetabular Revision with Hemispherical Porous Coated Prosthesis.

PURPOSE: To report the results of acetabular revisions performed with the cementless, hemi-spherical porous coated component supported by viable host bone and minimal allobone graft for acetabular deficiencies. MATERIALS AND METHODS: A retrospective study was completed for 22-revision acetabular components, using the cementless hemispherical porous coated prosthesis and minimal femoral head allograft. There was an average follow up of 48 months, with a range of 24 to 84 months. Radiographic measurements were performed in several aspects; cup-allograft contact, inclination, vertical and horizontal migration of acetabular cup, and acetabular zone by modified zone of DeLee and Charnley, in which location, size and progression or non-progression of radiolucent zone were recorded. The acetabular deficiencies were classified by the American Academy of Orthopaedic Surgeons Committee and were type I in 2 hips (9%), type II in 12 hips (55%), and type III in 8 hips (36%). Twenty acetabular cup cases of Harris-Galante II were used in this study, 1 case of Harris- Galante I, and 1 case of Duraloc. The average size of the cup was 57(44-66) mm. RESULTS: The average cup-allograft contact was 72.5% in 19 cases and we could not differentiate the margin between the host bone and the allograft in 3 cases. There was no significant vertical or horizontal migration of acetabular cup. Radiolucent zones in follow-up radiographs were 8 cases in zone IA, 4 cases in zone IB, 4 cases in zone IIB, 9 cases in zone IIC, 8 cases in zone IIIA, and 7 cases in zone IIIB. One case showed 2 mm radiolucent area in zone IIC and another case showed 3 mm radiolucent area in zone IIB, but the radiolucencies were not progressive. The remaining cases showed less than 0.5 mm radiolucent area or no radiolucent zone. The average period of bony incorporation was 13.1 months. CONCLUSIONS: We suppose that acetabular revision with the cementless hemispherical porous coated cup supported by viable host bone and minimal bone graft produces good results.

9.A Clinical Study of the Severe Ankle Fracture

The primary goal of treatment of the adult ankle fracture is to effect the returning of normal func tion that is accomplished by the accurete reduction and rigid internal fixation followed by the early institution of motion. The authors have reviewed a series of 41 cases of the bimalleolar and trimalleolar ankle fractures that treated in Orthopedic Department of Eulji General Hospital, Seoul, from January 1982 to June 1986. With the duration of follow-up of average 13.2 months, we summarized the obtaind results as followings. l. Among the forty one patients, thirty one was male patient(M: F =3.1: 1). The average age was 33.2 years old. 2. The main cause of the fracture was traffic accident, 18 cases(43.9%) and other causes were slipping down(24.4%), fall from a height(14,6%), industrial injury(12.2%) and other injuries(4.9%). 3. According to the modified Lauge-Hansens classification, the supination-external type was most common (41.5%). 4. As a diagnostic method, the computerized tomography was also available. 5. All cases were treated by open reduction and internal fixation with the good radiologic results of 91.3% in bimalleolar fractures and 88.9% in trimalleolar fractures. 6. Of the bimalleolar fractures, ten patientz showed normal range of motion in 2 to 4 weeks, average 18.4 days, by early joint exercise.
