1.A Case of Embryonal Carcinoma Developed in a Cryptorchid Testis.

A case of testicular embryonal carcinoma developed in s cryptorchid testis is presented with a review of the literature. According to Campbell's series, the incidence of malignant cryptorchid testis was 11.6%. and his statistical evaluation was similar to Gilbert and Hamilton's series of 7,000 cases of testicular neoplasms where 840 tumors were developed in undescended testes(12%). Both authors agree that the chance of neoplastic development in a cryptorchid testis is approximately 48 times greater then that developing in a normally descended testis. Several cases of testicular tumor were reported sporadically in Korea, but malignant tumors in cryptorchism were rarely seen in the literatures Only two cases of malignant cryptorchid testis were already reported in Korea.

3.A Case of Polymorphic Pemphigoid.

We report herein a case of polymorphic pemphigoid in a 65-year-old woman, who had one-year history of generlized polymorphic eruption with intensely pruritic excoriated vesicobiullous lesions and residual pigmentation. Face and oral mucosa were spared. The skin biopsy specimen showed a subepidermal blister containing many eosinophils. Linear deposition of 1gG and C3 along the basement membrane was noticed on direct irnmunofluorescerice. Polyclonal garnmopathy with a tendency of beta-gamma bridging and increased IgG, IgA and kappachain, was frund on serum protein electrophoresis and immunoelectrophoresis. Skin lesions were well coritrolled by a combined therapy of prednisolone(20 mg/d), dapsone(100 mg/d) and cyclophosphamide(100 mg/d) for 4 months. Direct immunofluorescence performed 11 rnonths after the cnmpletion of the therapy was negative.

7.Duodenal Obstruction due to Peptic Ulcer in Children.

Primary peptic ulcer disease in not known to be the result of underlying illness or trauma. These are most frequently duodenal or prepyloric. Since clinical features of peptic ulcer in children can easily be confused with many other disorders, the diagnosis is usually made when one of the more dramatic presentations, such as perforation, bleeding and obstruction. Recently, we experienced 2 cases of duodenal obstruction due to peptic ulcer in children. So, we report it with review of references.

8.Frequency of Neurologic Disorders in the Neonatal Intensive Care Unit and Associated Factors.

PURPOSE: Recent neonatal care unit survival rates have improved. These increasing survival rates have resulted in increased neurologic disorders. So we investigated the sorts and frequency of neurologic disorders at neonatal intensive care unit, association between gestational age and birthweight, neonatal mortality and tendency of neurologic disorders in survivors. METHODS: Retrospective reviews were made on 156 cases with one or more neurologic disorder of 1,399 neonates, who were admitted to the neonatal intensive care unit, Soonchunhyang Hospital from January 1997 to December 2001. Neurologic disorders were categorized into five groups:intraventricular hemorrhage, neonatal seizures, hypoxic- ischemic encephalopathy, central nervous system infection and central nervous system congenital malformations and chromosomal aberrations. RESULTS: Over the study period, 156 cases with neurologic disorders had a total of 193 neurologic disorders. The frequency of neurologic disorders increased from 3.2% in 1997 to 11.5% in 2001, while the neonatal mortality rates in intensive care unit have declined from 4.8% in 1997 to 3.7% in 2001. In 156 neonates, 134 had intraventricular hemorrhage, 28 had seizures, 24 had hypoxic-ischemic encephalopathy, 2 had central nervous system infections and 5 had congenital central nervous system malformation and chromosomal aberration. Frequencies of intraventricular hemorrhage and seizure has increased, but, that of hypoxic-ischemic encephalopathy has not changed, for the 5 years. CONCLUSION: Neonatal intensive care unit survival rates have improved for 5 years, but, the neurologic morbidity rates have increased. Because neurologic disorders have great influence on the long-term neurologic disability, early detection and treatment of neurologic disorders is important.

9.Anatomical Variations of the Hypophysis and the Diaphragma Sellae in Korean Adult Cadavers and Coronal CT.

The anatomical variations of 112 hypophysis and diaphragma sellae in Korean adult cadavers and coronal CT were studied. 1) The hypophysis was classified 4 types based on superior view. 2) The superior surface of the hypophysis was concave(65.9%) in cadavers and flat(55.3%) in CT. 3) The neural lobe was placed on the center of the posterior surface of the anterior lobe(72.3%). 4) The hypophysis was compressed by the internal carotid artery in 9.6%. 5) The mean A-P length, width and height of the hypophysis were 10.4mm, 14.2mm and 4.8mm in cadavers, respectively. The mean width and height in CT were 13.2mm and 5.0mm, respectively. 6) The width of the hypophysis was significantly different between man and woman. 7) The diaphragma sellae was concave or flat. 8) The diaphragmatic line was average 13.9mm in man and 14.6mm in woman. 9) The diaphragmatic foraman was circular or oval and the A-P diameter was greater than transverse one. 10) Type IIb that diaphragma sellae and hypophysis were concave according to Busch(1951) was 40.4%. 11) The empty sella was found in 14.4%.

10.Local production of specific IgE antibody to house dust mite in nasal polyp tissues.

OBJECTIVE: In order to confirm the local production of total and specific IgE antibodies in the nasal polyp tissues. MATERIAL AND METHOD: We measured total IgE and house dust mite(Dermatophagoides pteronpssinus .' DP)-specific IgE antibody using enzyme-linked immunosorbent assay(ELISA) in the supernatant of nasal polyp homogenates from 72 subjects undergoing nasal polypectomy. The subjects were divided into three groups according to skin reactivity to DP: 20 strongly atopic subjects to group I(mean wheal diameter) 3mm), 19 weakly atopic subjects to group II (mean wheal diameter 1-3mm) and 33 negative skin responders to group III.
