1.A Novel Fluoroscopic View for Positioning the AO Clavicle Hook Plate Decreases Its Associated in situ Complications.

BACKGROUND: The goal of this study was to evaluate whether a modified fluoroscopic technique for positioning a hook plate affected the clinical results of treating Neer type II distal clavicle fractures and Rockwood type V acromioclavicular (AC) joint separations with this device. METHODS: The study was a retrospective consecutive case series with data analysis. Sixty-four patients with a Neer type II distal clavicle fracture or a Rockwood type V AC joint injury treated between March 2009 and June 2013 were divided into 2 groups: traditional fluoroscopic technique (traditional view, 31 patients) or modified fluoroscopic technique ('hook' view, 33 patients). A visual analogue scale (VAS) score, the modified University of California-Los Angeles (UCLA) shoulder scale score, and radiographic osteolysis were the main outcome measures. RESULTS: The traditional group included a significantly larger number of patients with acromial osteolysis than the hook view group: 23 patients (74.2%) vs. 11 patients (33.3%), respectively (p=0.01). Before plate removal, the hook group reported less pain and higher UCLA shoulder scale scores than the traditional group: average VAS score, 1.55 vs. 2.26, respectively; average UCLA score, 30.88 vs. 27.06, respectively. However, there was no significant difference after plate removal. CONCLUSIONS: The hook view allows more accurate bending of the hook plate around the contour of the acromion, resulting in decreased osteolysis, decreased pain, and better function with the plate in situ.

2.Treatment of Unstable Osteochondral Dissecans Lesion of the Knee Joint Using Autologous Osteochondral Plug.

PURPOSE: The purpose of this study was to evaluate the clinical results of fixation of the unstable osteochondral dissecans (OCD) lesion with autologous osteochondral plugs. MATERIALS AND METHODS: This study was conducted in 19 patients who were relevant to the International Cartilage Repair Society 2, 3, or 4 who were treated with autologous osteochondral plugs and followed-up for more than 2 years from January 2004 to January 2012. Clinical evaluation was performed by comparing the preoperative and last follow-up scores of Lysholm score and subjective International Knee Documentation Committee (IKDC) score. RESULTS: All patients were male and the average age was 19.1 years. Of the 19 cases, there were 16 cases of medial femoral condyle lateral side lesion, 2 cases of lateral femoral condyle articular surface, and 1 case of femoral intercondylar notch lesions. The average size of the lesion was 5.68 mm2, and average use of osteochodral plugs were 4.3. Average follow-up period was 38 months. Preoperative Lysholm score, IKDC subjective score showed significant improvement. CONCLUSION: Fixation with autologous osteochondral plugs for unstable OCD uses the remnant tissues therefore conserving it, which is thought to be the positive aspect of this type of operation.

3.Etiologic Transition of Septic Arthritis of the Knee.

PURPOSE: The purpose of this study was to retrospectively analyze the etiologic transition of the predisposing factors and organisms for septic arthritis of the knee. MATERIALS AND METHODS: Between January 1995 and December 2006, 122 cases of septic arthritis of the knee were retrospectively evaluated for the etiologic transition and causative organism with using the patients' medical records. We researched the incidence and causative factor of septic arthritis of the knee, which was diagnosed via the patients' symptoms, physical examinations, hematologic exams, culture studies & cytology of the joint fluid. We assessed the correlation of disease with age, the year the disease happened, the causative factors and the incidence. RESULTS: Septic arthritis of the knee was increased after 40 years old, and it also increased after 1998. The most common predisposing factor was intra-articular injection of the knee joint. Even though there were no detected organisms in 79 cases (64.8%), the most common causative organism was S. aureus (20.5%). CONCLUSION: The most common etiologic factor of septic arthritis of the knee was an intra-articular injection. We should be prudent for using good aseptic technique and the correct procedure to reduce the secondary infection that's recently due to increased invasive treatment of the knee joint.

5.Improved analysis of palm creases.

Palm creases are helpful in revealing anthropologic characteristics and diagnosing chromosomal aberrations, and have been analyzed qualitatively and quantitatively. However, previous methods of analyzing palm creases were not objective so that reproducibility could not be guaranteed. In this study, a more objective morphologic analysis of palm creases was developed. The features of the improved methods include the strict definition of major and minor palm creases and the systematic classification of major palm creases based on their relationships, branches, and variants. Furthermore, based on the analysis of 3,216 Koreans, palm creases were anthropologically interpreted. There was a tendency for palm creases to be evenly distributed on the palm, which was acknowledged by the relationship between major and minor creases as well as by the incidences of major creases types. This tendency was consistent with the role of palm creases to facilitate folding of palm skin. The union of major palm creases was frequent in males and right palms to have powerful hand grip. The new method of analyzing palm creases is expected to be widely used for anthropologic investigation and chromosomal diagnosis.

6.Three dimensional evaluation of impacted mesiodens using dental cone beam CT.

PURPOSE: This study was performed to analyze the position, pattern of impacted mesiodens, and their relationship to the adjacent teeth using Dental cone-beam CT. MATERIALS AND METHODS: Sixty-two dental cone-beam CT images with 81 impacted mesiodenses were selected from about 2,298 cone-beam CT images at Chonnam National University Dental Hospital from June 2006 to March 2009. The position, pattern, shape of impacted mesiodenses and their complications were analyzed in cone-beam CT including 3D images. RESULTS: The sex ratio (M : F) was 2.9 : 1. Most of the mesiodenses (87.7%) were located at palatal side to the incisors. 79% of the mesiodenses were conical in shape. 60.5% of the mesiodenses were inverted, 21% normal erupting direction, and 18.5% transverse direction. The complications due to the presence of mesiodenses were none in 43.5%, diastema in 19.4%, tooth displacement in 17.7%, delayed eruption or impaction in 12.9%, tooth rotation in 4.8%, and dentigerous cyst in 1.7%. CONCLUSION: Dental cone-beam CT images with 3D provided 3-dimensional perception of mesiodens to the neighboring teeth. This results would be helpful for management of the impacted mesiodens.

9.A Case of Isolated ACTH Deficiency Manifestated by Atrial Fibrillation and Hypoglycemia.

Isolated ACTH deficiency is a rare cause of secondary adrenocortical insufficiency caused by the defect of synthesis or release of ACTH in pituitary gland. The clinical presentation can be simiilar to that of primary adrenal insufficiency, but there is a greater tendency for hypoglycemia and absence of hyperpigmentation. The patient, 80 year-old female, was admitted in chief complaint of deteriorated mental states. On admission, serum sodium was 127mEq/L, potassium 4.4mEq/L, blood glucose 27mg/dL and on routine E.C.G. was atrial fibrillation was revealed. The basal morning serum cortisol level was 9.97 pg/dL it dosent respond to insulin-induced hypoglycemia enoughly, but other pituitary functions were intact in pituitary cocktail stimulation test. In CRH stimulation test, there was no remarkable response in serum ACTH and cortisol level. Brain MRI failed to reveal any anatomic abnormalities of the sellar or suprasellar area consistent with the defect of pituitary ACTH secretion. This case was a isolated ACTH deficiency, So, we conclude that associated with atrail fibrillation and hypoglycemia.

10.Three types of the serial segmented images suitable for surface reconstruction.

Stereoscopic surface models of human organs can be manipulated in real time. This is a significant feature of an interactive simulation system used for clinical practice. Objective surface models are obtainable from the accumulation of each structure's serial outlines, followed by surface reconstruction. The segmented images including the outlines can be divided into outlined images, white-filled images, and color-filled images. The purpose of this study was to report the benefits of the three types of segmented images for surface reconstruction. For the raw data, sectioned images of a male cadaver head were used. In the sectioned images, 91 structures were delineated for the preparation of 234 serial outlined images. The outlined images were converted into white-filled and color-filled images; the reverse conversion was also possible. The outlined images, including the original sectioned images, could be the source not only of surface models but also of volume models. The white-filled images, with a minimal file size, were preferred for separate surface reconstruction of the individual structures. The color-filled images, which allowed for recognition of the entire outlined structures simultaneously, were regarded as a good choice for the construction of several surface models. For the process, we employed a variety of software packages including those for animation, where the images were compatible. This information can be used by other investigators to build their own three-dimensional models. In addition, the surface models of detailed structures in the head, accompanied by the corresponding sectioned and segmented images, will hopefully contribute to various simulations that can be useful to clinicians.
