Krukenberg tumor, which can account for 30-40% of metastatic cancers to the ovaries, arises in the ovarian stroma and is usually metastatic from the gastrointestinal tract, especially from the stomach. The pathognomonic feature is the presence of signet ring cells, which may be arranged in acini or appear as individual cells. We experienced a Krukenberg tumor which was bilateral and associated with left ovarian dermoid cyst. This patient was treated for a primary gastric carcinoma(Stage II) about 3 years ago. After bilateral salpingoophorectomy, she received adjuvant chemotherapy. But, she died about fow months after operation.

3.A Clinical Study of Ankle Fracture

The ankle joint is a modified complex hinge joint which plays an important role in weight bearing, walking and standing. Injuries about the ankle joint cause destruction of not only bony architecture but often also the ligamentous and soft tissue component. So the goal of the treatment of ankle fracture is restoring normal relations of ankle mortise and regaining a smooth articular surface. The author have reviewed 84 cases of ankle fractures which were treated through the admission in the Orthopedic Department, In Je Medical College Pusan Paik Hospital from June 1979 to August 1986. The average duration of the follow-up was one year. The results obtained from this study were as follows : 1. The most common type was Danis-Weber type C. Nexts were B and A in order. According to the classification of Lauge-Hansen, the most common type was Pronation-external rotstion(39.2%). 2. In Danis-Weber type B and C, satisfactory result was obtained by operative treatment. 3. Danis-Weber's classification was very practical criteria for treatment of the ankle fracture.

The two most common causes of acute arterial occlusion are embolism and thrombosis in sity. They are mainly originated from the cardiovascular sources. About 70-80 per cent of occlusions occur in the axial limb vessels. Therapeutic options include supportive measures, pharmacologic treatment, surgery, and non-operative interventions. There have been several successful case reports using percutaneous aspiration thromboembolectomy with the advent of new instruments and technical imprevement. We report a case of 70-year-old male with acute anterior wall myocardial infaction who experienced acute embolic arterial occlusion of the left popliteal artery from mural thrombus in the left ventricular apex. It wan managed successfully by percutaneous aspiration thromboembolectomy.

9.The Inhibition of Stress-Induced c-fos Expression by Superior Cervical Ganglion Block in Rat Brain.

BACKGROUND: Using c-fos expression one of the immediate early gene, as a marker of altered neuronal response, we investigated the effect of superior cervical ganglion block (SCGB) exhibiting the same effect of SGB of human on the activity of several brain regions which are considered as located on autonomic neural pathway and neuroendocrine axis in rat. METHOD: The 48 Sprague-Dawley strain rats were divided into 4 groups, as saline/stress (control) group, SCGB/stress (tested) group, saline group, SCGB group. Superior cervical ganglion block was conducted in the SCGB/stress group and SCGB group while saline/stress and saline group were sham operated. After then restraint stress was imposed on the animals of SCGB/stress group and saline/stress group. And 2 hour after injection (saline, SCGB group) or restraint stress (saline/stress, SCGB/stress group), c-fos protein (Fos) was localized by immunocytochemistry. RESULTS: Much stronger Fos immunoreactivity was induced in the several brain region of control group rats compared to other three groups and the numbers of Fos positive cell count of tested group were significantly decreased in paraventricular hypothalamic nucleus (p<0.01), A5 (p<0.01), raphe pallidus (p<0.05), nucleus tractus solitaius (p<0.01) compared to control group. CONCLUSION: This study demonstrate that superior cervical ganglion block attenuates stress induced neuronal activities of paraventricular hypothalamic nucleus, A5, raphe pallidus, nucleus tractus solitarius.

10.Comparative study of the Characteristics of Collagen Synthesis by Cultured Fibroblasts in the Fetus and Adult.

With the development of antenatal diagnostic tools such as ultrasonography, some congenital anomalies or diseases can be detected in early fetal life. Routine serial antenatal check-ups have made it possible to predict the prognosis of these problems, and a few life-threatening single anatomic malformations have been treated by open fetal surgery. The experience of fetal surgery revealed that the human fetus appears to heal without any scarring. In contrast to adult animals, the response to tissue injury in the fetus is conspiciously devoid of acute inflammation. Indeed, the absence of neutrophils is perhaps the most consistent observation in fetal wounds and seems to be followed by absent or scanty fibroblast infiltration, which results in healing with sparse and well organized collagen deposition. Actually, the amount and quality of the collagen deposition were decided by the fibroblasts which infiltrated the wound. It is well known that fetal wounds have sparse collagen deposition, however, the mechanisms are still unclear. This study was designed to evaluate the role of fibroblast activity in the differences of the scar formation between the fetus and neonate. Fibroblast activity such as the cell growth rate, the amount of collagen synthesis and the synthesized collagen types of fetus(IUP 18-22 weeks) was compared with that of neonate. The amount of collagen synthesis was measured by H-proline uptake and the amount of collagen type III was measured by Western blot using antihuman procollagen type III. The cell growth rate as determined by cell proliferation from the initial cell count of 5x10(5) to cell confluence was 3.6 x 10(6) in the fetal fibroblasts compared to 2.5x10(6) in neonatal fibroblasts. Fetal fibroblast synthesize 16.9 x 10(4) cpm of collagen and neonatal fibroblasts synthesize 2.7 x 10(4) cpm of collagen. The synthesized amount of type III collagen was 2.1x10(4) ug/ml, and 1.5x10(4) ug/ml by fetal and neonatal fibroblasts, respectively. In conclusion, fetal fibroblasts grow faster and synthesize a smaller amount of collagen, but produce more type III collagen than neonatal fibroblasts.
