2.The Role of Urinary Sodium at Transient Renal Acidification Defect during Acute Infantile Acute Gastroenteritis.

The pupose of this study is to verify the role of urinary sodium in transient renal acidification defect which frequently combine acute infantile gastroenteritis. We studied on twenty-five infants, 2month to 36 month of age, during the 4 month period, from August, 1991 to December, 1991. The patients had acidosis for a mean of 3 days and sixty urine samples were collected during this period. The mean pH of 23 rine samples with sodium concentration<10 mmol/L was significantly higher than pH of 37 samples with sodium concentration<10 mmol/L, We separately analyzed 15 urine samples collected during metabolic acidosis after completion. of rehydration. The result was that a urinary acidification defent was observed in the urine samples with low sodium concentration but not in the sodium rich samples. We concluded that impaired urinary acidification defect is frequently during metabolic acidosis in infants with acute gastroenteritis and results from a sodium deficit rather than from transient distal renal tublur acidosis.

3.Pharyngoesophageal reconstruction.

Microvascular tissue transfers have facilitated primary closure of various complex defects after radical ablation of head and neck cancers. From Oct 1991 to Feb 1992, we used forearm free flap in two patients and delto-pectoral flap in one patient who had preoperative irradiation for pharyngoesophageal reconstruction. The stricture and fistula formation were most troublesome complication in forearm free flap, so we designed as lazy S shape in distal flap margin to prevent circular contraction and longitudinal margin was deepithelialized (5 mm) and sutured double layer to withstand fistular formation and this can be considered useful in place of a free jejuna transfer.

4.Relationship between stress amounts and health habits about nurses in one hospital.

BACKGROUND: Stress is found to have influence on physical and mental disorders, and also to healthy life habits related to physical health mental stress. Job holders are groups that have suffered from chronic stressful condition. Consider medical people of job holders to received exess stress. So, we tested this investigation to know the relationship between health habits and stress amounts about nurse. METHODS: This study was performed on nurses who worked on one hosptal in Pusan Metropolitan city for one months(from April, 1, to April, 30, 2000). We used 128 results. Health habits were investigated about 5 articles out of "Breslow's 7 health habits" and stress amounts were measured by Korean translated BEPSI(Brief Encounter Psychosocial Instrument) and Bae Jong Myun's 30 items SRRQ(Social Readjustment Rating Questionnaire). RESULTS: There were differences in amounts of stress of subjects according to marital state(P<0.05), job department(P<0.05), BMI(Body Mass Index)(P<0.05), sleeping time(P<0.05). CONCLUSION: This investigation has showed that having excess stress in nurses. Body Mass Index, sleeping time appeared to be health habits related to amounts of stress.

7.Malignant Hyperthermia after Incomplete Relaxation of masseter Muscle.

Masseter muscle rigidity may herald a fulminant malignant hyperthermia reaction or may be an isolated abnormal anesthetic response. It is very hard to differentiate, Significant masseter muscle rigidity is a muscle spasm of at least 30 seconds duration that interferes with mouth opening despite an appropriate succinylcholine dose in a patient who has no temporo-mandibular joint dysfuntion. They thought mild to moderate foims of spasm are tolerable for anesthesia. We suffered incomplete relaxation of masseter muscle after succinylcholine that followed by fulminant malignant hyperthermic reaction. At that time, a little resistance was felt during mouth opening put intubation was done . without difficulty. After this experience, because Caffeine-Halothane contracture test is performed only in limited institutes and dantrolene is not available in our hospital, we decide to hold the anesthetic practice in case of any degree of masseter muscle rigidity.

8.A CLINICAL STUDY ON TMJ ARTHROCENTESIS.

The purpose of this study was to access the effect of TMJ arthrocentesis in locking 8 patients with anterior distal displacement without reduction ( 'closed lock' ). TMJ Arthrocentesis was defined as needle puncture of the joint space and usually combined with lavage. After lavage, the mandible is gently manipulated to evaluate the movement. Arthrocentesis with irrigation has been used successfully with few complications and low morbidity. Postoperative therapy requires repetitive opening and excursive exercises to maintain and improve the function. The mean follow-up period was 17.1 months(range, 8 to 30months). Preoperative and postoperative mean maximal interincisal opening(MIO) was 25.4mm preoperatively and 39.8mm postoperatively. The pain was reduced in 7 out of 8(87.5%) patients with limited mouth opening. The result of this study indicate that TMJ arthrocentesis is an effective means of controlling pain and improving jaw movement in patients with closed lock.
