Autogenous costal cartilage graft has been commonly used for reconstruction of auricular deformity. However, the risk of complication and discomfort at the donor site, as well as distortion of the graft due to morphological change in the cartilage have been serious drawbacks to this procedure. Previous studies examining the chondrogenic potential of perichondrium have suggested that perichondrium may be used as graft for cartilage reconstruction. When a perichondrial flap or a free perichondrium was used as graft, new cartilage formed appositional to the grafted perichondrium. However, the neocartilage was often irregular in shape and varied considerably in quantity. In this study, the feasibility of controlling the shape and the mass of neocartilage was investigated using coral, a porous biomaterial, as a template. A coral a template was wrapped with perichondrial flap from the ears of New Zealand white rabbits and placed into a subcutaneous pocket in the rabbits and placed into a subcutaneous pocket in the rabbit's back by incision. A total of 12 animals were used. Formation of new cartilage was later evaluated by gross and histological examination of the perichondrial flap and the coral template. New cartilage was formed in 11 animals. Immature chondrocytes were visible by 3 weeks after the surgery, and by 8 weeks the immature chondrocytes had formed a cartilage. New cartilage was formed only on the surface of the coral template. These results indicated that the shape and the mass of new cartilage may be controlled by using coral template. Therefore, the desired shape of cartilage may be achieved using a coral template of corresponding shape, and this may help in correcting subtle auricular contour defect and in correcting other structural defects that also require new cartilage formation.

Aspirin is one of the popular non -steroid anti -inflammatory drugs used in the management of pain. This study was performed to investigate the effects of aspirin on c -Fos expression in rat CNS after inducing somatic pain with formalin. Male S.D. rats were injected subcutaneously with 0.1 ml of 5% formalin in the plantar surface of right hindpaw. For experimental group, aspirin was administered orally before injection of formalin. Asprin -untreated group was utilized as the control group. Rats were sacrificed at 0.5, 1, 2, 6 and 24 hours after formalin injection. Rat brains were removed and sliced in rat brain matrix. Brain slices were coronally sectioned at interaural 5.70 ~6.70 mm. Serial sections were immunohisto-chemically reacted with polyclonal c -Fos antibody. The numbers of c -Fos protein immunoreactive neurons in the cingulate cortex, primary somatosensory area, and hippocampus were counted and analyzed statistically with Mann - Whitney U test. Results were as follows: 1. Higher numbers of c -Fos immunoreactive neurons were found in the cingulate cortex, primary somatosensory area and hippocampus. 2. Both aspirin -treated and -untreated groups, numbers of c -Fos immunoreactive neurons were significantly higher all time points than formalin -untreated group, which peacked at 2 hours. 3. The numbers of c -Fos immunoreactive neuron of the aspirin -treated group were less compared to the aspirin - untreated group at each time point. In conclusion, these results provide some basic knowledge in understanding the mechanism and control of formalin - induced somatic pain.

Thirty-nine patients with fixed drug eruption who visited the Department of Dermatology in National Medical Center from January 1g7g to December 1982 were selected. Among them, twentyseven patients performed skin biopsy and were evaluated histopathologically on the basis of duration. The results were as follows; ].In the early stage less than 1 week of duration, hydropic degeneration of basal cells, necrotic keratinocytes with eosinophilic cytoplasm and pknotic nucleus in the epidermis, and mixed cell infiltration with eosinophlis in the upper dermis were prominently. In the late stage more than 20 days of duration, acanthosis, an increased amount of melanin in the basal cell layer and presence of melanophages were found more prominent, however, hydropic degeneration of basal cells, papillary dermal ederna, inflammatory infiltration of mononuclear cells were found less prounced or absent.

The purpose of this study was to investigate the effects of different types of orthodontic force on the root resorption and repair in rat molar. 77 rats were divided into three groups; The control group was not equiped with orthodontic appliance between ncisor and first molar. The experimental group was subdivided into closed coil spring subgroup and elastic chain subgroup by the application methods of orthodontic force. Initial orthodontic force between incisor and first molar was 100g. Experimental period was 8 weeks; for 4 weeks the appliance was acting and for another 4 weeks, removed. Root resorption and repair in the root of first molar was examined by light microscope for histologic changes and by inductively coupled plasma spectroscopy(ICP) for quantitative changes. The results were as follows: 1. In the closed coil spring subgroup odontoclasts and root resorption were appeared one week earlier. 2. One week after orthodontic force was eliminated the repair response in the resorptive lacuna was seen in both subgroups. Delayed resorption was seen on the periphery of resorptive lacunae whereas reparative response was seen in the center of lacunae. A new resorption was seen one week after orthodontic force was eliminated. Root contour was partially restored by repairing of resorbed root. 3. The weight ratios of calcium and phosphorous to the sample were decreased during resorptive process but increased during repair process in both the orthodontic groups, but not more than the control group. 4. By different types of orthodontic force (closed coil spring or elastic chain) resorption process was affected but repair process was not.

Dandy-Walker Syndrome*

Erythropoietic protoporphyria(EPP) is an inherited inborn error of porphyrin metabolism caused by decreased activity of the enzyme ferrochelatase. EPP is characterized clinically by photosensitivity to visible light commencing in childhood, and biochemically by elevated red cell free protoporphyrin levels. We report herein a case of EPP which occurred in a 44-year-old man and his family. He had suffered from immediate photosensitivity since he was 4 years old. He was presented with burning, erythema, scars and waxy thickening of the sun-exposed skin. Red cell free protoporphyrin level was elevated and urinary porphyrins were normal. Histopathologically, homogeneous eosinophilic materials that stained with PAS were deposited in perivascular area of upper dermis. He was managed with light restriction and sunscreen.

BACKGROUND: Intracoronary stenting in large coronary artery with diameters >3 mm has been shown to be beneficial in the treatment of acute or threatened closures complicating balloon angioplasty and in the prevention of restenosis. However, whether equally favorable results are afforded by stent placement in small vessels (<3 mm) remains unclear. Accordingly, we evaluated the safety and feasibility of intracoronary stenting in native coronary vessels less than 2.75 mm in size. METHODS: Between January 1997 and July 1998, seventy eight patients with 81 lesions were treated with 83 stents, regardless of clinical setting. The angiographic criteria for enrollment included at least 70% stenosis and a vessel that reference diameter was less than 2.75 mm. Every patients received aspirin (300 mg qd, indefinitely) and ticlopidine (250 mg bid, one month) and was given a bolus dose of 10,000 U heparin during procedure. RESULTS: Angiographic success was achieved in 80 of 81 attempts (98%). There was one in-hospital death because of pump failure in AMI patient. There was no acute stent thrombosis. At 6 month follow-up, event free survival was achieved in 90% of patients and angiographic restenosis was found in 28% of patients (9/32). CONCLUSION: The present observational study demonstrates that angiography-guided stent placement in coronary artery < 2.75 mm in diameter is safe and effective in conjunction with current stent deplyment technique and antiplatelet protocol.

PURPOSE: In a previous report, it took several days for white matter lesions to regress in hypoglycemic encephalopathy. We present a case of rapid diffusion-weighted image (DWI) changes in hypoglycemic encephalopathy. CASE REPORT: A 58-year-old male patient was found semi-comatous with the only abnormality in his laboratory tests showing hypoglycemia (44 mg/dL). After rapid correction of glucose level, immediate brain DWI showed bilateral subcortical white matter lesions. After about 5 hours, follow-up DWI showed resolved subcortical white matter lesions, with newly-appeared bilateral fronto-temporo-parietal cortical lesions. CONCLUSION: Both white matter and cortex involvement in hypoglycemic encephalopathy has been shown in several reports, but rapid regression of white matter changes in hypoglycemic encephalopathy has been rarely reported. It is important to know that MR imaging changes in hypoglycemic encephalopathy can be made as quick as just a few-hour-long.
