1.A Case of the Tolosa-Hunt Syndrome.

Painful ophthalmoplegia due to idiopathic granulomatous inflammation of the cavernous sinus or superior orbital fissure has been termed Tolosa-Hunt syndrome. We have experienced a case of Tolosa-Hunt syndrome in an 11 year old boy who has diagnosed with several clinical signs and symptoms such as severe ocular pain, headache, ptosis, proptosis, slight limitation of eyeball motion, nausea and vomiting, and the brain and orbital C-T scan which shows the right ethmoid and sphenoid sinusitis and inflammatory thickening of dura an the planum spenoidale.

2.Clinical Study of Functional-Below-Knee Total Contact Cast on Tibial Fractures

Functional-Below-Knee Total Contact Cast characterized by early weight bearing and free motion of affected knee joint during the cast immobilizaton has brought to significant result on trearment of tibial fractures, and is considered to take early bony union by establishment of almost physiological environment on fracture site during immobilization. For recent 2 years, writers have experienced many tibial fractures to treat with Functional Below-Knee Total Contact Cast. In them, clinical data and results of 17 cases which were studied well are reviewed, and method of cast application is illustrated.

3.Secretion and Excretion of Tears in Normal Eyes of Korean.

The complaint of tearing may be due to 1) partial or complete obstruction of the excretory mechanism, 2) increased lacrimal secretion, or 3) decreased basic lacrimal secretion. So the physician should ascertain whether an excess of tears or obstruction of excretory mechanism is present. The most widely used tests for this purpose are the Schirmer test, Jones test and fluorescein dye disappearance test. The Jones test consist of Jones I test and Jones II test, which determine the functional and anatomical integrity of the lacrimal excretory system. In this study, Schirmer test, Jones test and fluorescein dye disappearance test were performed on 400 normal and 100 abnormal eyes to determine the secretion and excretion of tears.

4.A Case of Conjunctival Aspergillosis.

Conjunctival aspergillosis refers to the infectious process of conjunctiva caused by one of the numerous members of the genus Aspergillus. A 4 year old Korean boy was brought to our hospital due to localized injection and edema of the left temporal bulbar conjunctiva which did not respond to the systemic and topical antibiotics for about 2 weeks. Clusters of the Aspergillus element was identified by the conjunctival biopsy and the diagnosis was made.

5.Revision Total Knee Arthroplasty Using the PFC Sigma Knee System.

PURPOSE: We wanted to analyze the causes of revision and the clinical and radiographic results after revision total knee arthroplasty. MATERIALS AND METHODS: Thirty-eight knees in 34 patients underwent revision total knee arthroplasty using the PFC(R) Sigma revision system between March 1999 and May 2005. The average patient age was 71.2 years, and the average follow-up period was 6 years 7 months (range: 4 years~10 years 2 months). We evaluated the time interval between the primary and revision arthroplasty and the causes of revision. The clinical outcomes were assessed using the knee score and the functional score of the Knee Society Scoring system (KSS system), and the radiographic measurements were assessed using the roentgenographic method of the American Knee Society. RESULTS: The mean interval between primary and revision arthroplasty was 6 years 3 months. The causes of revision included 25 cases of aseptic loosening and osteolysis, 7 cases of infection, 3 cases of joint stiffness and 3 cases of polyethylene problems. At the follow up, the mean preoperative knee and functional scores improved from 45.1 points to 84.5 and 37.2 to 78.2, respectively. Radiographycally, the average femorotibial angle was corrected from 2.2degrees in valgus to 6.5degrees. The radiolucency score was below 4 in all the cases. The complications after revision arthroplasty were 2 cases of superficial infection and 1 case of wound dehiscence with a skin defect. CONCLUSION: The main causes of revision arthroplasty were aseptic loosening and osteolysis. Satisfactory results were obtained using the posterior cruciate ligament and substituting the PFC(R) Sigma knee system with appropriately selected metal augmentation and an allograft. This study showed favorable results on long term follow-up.

8.A Case of Palisaded Encapsulated Neuroma of the Lower Lip.

Palisaded encapsulated neuroma(PEN) is a rare intraneural neuroma. It usually occurs as a solitary, asymptomatic, skin-colored papule, and commonly affects the butterfly area of the face of middle-aged adults. However, it rarely involves oral mucosa including lip and should be differentiated from mucosal neuromas which generally occur as multiple small nodules of the lips and the anterior part of the tongue. We report a case of PEN occurring on the lower lip in a 33-year-old male. Histopathologically, it appeared as a well-circumscribed, encapsulated round nodule in the dermis. The nodule was composed of well-developed fascicles of wavy spindle cells separated by a loose matrix. Nuclear palisades were ill defined. On immunohistochemical staining, most tumor cells of the nodule were positive for S-100 protein and about 30% of tumor cells were positive for neural filament, but the capsule was negative for them. Epithelial membrane antigen was focally and discontinuously reactive on the capsule of the nodule.

9.A Case of ANCA-associated Pauci-immune Crescentic Glomerulonephritis in Juvenile Rheumatoid Arthritis.

Juvenile rheumatoid arthritis(JRA) is the most common major connective tissue disease in children. Renal involvement in JRA is rare. Among the renal lesions that have been reported in JRA, amyloidosis and drug-induced nephropathy are the most common. Crescentic glomerulonephritis in JRA has rarely been reported. We report a case of ANCA-associated pauci-immune crescentic glomerulonephritis in JRA. The patient was a 15-year old boy with a 3-year history of JRA. He presented with gross hematuria, proteinuria, positive p-ANCA and elevation of BUN and creatinine. Pathologic findings revealed focal necrotizing and crescentic glomerulonephritis. There were no significant immunoglobulin or complement deposits. His renal function recovered after intravenous methylprednisolone pulse therapy and oral steroid use. In Korea, this is the first reported case of pauci-immune crescentic glomerulonephritis in JRA.

10.New Zygometer Incorporating Exophthalmometer.

The zygoma is essential key element determining midface contour and symmetry. The height and width of the zygoma are changed during midfacial surgery such as reduction of fractured zygoma, esthetic malar reduction and correction of congenital midfacial deformity. The exact preoperative and postoperative evaluation of the zygoma is very important step in midfacial surgery. The estimation of height and width of the zygoma usually depends on gross feature analysis or radiographic measurement. But the results are not accurate and subjective. Several device were used for measuring the height and width of the zygoma, but those are inconvenient and difficult to use. The fracture of the zygoma is frequently combined with blow out fracture resulting enophthalmos. The simultaneous measurement of accurate malar height and degree of enophthlmos is helpful. For this reason, we developed a new zygometer with the function of exophthalmometer. This device is convenient to use and easy for comparative measurement of malar height preoperatively and immediate postoperative period, and the degree of enothphalmos is measured simultaneously with this device. This new zygometer will be applied to many midfacial surgery and craniofacial study.
