1.Immunohistochemical Detection of Prekeratin and Keratin in Hair Follicles and Epidermis of Human Fetus.

Authors attempted to examine the development of keratinocyte in the human fetal epiderrnis in term of the appearance of principal product of differentiation, the keratins. Immunohistochemical study (ABC method) using anti-prekeratin antibodies and anti-keratin antibodies was carried out on sampled skin from 9 human fetuses ranging in estirnated gestational age from 12 weeks to 27 weeks. As early as 12 weeks of embryonic life, the epidermis contained cytokeratin. At this stage of development, the embryonic epidermis consists of three-cell layer .' basal layer, intermediate layer, and periderm, all of them staining for prekeratin and keratin. A thick cornified layer developed at 24 weeks but was not stained for prekeratin and keratin. Hair germ of 12 week-fetus was recognized as bulges of basal cells that were stained for prekeratin and keratin. At 18 weeks of estimated gestational age, development has advanced to the bulbous hair peg stage at which point the primordia of nearly all the final structural components of follicles were established. In the final structureal cornponents of the follicles, outer root sheath and sebaceous gland were stained for prekeratin and keratin, whereas cortical and cuticular elements and inner root sheath were negative. There was no difference between immunostaining results for prekeratin and keratin except for the staining intensity. The appearance of these proteins early in development suggests their potential value in utero in the recognition of keratinization disorders.

There was decreasment of the domestic animal rabies, but increasment of the sylvatic animal rabies now a days in the world. We could see stable statistics of the ministry of health and social affairs annuala report, however, only rabies we could occationally find out suggestive human rabies by domestic animal for the slight more incidence in Korea. Now, we report on the three cases of sugg. Rabies in our children and review of literature. We think the improvement should be needed for postmortem examination that isn't taking by the Korean custom.

5.Immunohistochemical Study on Cytokeratin Expression in Epidermis of Human Fetus.

To identify the developmental characteristics of intermediate filaments, the expressions of various cytokeratines (CK), desmin and vimentin in fetal (14032 weeks of gestations) and adult epidermis were studied immunohistochemically. The primary antibodies used were CK7, 8, 10, 14, 18, AE8, 5D3, and MNFl16 for cytokeratins, D33 for desmin, and V9 for vimentin. At 14 weeks of gestation, the epidermis consisted of basal cells and periderm. The periderm exhibited positive staining for CK8 and AE8, and weak staining for MNF116 and D33. The basal cells showed positive staining for MNF116 and D33. The epidermis did not reacted for CK7, 10, 14, 18, 5D3, and V9 at this period. At 16-20 weeks of gestation, the epidermis was composed of basal, intermediate, and periderm layers. The periderm was positive for CK8, 18, AE8, MNF116, and D33. The intermediate cells were positive for CK10 and the basal cells CK14, MNF116, and D33. Few cells were stained positively with V9 among the basal cells. At 24-32 weeks of gestation, the epidermis exhibited no longer positive reactions for CK8, 18, AE8 and D33. The intermediate cells were positive for CK10. Immunoreactivity for MNF116 was noted in intermediate layer just above the basal layer. CKl4, MNFl16, D33, and often V9 were expressed in basal cells. The expressions of CK7 and 5D3 were not observed at any period of gestation. In adult epidermis, basal cells exhibited positive staining for CKl4, MNFl16, and D33. The intermediate cells were strongly positive for CK10, and weakly positive for CK7, 8, and MNFl16. The cells positive for V9 were often present among the basal cells. These results indicate that CK8 and 18 may serve as useful markers for periderm, CK10 for intermediate cells, CKl4 for basal cell, and suggest that the vimentin immunoreactive cells in basal cell layer are Langerhans cells.

6.A Case of Myxoid Malignant Fibrous Histiocytoma.

Malignant fibrous histiocytoma is the most common soft tissue sarcoma in adult life and shows a mixed proliferation of both fibroblsstic & histiocytic cells. We report a case of myxoid malignant fibrous histiocytoma involving right lower leg in a 68-year-old male patient, who presented with a childs palm-sized nodular mass of 3 years duration. Histoapthologically, the tumor had varying proportion of myxoid cellular components. The myxoid areas showed spindle and pleomorphic cells arranged in a vague storiform pattern, abrupt transition between a solidly cellular area and a myxoid area, and lipoblast-like cells. The cellular areas showed spindle cells arranged in a storiform pattern characteritic of the typical pleomorphic malignant fibrous histiocytorna.

7.Therapeutic Effect of Ketoconazole in Onychomycosis.

This study was undertaken to evaluate the efficacy and safety of orally administered ketoconazole in the treatment of onychomycosis. Thirteen patients with onychomycoiis were selected for this study. For evaluation of ketoconazole efficacy twelve patients received a 200 mg tablet of ketoconazole daily and one patient received a. 10() mg tablet of ketoconazole daily. All patients were studied monthly clinically and mycologically by KOH mount and culture on Sabourauds medium. The results were as follows: 1. Seven(53.8%) of thirteen cases achieved remission, marked improvement occurred in three cases(23.1%) and moderate improvement occurred in three cases (23. 1 %). 2. The mean duration of remission or clinical improvement was 5 months for all lesions and toenail lesions(5. 4 months) took longer to heal than finger nail lesions (4 months). 3. One hundred percent of patient who previously failed to respond to griseofulvin were cured or clinically improved by ketoconazole. 4. Two patients cornplained of indigestion and one of headache. In our study, ketoconazole was highly effective in the treatment of onychomycosis.

8.Histochemical Studies of Fetal Arteries of Koreans with Special Reference to Atherogenesis in Adults.

In view of the sparsity of report on normal and abnormal pattern of the major arteries during fetal life, the authors undertook investigation of the aortas, coronary arteries, renal arteries, and the umbilical arteries of 76 korean fetuses, ranging from 21/2 months to full-term, A normal pattern and its evolution of the intima, media, and the adventitia was described. The P. A. S. positive substance was most abundantly found in the media of the umbilical arteries, medium amount in the media and intima of the renal arteries and the aortas, and lesser amount in the media of the coronary arteries. A surprisingly high incidence of the alteration of the internal elastic membrane of the aorta simulating to the early lesions of atherosclerosis in adult and neonatal life was observed. In e1even instances, microcystic degeneration of the inner media of the aorta was observed, and its relationship to the idiopathic cystic medial necrosis and dissecting aneurysm was discussed.

9.Clinical correlation with CSF fingings of Herpes Zoster.

BACKGROUND AND OBJECTIVES: Herpes zoster(HZ) is a well known viral infection of the nervous system. HZ is associated with the neurologic complication. It may have been expected a change in the cerebrospinal fluid(CSF) of patients with HZ. The purpose of this paper is to present the correlation between CSF findings and neurologic complications in HZ. METHOD: The patients(n=75) with HZ were 31 male and 26 females, who were ranged in age from 15 to 79years(mean 53.4 years) admitted to department of Neurology of Chung Ang Gil General Hospital from June 1992 to May 1995. The patients were evaluated for the etiology, lesion distribution, and complications. CSF study was performed in 32 patients(male/female=20/12), whose age ranged from 16 to 79 years(means 51years) during 5 days to 10days after initial symptom onset. CSF findings were analyzed for their etiology, lesion distribution and complication. RESULTS: The CSF of 25 patients(78.1%) showed pleocytosis(5 to 49/mm3 in 10 patients, means, 62.1/mm3). 7 patients(12.3%) had meningitis symptoms, whose cell counts of CSF were more than 50/mm3(means 128.4/mm3) postherapeutic neuralgia was more common in elder patient than younger patients(p=0.005). Complications tended to correlate with the cell counts of CSF. There was no difference in CSF finding between cranial segmental involvements. CONCLUSION: We confirmed the pleocytosis in CSF of patients with HZ and suggest the development of complications in patients in patients with HZ tend to correlate with the cell counts of CSF.
