1.A Case of Disseminate and Recurrent Infundibulo-folliculitis.

Disseminate and recurrent infundibulo-folliculitis is a rare,but distinctive dermatosis characterized by pruritic papular follicular eruption over the trunk and proximal extremities with histologic findings of lymphocytic infiltrations and spongiosis confined to the infundibulum. Little is known about the cause. Resistance to treatment is one of the characteristics of this disease. We herein report a case with typical clinical and histological findings in a 22 year old male.

2.Testosterone replacement therapy and monitoring for the male patients with testosterone deficiency syndrome.

Since the elderly population has been increasing recently in our country, old male patients with testosterone deficiency syndrome (TDS) with a significantly decreasing quality of life are becoming increasingly common. TDS in males is defined as a biochemical syndrome associated with advancing age and characterized by clinical manifestation and a deficiency in the serum testosterone level. These patients should be treated with extrinsic testosterone to improve quality of life. TDS in males should be diagnosed in the case of clinical manifestation with serum total testosterone <8 nmol/L (230 ng/dL) or calculated free testosterone <225 pmol/L (65 pg/mL) but not diagnosed in the case of serum total testosterone >12 nmol/L (350 ng/dL). Products for testosterone replacement therapy (TRT) are administrated orally, transdermally, and through injectable preparations. Daily testosterone undecanoate is widely used for oral administration with good results and no hepatotoxicity. Short-acting intramuscular preparations are very effective but show wide swings in the resulting supra-physiological level of serum testosterone. Long-acting intramuscular preparations is also very effective and lasting for 3 months with normal physiologic levels. Many products for TRT on the market are effective and generally safe. However, those have a few significant adverse events each other. The ideal product should have notable effectsand few side effects, (such as selective androgen receptor modulators), be easy to administrate, maintain physiologic serum concentration, and be inexpensive. TDS in males can easily be correct by TRT. However, the advantages and disadvantages of the individual products and follow-up management of complicated adverse events should be understood before starting and maintaining TRT.

3.MR Fundings of the Transligamentous Type of the Herniated Lumbar Disc.

PURPOSE: The purpose of this study is to describe the findings of the transligamnetous type of the herniated lumbar disc (HLD) with magnetic resonance MR imaging. MATERIALS AND METHODS: We retrospectively analyzed the MR images of surgically proven 20 cases of transligamentous type of HLD from January 1, 1992 to August 20, 1992. The MR imaging was performed with 1. 0T MR unit, using sagittal spin echo (SE) and axial gradient echo (GE) techniques. RESULTS: The results were as follows;1) the interruption of black line of the posterior longitudinal ligament (PLL) was identified in 19 levels and 17 levels in sagittal SE and axial GE images, retrospectively;2) the widening of adjacent epidural fat space was demonstrated in 16 cases of central or posterolateral HLD. The herniated disc material, as compared with the parent intervertebral disc, showed intermediate signal intensity (SI) in 19 and low SI in 1 level on T1WI, high SI in 10, intermediate SI in 7, low SI in levels on T2WI, and high SI in 10, intermediate SI in 9, low SI in 1 level on GE images. CONCLUSION: The most important sign of the transligamentous type HLD on MRI was the interruption of the black line with additional finding of the widening of adjacent epidural fat space. The MR signal intensity of the herniated disc was variable. Axial GE image was valuable for the evaluation of the direction of HLD and it relationship with neural structure, but had no addiational information for the degree and biochemical change of HLD over SE image.

7.Subdural Empyema Concomitant with Bilateral Subdural Effusion in Infant after Meningitis.

Subdural empyema is a fulminating, purulent, bacterial infection located between the dura and arachnoidal membrane. Early diagnosis, adequate antibiotic therapy and surgical treatment played important roles in the successful management of subdural empyema. Subdural empyema can develop secondary to sinusitis, otitis media, meningitis, trauma or craniotomy. Subdural empyema in infants commonly develops secondary to infected subdural effusion associated with purulent meningitis. The incidence of this complication of meningitis in infants is approximately 2%. We report a case of subdural empyema concomitant with bilateral subdural effusion, which was successfully treated with surgical intervention and antibiotic therapy.

DP)-specific IgE antibody using enzyme-linked immunosorbent assay(ELISA) in the supernatant of nasal polyp homogenates from 72 subjects undergoing nasal polypectomy. The subjects were divided into three groups according to skin reactivity to DP: 20 strongly atopic subjects to group I(mean wheal diameter) 3mm), 19 weakly atopic subjects to group II (mean wheal diameter 1-3mm) and 33 negative skin responders to group III. 10.Stereotactic Aspiration of HICH by CT-Guided U-Loop and Leksell System.

We reviewed 178 patients with hypertensive intracerebral hematoma(HICH), which were treated with computed tomography(CT) guided stereotactic aspiration from Aug. 1986 to Dec. 1993(166 CT guided U-loop and 12 Leksell system). Our series consisted of 82 males and 96 females. The hemorrhages were located in putamen in 67.4%, thalamus in 20.2%, subcortex in 9% and cerebellum in 3.4%. Mean removal rate of hematoma was 55.3%. The results for the 178 cases who underwent stereotactic aspiration were good recovery in 45.5%, moderate disability in 28.0%, severe disability in 9.0% and vegetative state in 4.0%. The infection rate and rebleeding rate of all cases were 1.7% and 6.8% respectively. The mortality rate of all cases was 13.5%. We analysed the factors affecting the prognosis in 178 patients. Factors affecting good prognosis were high initial GCS, less amount of initial hematoma and no presence of intraventricular hemorrhage. Stereotactic aspiration by CT-guided U-loop and Leksell system is simple, safe and can minimize the brain damage. This method can e performed under local anesthesia.
