The development of carcinoma in a subcutaneous dermoid cyst is a rare event. We report a 54-year-old Korean woman in whom excised subcutaneous dermoid cyst showed squamous cell carcinoma in the cyst wall.

Authors attempted to examine the development of keratinocyte in the human fetal epiderrnis in term of the appearance of principal product of differentiation, the keratins. Immunohistochemical study (ABC method) using anti-prekeratin antibodies and anti-keratin antibodies was carried out on sampled skin from 9 human fetuses ranging in estirnated gestational age from 12 weeks to 27 weeks. As early as 12 weeks of embryonic life, the epidermis contained cytokeratin. At this stage of development, the embryonic epidermis consists of three-cell layer .' basal layer, intermediate layer, and periderm, all of them staining for prekeratin and keratin. A thick cornified layer developed at 24 weeks but was not stained for prekeratin and keratin. Hair germ of 12 week-fetus was recognized as bulges of basal cells that were stained for prekeratin and keratin. At 18 weeks of estimated gestational age, development has advanced to the bulbous hair peg stage at which point the primordia of nearly all the final structural components of follicles were established. In the final structureal cornponents of the follicles, outer root sheath and sebaceous gland were stained for prekeratin and keratin, whereas cortical and cuticular elements and inner root sheath were negative. There was no difference between immunostaining results for prekeratin and keratin except for the staining intensity. The appearance of these proteins early in development suggests their potential value in utero in the recognition of keratinization disorders.

Nevertheless the hospital incidence figures usually offer the only available means of assessing prevalence, and therefore provide us with a potentially valuable source of information on the ecology of many skin disorders. 27 Colles' fracture were treated with closed reduction and percutaneous k-wire fixation under the C-arm fields, from July 1980 to September 1984 at the Department of Orthopedic Surgery of Eul Ji Hospital. The average duration of the follow up was 4.9 Months. A prospective study was made and evaluated under the subjective and objective criteria of Gartland and Werley, and the objective criteria of Scheck. The results of this study were as follows: 1. The incidence was high in 4th and 5th decade as 66. 5%, not significant between females and males, 45% of the cause of injury was falling accident. 2. In 27 cases, showed the satisfactory result about 92.6%, but, in the eomminuted intraarticular fracture group, 2(15.4%) out of 13 cases showed unsatisfactory result. 3. Better result was obtained with more secure reduction and skillful technique, in certain case, considered that the open reduction and internal fixation with k-wires would be better than percutaneous k-wire fixation. 4. In the aspect of hand motion and the concept of improved anatomical restoration and maintenance would lead to improved function, ideal method was percutaneous k-wire fixation and casting after closed reduction under the C-arm fields.

We report a case of cerebriform intradermal nevus. a cause of cutis verticis gyrata that clinically they resembled neurofibroma and revealed histopathologic finding consistent with intradermal nevus. The patient was a 6 year-old boy, who had a 17x18cm, soft, convoluted. pigmented mass over the parietal and occipital regions. The lesion had been present since birth and had been slowly increasing in size with convolution.

The occurrence of the colonic obstruction secondary to colorectal carcinoma (CRC) has been reported in 7~30% of the CRC patients. It is generally believed that obstructive CRC is associated with a poor prognosis with respect to operative mortality and five-year survival. A series of 1064 cases of the CRC treated surgically at Asan Medical Center from June 1989 to December 1996 has been analyzed to compare clinicopathological findings between obstructive and non-obstructive CRC and to evaluate surgical treatment options in obstructive CRC. Complete obstruction was present in 49 cases (4.6%). There were no differences between obstructive and non-obstructive CRC in tumor location, size, Dukes' stage, and differentiation. In forty-nine obstructive CRC cases, primary resections were performed in 29 cases after peri-operative bowel decompression. In this group, right colon cancer was more prevalent than staged operation group (45% vs. 5%, P<0.05) and hospital stay was significantly short (16 days vs. 38 days, P<0.05). Postoperative complication rate was higher in staged operation group (65% vs. 28%, P=0.01). It may be due to stoma related wound complication. In obstructive left colon cancer, there was a significant difference in complication rate between primary resection and staged operation (P<0.05). Overall 5-year survival rate were 66% and 53% in non-obstructive and obstructive group, respectively. Survival rate according to the Dukes' B and C stages did not show statistical differences, either. Conclusively, primary resection is preferred to the obstructive CRC when supportive care, preoperative bowel decompression, and intraoperative colonic irrigation were performed adequately.

Recently, a topical preparation of 5% propantheline bromide and 10% aluminum hydroxychloride in an emulsion ground substance was introduced as an effective antiperspirant. According to the literature, aluminum salts may produce functional closure of swet ducts. Nevertheless, histologic study has as yet failed to reveal a solid anatomic basis for occlusion. In this study, the scanning electron microscope (SEM) was used to demonstrate eccrine gland ostial occlusion due to topical 5% propantheline bromide and 10% aluminum hydroxychloride.
