1.Vitrectomy in Complicated Pars Planitis.

Consecutive twelve patients' twelve eyes with complicated pars planitis underwent pars plana vitrectomy at our center in the period from January 1988 to December 1990. Patients' ages ranged from 24 to 60(mean 39.9)years and all were male. Nine patients had histories of topical or systemic steroid therapy and at the time of surgery, all eyes were quiet from active inflammation. With various degrees of vitreal opacities, five eyes had macular pucker or membrane only and in other seven eyes, retinal detachments of either tractional or tractional-rhegmatogenous origin were arisen. In the later group, macular hole was associated in two eyes. Surgical procedures combined were membrane peeling(five eyes), scleral buckling(ten eyes), fluid/air-gas exchange(eight eyes), lensectomy(two eyes) and photocoagulation(two eyes). During the follow-up period ranged from four to twenty-one(mean 7.75)months after first operations, the uveitis recurred or continued to be activated in four eyes and as a complication, retinal detachment occurred in two eyes which had macular pucker only preoperatively. One refused reoperation and the other was reattached successfully by second operation. Final visual acuities were improved in eight eyes, stationary in one and worsen in three. The causes of worsen vision were retinal detachment, macular edema and macular slanting by buckle respectively. Preoperatively, the majority of eyes(10 eyes: 84%) had less than 0.09 vision and postoperative vision turned over better than 0.09 in 7 eyes(59%). Although we did not confirm extinguished therapeutic effects of vitrectomy on the pars planitis perse, our results revealed some positve role of surgical intervention in the complicated pars planitis.
2.Taxonomic Studies on the Genus Marssonina in Korea.

Eight species of Marssonina parasitic on 21 species of host plants from Korea are described and illustrated. They are Marssonina brunnea (Ellis & Everh.) Magnus, M. capsulicola (Rostr.) Magnus, M. celastri H.D. Shin & H.T. Lee, M. coronaria (Ellis & Davis) Davis, M. fragariae (Lib.) Kleb., M. juglandis (Lib.) Magnus, M. rosae (Lib.) Died. and M. sennenis (Gonz. Frag.) Vassiljevsky & Karak. Of these, M. capsulicola and M. coronaria sometimes poss three-celled conidia and M. sennenis is characterized by a short appendage at the basal end of the conidia. Morphological features of conidia and host ranges were of taxonomic values for species delimitation.

3.Clinical Evaluation on Serial Changes of Serum Immunoglobulin E in Patients with Acute Myocardial Infarction.

Serial changes of serum IgE, IgG, eosinophils were observed in 25 patients with acute myocaridial infarction and 20 ischemic heart disease without evidence of acute myocardial infarction and evaluated in terms of several parameters and its clinical significance. The results observed were as follows : 1) Serum IgE levels were propgressively elevated from the first hospital day(259+/-3IU/ml) up to peak level of the fifth hospital day(415+/-2IU/ml) and progressively lowered and returned to almost same level as the first hospital day on the twenty first hospital day. On the other hand control group showed significantly lower IgE levels throughout all hospital day and also did not showed serial change. 2) In the patient group with the initial serum IgE level above 200IU/m; showed significantly lower level of serum SGOT, CPK level than the group of below 200IU/ml group. This suggests the initial serum IgE level might have some correlation of the extent of myocardial necrosis. 3) In patients of acute myocardial infarction, ejection fraction was checked at discharge. Initial serum IgE level above 200IU/ml group showed significantly higher ejection fraction than below 200IU/ml group(59.4+/-13.5% vs 38.4+/-13.7%). 4) Serum IgE was checked concomittantly with serum IgE. It showed slightly decreasing tendency at third hospital day but not statistically significant. Eosinophil changed similar pattern as serum IgE but it was also not statistically significant. In conclusion, serial checking of serum IgE level in patient of acute myocardial infarction may give some help in prediction the clinical course and prognosis.

5.Clinical Observation on Effect of Diltiazem(Herben(R)) in Angina Pectoris.

We evaluate the effects of diltiazem in 19 patients with ischemic heart disease (15 patients) of classical anginal pectoris & 4 patients of variant angina) by means of clinical status & electrocardiographic changes and obtain the results as follows: 1. The pulse rate & blood pressure were decreased by diltiazem slightly but these decreases were not significant in statistical meaning. 2. Diltiazem administration of 4 weeks duration normalized EKG completely in 4 patients & partially in 3 patients among the 13 patients who showed abnormal resting EKG initially. 3. All 19 patients who received diltiazem showed clinical improvement; 9 patients had excellent responses, 7 patients good responses & 3 patients fair responses. 4. Diltiazem had side effects in 3 patients, drowsiness, mild euphoria & possibly tolerance respectively in each patients.

6.Differences in clinical laboratory data between the healthy elderly and the healthy young adults.

BACKGROUND: The elderly is known to have many different clinical laboratory data compared with the young adults. But, in Korea, such study is lacking. METHODS: We gathered 98 healthy elderly cases and 98 healthy young adult controls who have taken periodic health examination from January 1993 to May 1996 at one hospital and we compared the mean of various clinical laboratory data between the two groups. RESULTS: In both sexes, cholesterol, ESR, glucose were significantly higher in the elderly t,han in the controls and direct bilirubin was significantly lower in the elderly than in the controls(P<0.05). In men, albumin, calcium, hemoglobin were significantly lower in the elderly than in the controls and MCV was significantly higher in the elderly than in the controls(P<0.05). In women, alkaline phosphatase, LDH, triglyceride, GOT, GPT, T3,TSH, phosphate were significantly higher in the elderly than in the controls and total bilirubin, protein, uric acid were significantly lower in the elderly than in the controls(P<0.05). WBC count, platelet count, neutrophil count(%), lymphocyte count(%), eosinophil count (%), monocyte count(%), MCH, MCHC, BUN, creatinine, HDL were not significantly different between two groups. CONCLUSIONS: Many clinical laboratory data are different between the elderly and the young adults, and some clinical laboratory data have sexual differences.

7.Cutaneous Plasmacytoma Associated with Multiple Myeloma.

A 52-year-old man with multiple myeloma developed cutaneous nodules while being treated with melphalan and prednisolone. A biopsy specimen showed dermal infiltration by well differentiated plasma cells similar to those found on bone marrow biopsy. The use of peroxidase anti-peroxidase to demonstrate the monoclonality or polyclonality of the cytomplasmic immunoglohulins in the tumor cells revealed a positivity for IgG and 1 chain (ie, monotypic staining). Ultrastructurally, each plasmacytoma cell contained varyting amounts of rough endoplasmic reticulum and Golgi-apparatus. The cutnneous nodules completely disappeared after radiotherapy
