1.Computed tomographic evaluation of brain abscess

The use of CT is most reliable in diagnosis and management of brain abscess. Authors analized 17 cases of pathologically and clinically proven brain abscess during the period of 39 months from Jan. 1978 to Mar, 1982 at National Medical Center. The results were as follows; 1. The sex ration 9 males to 8 females, and no sexdifference was seen, and the greatest number of cases were seen below the age of 30(65%). 2. The otogenicinfection was the most frequent predisposing factors(8 cases). Other predisposing factors were postoperative infection (2 cases), pulmonary infection (2 cases), and congenital heart disease(2 cases). The most common site ofinvolvement was posterior fossa(5 cases). Next was temporal lobe (4 cases), and temporoparietal lobe (3 cases). 3.Most common presenting symptoms were headache, fever, focal neurological signs, and dizziness. 4. Among the 22 brain abscesses of 17 patients, the msot frequent CT finding in precontrast scan was a low density surrounded by afaint dense or dense ring (11 cases). Next was purely low density (6 cases). Associated hydrocephalus was found in4 cases, and multiple or multiloculated abscess was seen in 4 cases. 5. In post contrast scan, brain abscessusually show complete, oval or round shaped, thin, evening righ enhancement with mild or moderate surroundingedema, but there was no specific enhancement.

2.A Case of Severe Pituitary Dwarfism due to Agenesis of Anterior Pituitary Gland with Pituitary Stalk Transection.

We experienced one case of severe pituitary dwarfism in a 10 years old female girl. Magnetic resonance image (MRI) revealed transection of the pituitary stalk stalk with the formation of high intensity ectopic posterior lobe located at the median eminence and agenesis of an anterior lobe of pituitary gland. The serum growth Hormone (GH) response to clonidine and L-dopa revealed severe GH deficiency. The patient had responses to TRH, normal TSH and partial prolactin response, respectively. There was not response LH and FSH to GnRH. The morning cortisol concentration and serum T4 concentration were decreased below the normal range. These findings and no hyperprolactinemia suggested the presence of a vascular connection between the pituitary gland and hypothalamus, which is not visible on MRI. Sofar, the primary cause of idiopathic pituitary dwarfism in many patients is injury to hypothalamus by perinatal insults. In this patient, there was no history of perinatal insults and postnatal head trauma but transection of the pituitary stalk. We report a case of severe pituitary dwarfism due to agenesis with brief review of related litereature.

5.Radiological study of primary lung cancer

The incidence of primary lung cancer is increasing in our country. This presentation is an attempt to correlate a useful diagnosis with radiological findings of primary lung cancer. Histologically proved 210 cases of primary lung cancer are as follow: Epidermoid Ca. 65%, Adeno Ca. 18%, Small cell Ca. 9.5%, Large cell Ca. 2%, Unclassified Ca. 3.3%, Mucoepidermoid Ca. 2.4%, Mixed Ca. 0.5%. 3. In smoking history; 91% of epidermoid Ca, 17%of small cell Ca. 30% of adeno Ca. has smoking history. 4. In mass size; Average diameter of mass was most was5.3cm. Hilar mass was seen in 48 cases. Hilar mass was most frequent finding in small cell Ca. and the peripheral mass in adeno Ca. 5. Cavitary lung Ca. was 26 cases (12.4%) and most cases had relative thick wall and its mean thickness was 3.6mm. 6. Coexistence of pulmonary tuberculosis and lung cancer was 22 cases (10.5%), and most common in epidermoid cell Ca. as 16 cases. 7. Other findings include metastasis to lymph node, bone and other organ, and pleural effusion.

6.Clinical Analysis of the infection on Open Wound in Traumatic Patients

One hundred and fifty two open wounds of extremities including open fractures were treated at the Kyoungpook Nstional Univ. Hospital from March 1977 to August 1977. Of 152 open wounds there were 32 cases (21.5%) of wound infection even though the initial wound culture revealed bacterial growth in 44 cases(28.9%). However, there were only 10 cases of infection among the cases with positive bacterial growth in the original culture and the same organisms were found in the 6 cases between the initial wound culture and post infection culture. Sensitivity studies of the organisms, the Cephalosqorines appeared to be most effective as a prophylactic antibiotics. It was sensitive to all gram positive organisms and majority of gram negative organisms including Escherichia coli. In bacterial culture and sensitivity studies of the 82 open fracture wounds 18 of them showed gram positive and gramnegative bacteria. The prominant organism were Staphylococcus, Escherichia coli, Streptococcus and Pseudomonas. The infection rate was lower in the cases whar the wound was managed within six hours from the time of injury than after six hours.

7.Nonclostridial Gas Producing Infection in the Diabetic

Gas producing infection accompanied by evidence of subcutaneous gas usually are diagnosed as clostridial gas gangrene. The occurrence of nonclostridial gas producing infection is thought to be relatively rare. Two cases of nonclostridial gas producing infection simulating clostridial gas gangrene in diabetes occurred. Gram stains of pus at the time of admission can isolate the E-coli and streptococcus, and can distinguish these two entities. Chief conplaints were swelling, pain and tenderness of right lower extremity by infection. Multiple incisions to free the tissues of gas and pus and the administration of antibiotics are the treatment of choice.

8.Pena-Shokeir I Syndrome in a Newbonrn Infant.

Pena-Shokeir I syndrome is a multiple malformation syndrome displaying characteristics of camptodactyly, multiple ankylosis, severe muscle weakness, facial anomalies (low set ears, hypertelorism, depressed tip of nose), polyhydramnios, fetal growth retardation & pulmonary hypoplasia which are inherited by autosomal recessive trait. We experienced 1 case of Pena-Shokeir I syndrome in a neonate (41 weeks, 2.08Kg). This patient suffered from dyspnea. Respiratory destress was not relieved after ventilatory care. He died aged 10 days. We report this case with brief review of literature.

9.Expansive Laminoplasty for Cervical Compression Myelopathy.

In an attempt to eliminate some negative aspects of conventional extensive laminectomy, 4 cases of multiple level of cervical compression myelopathy, 1 OPLL (ossification of posterior longitudinal ligament) and 3 cervical stenosis, were treated with a technique of expansive laminoplasty. Operative results in all patients were satisfactory without surgical complications and all patients had a neurological improvement. We suggest that our technique is more effective one for cervical canal stenosis, OPLL, and spondylosis than conventional extensive laminectomy.

Tuberous sclerosis is reported rarely and is associated with systemic lesions including central nervous system, skin, heart, eye and kidney. Approximately 5-15% of individuals with tuberous sclerosis will develop brain neoplasia, almost invariably subependymal giant-cell astrocytoma (SGCA). We experienced a case of SGCA with tuberous sclerosis operated by the transcallosal approach and report with literature review.
