3.The Effects of the Application of Human Amniotic Membrane Ointment in Filtering Surgery on Rabbits.

PURPOSE: We investigated the anti-inflammatory and anti-fibrotic effects of amniotic membrane(AM) ointment in filtering surgery of rabbits. METHODS: After filtering surgery was performed on both eyes of 10 rabbits, the AM ointment was applied over and under the scleral flap on one eye whereas base ointment excluding AM element was applied to the other as a control. The shapes of blebs were observed under slit-lamp biomicroscopy at postoperative 3 days, 1 week, 2 weeks and 4 weeks, and the tissues were excised for histologic studies. RESULTS: Serial changes of intraocular pressures had no significant difference. Slit-lamp biomicroscopy demonstrated that conjunctival blebs of AM ointment group were more elevated and cystic at 1 and 2 weeks. The heights of conjunctival blebs were significantly higher at 1, 2 and 4 weeks, and the width of scleral tracks was significantly wider at 1 and 2 weeks in AM ointment group. The number of inflammatory cells in the scleral track was significantly less at 3 days and 1 week, the number of fibroblasts in scleral track was significantly less at 2 and 3 weeks in AM ointment group. Masson s trichrome stain was less positive in AM ointment and the stain area of alpha-smooth muscle actin-positive fibroblasts was insignificant with image analysis system. Collagen type 3 appeared in scleral track from postoperative 1 week in control group, however, in AM ointment group, from 4 weeks. CONCLUSIONS: AM ointment is beneficial to suppress fibroblast differentiation, proliferation and inflam-matory reaction in filtering surgery and might be safe and effective adjunctive for enhancement of success in filtering surgery.

4.Subconjunctival Sparganosis.

PURPOSE: The subconjunctival sparganosis is rare tissue helminthiasis which developed frequently in abdominal, urethral, ureteral and vertebral cases. Authors report a case of sparganosis in the subconjunctival mass. METHODS: A 51-year-old man with reddish non painful mass in his left eye on the subconjunctiva that was unresponsive to medications for 6months was evaluated and treated surgically. On surgical exploration, a white thread-like plerocercoid 0.2 x 4.7 cm in size was extracted from the subconjunctival mass and actively motile in the bottle filled with saline. It was confirmed as a typical sparganum under histologic examination. Three months after surgery, the lesion disappeared completely. RESULTS: Although rare, parasitic disease should be suspected in a palpable subconjuntival mass unresponsive to the medical treatment.

5.Inflammatory Pseudotumor of the Extraorbital Head and Neck: CT and MR Imaging Findings.

PURPOSE: To determine the MR and CT imaging findings of inflammatory pseudotumor of the extraorbital head and neck. MATERIALS AND METHODS: We reviewed the MR (n=10) and CT (n=9) imaging studies of 11 patients with this condition (M:F=5:6, age range: 35-75 years), analysing each case in terms of location, occupying space, signal intensity, intracranial involvement, degree of contrast enhancement and adjacent bone change. Follow-up images were obtained in nine cases, and the response of each patient to steroid treatment was reviewed. RESULTS: Lesions involved the masticator space (n=8), the buccal space (n=6), the nasopharynx (n=5), the paranasal sinus (n=4), the parapharyngeal space (n=3), the prevertebral space (n=2), the orbit (n=2), the carotid space (n=2), the paravertebral space (n=1), parotid space (n=1), and the oral cavity (n=1). In ten of eleven cases, there was adjacent bone change. In three cases, the cavernous sinus was involved, and in two, the dura. One case involved both of them. At T2-weighted imaging, the lesions showed hypointensity in nine of ten cases; in four of nine, signal intensity was markedly low, and in no case was it diffusely high. In five of nine cases, the mass decreased in size after steroid therapy. CONCLUSION: Inflammatory pseudotumor showed iso-to hypointensity at T2-weighted imaging. Lymphadenopathy was not apparent.

6.A Case of Sporadic Nasopharyngeal Burkitt's Lymphoma.

Burkitt's lymphoma is a high grade non-Hodgkin's lymphoma of B-cell origin. There are three clinical subtypes, namely, endemic African, sporadic American and human immunodeficiency-associated subtype. It usually occurs in children or younger person. Sporadic American Burkitt lymphoma involves mainly the abdomen and rarely head and neck. Complex chemotherapy is the choice of treatment. We present one case of sporadic American Burkitt lymphoma that involves the nasopharynx manifested with frequent epistaxis and nasal obstruction. Clinical and histological features are reviewed.

7.Problems in the Treatment of Patients with Ankylosing Spondylitis.

The characteristic pathological lesions in ankylosing spondylitis are vertebral body osteoporosis, ankylosis of the apophyseal joints, intervertebral disc calcification, and ligamentous ossification. Calcification of the annulus fibrosis reduces the movement and elasticity of the intervertebral disc, causing this point to be the site of least resistance when the spine is subjected to trauma. The rigid spine may secondarily develop osteoporosis, further increasing the risk of spinal fracture. The ligamentous fragility and multiple fused vertebral segments cause the fractured ankylosing spondylitic spine to resemble a long-bone fracture. In ankylosing spondylitis patient who have sustained minor trauma, a complete radiological study of the entire spine and not just the symptomatic region must be performed for an accurate diagnosis, so that any fracture possible along the spine can be detected and thus prevent any medicolegal problems. The authors report 4 cases of ankylosing spondylitis including 2 cases with severe neurological abnormalities that had occurred after minor trauma.

8.Short Term Clinical Experiences of 52 Sorin Bicarbon Mechanical Valves.

From June 1995 to May 1997, we have implanted 52 Sorin Bicarbon mechanical valves in 41 patients. They were 16 men and 25 women, and their mean age was 47.4+/-14.8 (range; 18~74 y.o.). 35 (27 mm~31 mm) were in mitral position, 15 (19 mm~25 mm) in aortic position, and 2 (31 mm) in tricuspid position. 3 CABGs and a tumor excision were taken concomittantly. 35 patients were primary operation, and 6 were re-do operations. By intraoperative transesophageal doppler echocardiography, transvalvular peak/mean pressure gradient was 6.1+/-2.7/2.4+/-1.4 mmHg in mitral position and 27.6/10.7 mmHg in aortic position. The effective valve opening area in mitral position was 3.2+/-0.6 cm2. Follow-up was total 508.6 patient-months, and mean follw-up was 12.7+/-9.2 months. NYHA class was improved from 2.6+/-0.6 to 1.2+/-0.3 in average postoperatively. During that period, there was no operative death. 2 late non-valve related deaths were occurred. One was died of COPD, and the other was possible acute myocardial infarction. Among 7 postoperative complications, one valve related complication (minimal paravalvular leakage) was noticed. In conclusion, Sorin Bicarbon mechanical valve is believed one of the safe choice in clinical settings. It showed excellent hemodynamic and mechanical functions, and very low postoperative valve related complications in short term clinical experience.

9.Initial Transient Neurologic Recovery Followed by Delayed Deterioration of Osmotic Demyelination Syndrome: A Case Report.

In literatures, most of the studies of severe hyponatremia during or following its treatment has been concentrated with special references to the rate of correction and its neurologic outcomes. But, there is relatively few ones analyzing the diverse clinical manifestations of neurologic symptorns or complications during the course of treating severe hyponatremia. We experienced a catastrophic course related to hyponatremia in a 51 year woman with severe rheumatoid arthritis, who underwent knee joint replacement, and this case revealed the initial transient neurologic recovery for 3 days by the initial rapid correction of hyponatremia, then followed by delayed deterioration of osmotic demyelination syndrome leading to locked-in syndrome. Reported cases with similar clinical course (biphasic course) in the world lituratures were reviewed with special interests in the initial maximum rate of correction of hyponatremia and radiologic findings. This review suggests that clinicians treating the patients with severe symptomatic hyponatremia should be aware of the possibility of delayed neurologic sequelae despite the recovery of neurologic status as well as the degree of hyponatremia in the early treatment course of hyponatremia.

10.Transoral Adenoidectomy with the Microdebrider under Transnasal Endoscopy.

BACKGROUND AND OBJECTIVES: Although there have been various methods to remove adenoid, it is still difficult to achieve complete adenoidectomy due to its unfavorable anatomic structure. The aim of this study was to introduce transoral adenoidectomy using a microdebrider under transnasal endoscopy and to evaluate its efficacy. MATERIALS AND METHOD: The subjects consisted of 80 patients with adenoid vegetation. Among them, 40 patients underwent adenoidectomy using the microdebrider and other 40 cases were performed using the adenoid curette via transoral approach. Preoperative and postoperative lateral skull radiographs were obtained and compared. Also, the amount of intraoperative bleeding, complications, postoperative symptom score, and the duration of operation were compared. RESULTS: There were no statistical differences in the intraoperative bleeding, subjective symptoms after surgery and postoperative complications. But, patients, who had been performed adenoidectomy using the transoral debrider under transnasal endoscopy, had better outcomes in the nasopharyngeal ratio and the shortest nasopharyngeal diameter in lateral skull radiographs. CONCLUSION: Transoral adenoidectomy using the microdebrider under transnasal endoscopy is an effective method for more complete adenoidectomy.
