2.Retrospective analysis on surgical site infection in patients in department of neurosurgery

Objective To explore characteristics and risk factors for surgical site infection(SSI)in patients in the department of neurosurgery,so as to provide theoretical basis for prevention and control of SSI.Methods Clinical data of patients who were admitted to a department of neurosurgery from January to December 2015 were collected with retrospective survey method,SSI and risk factors in patients were analyzed.Results Among 715 patients un-dergoing neurosurgery,40(5.59% )had SSI. SSI mainly occurred in patients following cerebral vascular surgery, accounting for 7 .69% ,followed by patients following intracranial tumor surgery(5 .94% ). 40 patients were all with organ space/intracranial infection. Difference in SSI in patients with different types of operation,duration of opera-tion,length of hospital stay,and National Nosocomial Infections Surveillance (NNIS)scores were all significant(all P<0.05).Conclusion Incidence of SSI in patients in the department of neurosurgery are related with operation type,duration of operation,length of hospital stay,and operation risk index,preventive measures should be taken to reduce the incidence of SSI.
4.Stereotactic radiotherapy for trigeminal neuralgia

BACKGROUND: There are several ways for treating the trigeminal neuralgia, such as medication, block therapy and surgery, etc. But few reports throughout the world described stereotactic radiotherapy(SRT) for it, which is efficient, non-invasive and painless.OBJECTIVE: To investigate the improvement of SRT for trigeminal neuralgia.DESIGN: Case analysis based on patients.SETTING: X-knife Treatment Center, First Affiliated Hospital, Henan University of Science and Technology.PARTICIPANTS: Totally 30 patients with trigeminal neuralgia were selected from X-knife Treatment Center, the First Affiliated Hospital of Henan Science and Technology University between March 1999 and March 2002. All patients were diagnosed with CT or MRI, and no other diseases were found. Patients without obvious effect from a long-term medicine treatment or surgery were selected as the investigative subjects.METHODS: Thirty patients with trigeminal neuralgia wearing face cover were treated with non-coplanarity fractional stereotactic radiotherapy. The patients received 2 target regions and peripheral does of 60 - 75 Gy. 5 mm collimator and isodose curves referred to ranged from 85% - 90%.MAIN OUTCOME MEASURES: Results of therapeutic assessment and follow up.RESULTS: The follow up period ranged from 1 to 22 months. Among 30patients, 6 patients' symptoms were lightened or eased up over 12-24hours, 20 patients received a marked effect over 1 - 3 months, 4 patients'treatment effects were not obvious. The effect rate reached 87%. A patient occurred numbness in sick face, and a patient showed no obvious effect in the first treatment, but the efficacy became evident in the second treatment.CONCLUSION: It is efficient in the treatment of trigeminal neuralgia with stereotaetic radiotherapy and there is no special side effect.

Duchenne muscular dystrophy is the most prevalent muscular dystrophy caused by dystrophin gene mutations. In the central nervous systerm, there are at least six dystrophin isoforms, in which Dp140 is closely associated with cognitive impairment. This paper re-viewed the discovery, structure, gene location, detection methods and function of Dp140.
7.Studies on constituents from pericarps of Juglans mandshurica with anti-tumor activity

Objective To study the chemical components from the pericarps of Juglans mandshurica with anti-tumor activity.Methods It showed that pericarps of J.mandshurica had a good performance in treating cancer and low toxicity from CHCl3 and EtOAc portions by a lot of pharmacological experiments and modern clinical application in the past.Compounds were isolated by chromatography on silica gel columns and AB-8 macroporous resin.On the basis of spectral evidences including 1D-NMR and 2D-NMR,the structures of these compounds were determined.Results These compounds from CHCl3 portion were identified as 20(S)-protopanaxadiol-3-one(1),dammara-20,24-dien-3?-ol(2),galeon(3),juglanin A(4),2?,3?,23-trihydroxy olean-12-en-28-oic acid(5),2?,3?,23-trihydroxy urs-12-en-28-oic acid(6),oleanolic acid(7),ursolic acid(8).Conclusion Compound 1 is a new natural product.Compounds 2 and 5-8 are isolated firstly from the plants of Juglans L.which are due to pentacyclic triterpane.Compound 3 is isolated from the pericarps of J.mandshurica for the first time.
8.The application of problem-based learning with computer aided technology in teaching of breast surgery clinical practice

Objective To probe the teaching method of application of problem-based learning (PBL)with computer aided technology in teaching of breast surgery clinical practice.Methods 48 students were randomly divided into two groups:control group(n=24) were taught by lecture-based learning(LBL),experimental group (n=24) were taught by method of application of problem-based learning(PBL)with computer aided technology.Questionnaire was provided to the students of experimental group to investigate the degree of recognition to the teaching method of problem-based learning (PBL)with computer aided technology.5 patients from experimental group and 5 patients from control group participating in teaching were given questionnaires to investigate if their right of privacy were violated.The theory examination score(out of 50) and comprehensive skills assessment score (out of 50) were compared between the two groups; the mean differences between the two groups were compared by t test.Result The teaching method of application of problem-based learning(PBL) with computer aided technology was recognition by 91.7 %(22 persons) of students.20 percent(one person) of the experimental group patients felt their right of privacy violated; 60 percent(three persons) of the control group patients felt their right of privacy violated.The theory test scores of the experimental group and control group were (46.23 ± 2.45) and (45.52 ± 3.29).There was no statistical significance (t=1.230,P=0.225).The skill test scores of the experimental group and control group were(47.23± 2.45) and (36.52 t 3.29).There was statistical significance(t=12.791,P=0.000).Conclusion The teaching method of application of problem-based learning(PBL) with computer aided technology can help encourage the study interest of students,and cultivate their clinical thinking method and capacity,and protect the right of privacy of the patients,avoiding patient-hospital disputes.
9.Cognition of Patients with Duchenne Muscular Dystrophy

Objective To investigate the cognitive condition in patients with Duchenne muscular dystrophy (DMD). Methods Wechsler Intelligence Scale for Children IV (WISC-IV) was administered for 127 DMD patients aged 6~14 years from April, 2013 to December, 2015. The verbal intelligence quotient (VIQ), performance intelligence quotient (PIQ) and full-scale intelligence quotient (FSIQ) scores were analyzed. Results The FSIQ, VIQ and PIQ were (93.2±18.6), (91.5±18.9), and (96.6±19.5), respectively, which were lower than 100 in normal population (P<0.001), and higher than that of patients abroad in the previous studies (P<0.001). There were 13 patients (10.24%) whose scores of FSIQ were less than 70. The VIQ-PIQ discrepancy score was (-5.0 ± 16.6). Conclusion About 1/10 of Chinese DMD pa-tients impaire in cognitive function, which is significantly lower than that abroad. VIQ is more affected than PIQ.
10.Oxidative stress effect on viability of umbilical cord-derived mesenchymal stem cells in storage solution of transplantation

BACKGROUND:The viability of human umbilical cord-derived mesenchymal stem cel s is often declined with the commonly used transplantation storage solution in clinics, which may influence the therapeutic effects of cel ular transplantation. However, reasons for this are stil unknown. OBJECTIVE:To investigate the role of oxidative stress in the reduction of human umbilical cord-derived mesenchymal stem cel s viability in the storage process during clinical transplantation and to observe the effects of radical scavenger on the results. METHODS:Human umbilical cord-derived mesenchymal stem cel s were harvested and cultured in normal saline for 0, 2, 4 and 6 hours at room temperature. Intracel ular reactive oxygen levels were detected at those time points. Antioxidant enzyme activities and levels of malondialdehyde were measured to determine the intracel ular oxidative stress levels after storage. Cel adhesion rate changes were retested after adding N-acetyl cysteine to the storage solution. RESULTS AND CONCLUSION:The reactive oxygen levels in human umbilical cord-derived mesenchymal stem cel s were increased significantly after normal saline storage and levels of malondialdehyde were increased in a time-dependent manner. Activities of superoxide dismutase, catalase and glutathione peroxidase were al reduced. Addition of N-acetyl cysteine into the storage medium decreased the reactive oxygen levels and improved the human umbilical cord-derived mesenchymal stem cel s viabilities. Experimental findings indicate that, increased reactive oxygen species in human umbilical cord-derived mesenchymal stem cel s is one of the reasons for reduced cel viability. Adding the radical scavenger N-acetyl cysteine can improve the storage effects of human umbilical cord-derived mesenchymal stem cel s.
