2.Rate of Pedicle Disruption after Screw Fixation

Postoperative CT scans for accuracy of pedicular screw placement were assessed in 20 patients (82 pedicular screws) treated with several kinds of pedicular screws. When putting screws, the position of the guide pin was confirmed by the image intensifier. The measurement of canal encroachment from the medial border of the pedicle and the angle of insertion through the body of each vertebra were assessed; 1. Fifty-five(69.1%) were placed within the pedicle. 2. Ten(12.2%) were placed within 2mm of the medial border of the pedicle. 3. Three(3.6%) had 2mm to 6mm canal encroachment with the patient developing neurologic complication that was improved 8 months after the operation. 4.Fourteen(17.1%) perforated the lateral border of the pedicle, but they were not associated with neurologic complication. 5. The screws placed within 2mm of the medial border of the pedicle had higher angle of insertion than the screws exactly within the pedicle. Even with the use of image intensifier, there was a high rate of pedicle disruption. We have to insert the screws more exactly according to true anatomic angle of the pedicle on each level of every vertebra from the entry point avoiding high angle of insertion to prevent any disruption of the medial border of the pedicle which may result in neurologic complication.

3.Expression of bcl-2 and p53 Protein in Invasive Cervical Cancer.

Recently, the bcl-2 and p53 protein have been recognized as important factors that is contributed to programmed cell death. The objective of this study was to evaluate the prognostic significance of bcl-2 and p53 protein expression in uterine cervical carcinoma. The expression of bcl-2 and p53 in 59 cases of uterine cervical carcinoma (stage IB to IIB) were surgically treated from January 1993 to June 1994. The expression of bcl-2 and p53 was examined by immunohistochemical method using formalin fixed paraffin embedded tissue specimens. The 48 cases were squamous cell carcinoma and 11 cases were adenocarcinoma. The results were as follows: 1. The expression rate of bcl-2 protein was 28.8%(17/59) and there was no significant correlaltion between the expression of bcl-2 protein and the clinicopathologic parameters (histologic type, grade, FIGO stage, cervical invasion depth, lymph node metastasis, parametrial invasion, tumor size, neoadjuvant chemotherapy response, recurrence, survival). 2. The expression rate of p53 protein was 32.2%(19/59) and there was no significant correlation between expression of p53 protein and the clinicopathologic parameters. 3. There was significant correlation between and expression of bcl-2 and p53 protein (P 0.05). In conclusion, bcl-2 and p53 protein are thought to be possible factors in the carcinogenesis of uterine cervical carcinoma and correlate with progression of it. But further study will be required to clarify the role of bcl-2 and p53 in carcinogenesis of the uterine cervix.

5.Neurologic Complication after Spine Surgery

We examined the rates of postoperative neurologic complications, as recorded in the medical record department in Severance Hospital from 1989 through 1991 for patients who had had operations on the spine. There were 538 hospitalizations for procedures on the spine; 327(60.78%) of herniated disc, 120(22.30%) of spondylolisthesis, 64(11.90%) of infection, 27(5.02%) of spinal stenosis were included. Neurologic complications oecurred in 12(2.23%) patients; 3 cases in herniated disc, 4 in spinal stenosis, 4 in spondylolisthesis, and 1 in infection. Regarding techniques of the operation, 8 complications occurred in arthrodesis, among these, 2 were in anterior approach and 6 in posterior approach. Four cases of the complications occurred in laminectomy and discectomy procedure. Complications were more frequent in the patients who had had spinal stenosis when arthrodesis was performed than decompression procedure only. Possible causes of neurologic complication were postoperative hematoma formation, translation of the vertebral column due to instability after anterior debridement and forceful traction of the nerve root during the operation.

6.The Dopamine D2 Receptor Locus as a Modifying Gene in Korean Schizophrenia, Alcoholism and Drug Addiction.

The authors attempted to examine the allelic association between the A1 allele of Dopamine D2 receptor and schizophrenia, alcoholism, drug addiction in Koreans. Schizophrenic patients(n=31), alcoholism(n=65), drug addiction(n=18) and controls(n=52) were examined by case-control study for distribution of the Taql polymorphism of the dopamine D2 receptor gene in Korean population to minimize the effect of racial differencies in gene frequencies. In schizophrenics, the numbers of schizophrenics with A1A1, A1A2, A2A2 were 9(29%), 15(48%) and 7(22%) respectively and in alcoholics with A1A1, A1A2, A2A2 were 14(21.5%), 36(55.4%) and 15(23.1%) respectively and in drug addiction with A1A1, A1A2, A2A2 were 4(7.6%), 24(46.2%) and 24(46.2%) respectively. The prevalence of the A1 allele in schizophrenics, alcoholics, drug addiction and controls were 77%, 76.9%, 67% and 53.8% respectively. And the frequency of the A1 allele in schizophrenics, alcoholics, drug addiction, and controls were 0.53 0.49, 0.39 and 0.31 respectively. There was significant difference in the frequency of the A1 allele between schizophrenics, alcoholics and controls. We also classified our alcoholic population. For classification by severity, we used the median MAST score 30 in our samples. There was also significant difference in the frequency of the A1 allele between less severe group(0.42) and more severe group(0.57). This data suggest that the A1 allele is associated with schizophrenia and alcoholism in Korean. Furthermore the prevalence of the A1 allele increased in more severely affected alcoholics. The authors conclude that our data support an allelic association between the A1 allele at dopamine D2 receptor and schizophrenia, alcoholism. These results suggest the A1 allele of the DRD2 gene is associated with a number of behavior disorders in which it may act as a modifying gene rather than as the primary etiological agent.

7.Effect of sodium hyaluronate on prevention of osteoarthritis.

The changes in the surface of articular cartilage of femoral condyle from rabbits were evaluated after degenerative changes were made by the technique advocated by Hulth. The medial collateral and both cruciate ligaments were excised, and a medial menisectomy was done. Then the right knee joint was injected with 1 ml of Na-hyaluronate gel every two weeks. The animals were sacrificed at two, four, six, eight, or sixteen weeks postoperatively. After sacrifice, the medial femoral condyle was excised and prepared for the light microscopic and scanning electron microscopic study. At eight to sixteen weeks, there were chondrocyte clones with clefts to the radial zone and increased loss of the height of articular cartilage on the control side; but, on theexperimental side there was a significant delay and lessening of the arthriticresponse. The biocompatibility and the protective effect of joint degeneration of this device make this material a valuable adjuvant in the treatment ofosteoarthritis and the traumatized joints.

8.Atypical manifestation of solid and papillary epithelial neoplasm of the pancrease: case report.

We report a rescently experienced case with atypical radiologic manifestation of solid and papillary epithelial neoplasm of the pancreas in a 37 years old female patient. The tumor had heavy calcification on its capsule wall and septa. Instead of the usual encapsulated lesion with partly solid and partly cystic-hemorrhagic components, the lesion consisted of numerous fine cavitations containing air without fluid component except focal abscess fluid.

10.Study on a Method to Improve Vital Statistics and Family Planning Simultaneous1y among Maternity Cases.

To demonstrate and assess the possibility of improving simultaneously vital statistics and family planning in urban and rural Korea, a action-cum-research project was conducted for a period of one year, from May 1966 to April 1967, over the area covering Kimchun city and a part of surrounding Kumnung Gun, Kyongsangpuk-do province, Korea. As the method of study, a sterilized simple delivery kit was distributed to the expectant mothers in the experimental areas, and its effects on the reporting of vital events such as births and deaths and the acceptance of family planning were compared with the same in the control areas without such an action program. A single delivery kit contained one vinyl sheet for covering and one for lying on, one cotton sheet, a pair of scissors, one foot of thread, two pieces of gauze, a piece of soap, a weight scale and one doubled envelope. From the study, the system of distributing simple hygiene delivery kits to all expectant mothers in an area is an effective method of obtaining accurate statistics of births. It is also effective in the reduction of infant mortality, which, in turn, promotes acceptance of family planning in rural Korea.
