1.Expression of CD44 Splice Variants(v4/5 and v6), alpha-Smooth Muscle Actin, and nm23 Proteins in IB-IIB Uterine Cervical Cancer.

We examined the expressions of CD44 splice variants (v4/5, v6), alpha-smooth muscle actin, nm23 to evaluate their roles as prognostic factors in 70 cases of uterine cervical carcinoma (stage IB to IIB) who were surgically treated from January 1989 to June 1990 with a clinical follow-up of a minimum of 5 years. The expression was examined by an immunohistochemical method using archival formalin fixed paraffin embedded tissue. In the 70 cases, 61 cases were squamous cell carcinoma and 9 cases were adenocarcinoma. CD44v4/5, CD44v6, alpha-smooth muscle actin, and nm23 were detected in 41.4%, 70%, 100%, and 74.3% of tumor samples, respectively. CD44 splice variants and nm23 showed membrane and cytoplasmic staining of tumor cells, respectively. The expression of alpha-smooth muscle actin showed cytoplasmic staining confined to stromal cells and was classified into three grades by the extent in stromal cells: with less than 10% of stromal cells; 32.9%, 10-50% of stromal cells; 40.0%, more than 50%; 27.1%. These expressions were not correlated with histologic types, lymph node involvement, recurrence, and grades of tumor infiltrating lymphocyte (TIL). But CD44v4/5 had significantly inverse correlation with TIL (p=0.049). The expression of CD44v4/5 was significantly correlated with that of CD44v6 (p=0.05), and that of alpha-smooth muscle actin was inversely correlated with that of nm23 (p=0.049). In conclusion, in FIGO IB-IIB uterine cervical carcinoma CD44 variants, nm23, and SMA show high prevalence, however, with little prognostic significance assessed by recurrence and lymph node metastasis.

Fracture of the femoral neck occurred after internal fixation of intertrochanteric fracture of the femur is very rare and have been described previously in terms of stress fracture, stress-riser fracture, Youngs modulus fracture or iatrogenic fracture in the literature. This fracture documented about 20 cases in the English literature and usually occurred in elderly patients with osteoporosis and it always occur in the subcapital region. We report a case of subcapital stress fracture of the femur occurred after internal fixation with compression hip screw of intertrochanteric femur fracture.

3.Surgical Management of Cervical Spondylotic Myelopathy.

Patients admitted with the clinical symptomatology of a progressive myelopathy associated with radiologic findings compatible with spondylotic degeneration of the cervical spine and who manifest appropriate neurophysiological abnormalities should be considered as candidates for surgical treatment. The authors have undertaken a study of 20 patients who had received decompressive surgical procedures and possible follow up studies for CSM, which was defined as a myelopathy related to osteophytic overgrowth and ossification of posterior longitudinal ligament(OPLL) in the cervical spinal canal causing impingement upon the spinal cord from Sep.93. To Aug. 1995. It was concluded with the following results. 1) man was exclusively prevalent in most cases. 2) age at surgery: There was a signigicant negative correlation between the age at surgery and the recovery rate. 3) Original cause of compression: There was no signigicant difference between the CSM and OPLL. 4) The involved level : three level involvement was the most common(8cases), followed by two level(6 cases), four level(3 cases), and single level(2 cases). There was no correlation between the preoperative Harsh scale and the number of the involved level. 5) Recovery rate: excellent(Grade O.I). 12 cases(60%), good(Grade II, IIIA), 4 cases(20%), poor(Grade IIIc, IV, V), 4 cases(20%).

7.Augmentation of the Chin with a Silicone Implant.

A line is drawn from the nasal tip to the most anterior point of the lower lip. The distance from soft tissue pogonion to the ideal soft tissue pogonion is the amount of correction. Alloplastic augmentation does not correct the soft tissue in a 1:1 ratio. In the case of a patient having normal soft tissue(8~11 mm), the ratio of the soft tissue correction is 1:0.66. In the case of a patient having a soft tissue deficiency(less than 7mm), the ratio of the soft tissue correction is 1:0.8. In the case of a patient having a soft tissue excess(more than 12mm), the ratio of the soft tissue correction is 1:0.5. The length of the implant is required to be more lateral to the mental foramen by 1~1.5 cm for restoration of the prejowl sulcus. The posterior surface of the implant must be carved to shape precisely to the bony surface. I usually make several vertical etchings and 20~30 holes in the implant. The vertical etchings help expand the implant to securely fit the mandibular contour. Fenestrated silastic implants can be further stabilized with fibrous tissue ingrowth and future reconstruction if bony erosion occurs.

8.Total Facelift: Forehead Lift, Midface Lift, and Neck Lift.

Patients with thick skin mainly exhibit the aging processes of sagging, whereas patients with thin skin develop wrinkles or volume loss. Asian skin is usually thicker than that of Westerners; and thus, the sagging of skin due to aging, rather than wrinkling, is the chief problem to be addressed in Asians. Asian skin is also relatively large in area and thick, implying that the weight of tissue to be lifted is considerably heavier. These factors account for the difficulties in performing a facelift in Asians. Facelifts can be divided into forehead lift, midface lift, and lower face lift. These can be performed individually or with 2-3 procedures combined.

9.Analysis of DNA Ploidy Pattern of Hepatocellular Carcinoma with Comparison between Flow Cytometric and Image Cytometric Study.

Fifty cases of hepatocellularcarcinoma were studied using retrospective flow cytometric(FCM) and image cytometric(ICM) DNA analysis to determine the prevalence of aneuploid cell population and whether they were associated with any particular clinico-pathologic findings. At the same time, we compared the difference between FCM and ICM. The materials were prepared from 50 micrometer cut of paraffin embedded blocks. The DNA modal values, which could be defined in 42 cases by FCM with 74% aneuploidy and in 50 cases by ICM with 76% aneuploidy. So 95% of the cases had concordant DNA ploidy results by both techniques of FCM and ICM. Abnormal DNA pattern was correlated with age(<50), presence of cirrhosis, pathologic grade and some pathologic types(p<0.05), but was not correlated with presence of HBsAg, sex, alphafeto protein, and alcohol history(p>0.05). Also we found that ICM technique was easier to perform and interprete.

10.Effect of Hyperbaric Oxygen & Allopurinol on the Survival of Irradiated Rat Skin Flap.

The objectives of this study sere to investigate how the local irradiation affect the survival of random pattern skin flap, and whether or not hyperbaric oxygen and allopurional can improve the survival of the irradiated rat skin flap. There have been many reports about the effects of hyperbaric oxygen and allpurinol on skin flaps. However, very few reports have been presented on the effects of hyperbaric oxygen and allpurinol in treating irradiated skin flaps. The author examined the local irradiation effect on rat skin flap survival after irradiation of 20 Gy, 3 days postoperatively, on cranially based random pattern dorsal skin flap, which was 3 x 9cm in size. The flap survival length was measured in experimental groups treated with hyperbaric oxygen (2.5 atm absolute, 100% oxygen, once a day for 7days) after irradiation and with hyperbaric oxygen combined with allopurinol (100mg/kg, once a day for 7 days), in comparison with a radiation-only group. On reviewing the flap survival length 10 days postoperati-vely, the average flap survival length in the radiation-only group was 2.2+/-0.5cm, while in the non-radiation group it was 5.5+/-0.3cm. The reduction ratio of flap survival by irradiation was 60%. There was a significant increase in the mean flap survival length in the groups treated with hyperbaric oxygen (4.0+/-1.6cm) and hyperbaric oxygen combined with allopurinol (5.5+/-1.8cm). The increased ratio of flap survival in each group was 85% and 150% when compared to the radiation-only group. The author found that rat skin flap survival decreased, even at an early stage, as a result of high dose local irradiation and that decreased flap survival by irradiation could be restored by hyperbaric oxygen and allopurinol. The group treated with hyperbaric oxygen combined with allopurinol showed increased flap survival over the group treated with hyperbaric oxygen only. The results showed a method which could possibly increase flap survival in cancer patients who require early radiation after flap surgery.
