The pedicled fillet flap concept has been successfully applied in both the upper and lower extremities for the treatment of difficult wound. In cases of inevitable extremity amputation, the transfer of pedicled flap from the amputed part is possible. In such substance, it is possible to obtain the pedicled fillet flap from the amputed limb to provide stump coverage. We experienced two cases of below-knee amputation due to severe flame burn which the stump was covered with island pedicled sole-fillet flap.

It is indispensable to cover the skin defect when bone or tendon is exposed. In case of inevitable amputation of an extremity, it is possible to harvest a free flap from the amputed limb for providing coverage of the other wound. This technique allows immediate wound coverage without the morbidity of an additional donor site. We experienced an electrical burn case with inevitahle wrist disarticulation and successfully treated soft tissue defect of ankle using free vascularized flap.

9.A Role of Routine Lumbar Puncture in Children Presented with Their First Seizure with Fever.

PURPOSE: This study was performed to find out the role of routine lumbar puncture in children presented with their first seizure with Fever. METHODS: This study included 220 children, over a 5 year period, from April 1999 to March 2003, who visited or were admitted at Dae Dong Hospital with their first febrile convulsion. Lumbar puncture was performed in all children. We analyzed their age, family history, type of seizure, duration of seizure, cause of fever, and the results of lumbar puncture. RESULTS: In the sex distribution, males(58.6%) outnumbered females(41.4%) and the ratio was 1.2:1. 81.7% of the patients had febrile convulsion from 6 months- to 3 years of age(P<0.05). 30.5% also had family history of febrile convulsion. The types of seizure were generalized tonic-clonic(72.7%), generalized tonic(17.7%), and generalized clonic(6.4 %). For the duration of seizure, 90.4% of the patients were estimated less than 15 minutes(P<0.05). The causes of fever were pharyngotonsilitis(40.5%), gastroenteritis(19.1 %), pneumonia or bronchitis(13.2%), meningitis(11%), otitis media, urinary tract infection, and exanthem subitum. However, 9.1% of the patients were diagnosed meningitis, with 18 of 20 patients under 3 years of age. CONCLUSION: In the first seizure with fever, lumbar puncture is a useful method for meningitis, especially under 3 years of age.

10.Analysis of Papers in Radiological Journals in Recent Years: A Comparison of Journal of Korean Radiologic Society and Radiology.

PURPOSE: The purpose of this study was to determine current trends and the mode of future development in the field of medical radiology and to promote research among the nation's radiologists by analyzing the contents of the Korean Radiologic Society(JKRS) and Radiology. MATERIALS AND METHODS: The number of articles published in JKRS each year between 1990 and 1994 was counted. The research articles in JKRS(n=740) and in Radiology(n=1748) between 1992 and 1994 were categorized according to the objective, type, topic, materials, and radiologic techniques of their contents on the basis of predetermined criteria. Domestic Masters theses(n=126) and doctoral dissertations(n=75) accepted between 1990 and 1994, and domestic materials published in international journals(n=416) between 1986 and 1994 were also categorized using the same criteria. RESULTS: The greatest increase in the number of articles published in JKRS was seen during 1994. The majority of these aimed to retrospectively analyse the findings of diseases while the majority of articles published in Radiology dealt prospectively with the development and/or evaluation of diagnostic methods. More variety of topics and issues was seen in Radiology than in JKRS. The number of articles of domestic materials published in international journals increased from1986 to 1994, while the number of articles of foreign materials published in korea was relatively stationary. A significant number of theses and dissertations dealt, mostly prospectively, with studies of pathophysiologic and/or pharmacologic mechanisms using animal models. CONCLUSION: In order to understand both current trends and the direction and mode of future developments in the field of radiology, and to be able to actively deal with challenges at the forefront of radiologic development, it is essential to review research articles published in radiology-related journals.
