3.Reconstruction of the soft tissue deffect of lower extremity in complicated case.

This study includes 19 cases of complicated low extremity injury to whom underwent microvascular free-tissue transfer in cases of open comminuted fracture, osteomyelitis, and vascular compromise or extensive soft tissue defect from Nov. 1994 to July 1997. The average time after injury to coverage was 25 days. The nineteen patients underwent a total of nineteen free tissue transfers primarily, and eighteen (94.7 per cent) of the transfers survived. Major complications were encountered recurrent failure of the free tissue transfer in one patient and partial necrosis in the other flap, which were successfully treated by flow-through radial forearm free flap and distally based superficial sural artery flap respectively. Two patients were sustained below knee amputation, one above knee amputation, despite of survival of flaps. This reveals limb salvage in 84 percent. The microvascular reconstruction is a versatile and reliable methods in complicated cases after severe trauma. Free tissue transplantation should be considered as a primary treatment in such complicated cases.

5.Operative Errors in Interlocking Intramedullary Nailing for Tibial Fractures

From retrospective review of 111 tibial fractures treated by interlocking intramedullary nailing, 38 operative errors in 28 cases(25%) were included in this study. The errors were classified into two categories. The first category included eighteen errors that related to fracture reduction; nine angular malalignments, four failures in closed nailing to open the fracture site, two peroneal nerve palsies, two shortenings of tibial length(more than 1cm), and one rotational malalignment. The second category comprised twenty errors that related to the nailing itself; eight protrusions of proximal nail tip, five overdistractions of fracture gap (more than 3mm), four additional fragmentations, two mistakes in interlocking screw insertion, and one ankle joint injury by the distal nail tip. Functional results of those 28 cases at the last follow-up(average, 17.5 months; range, 9 to 29 months) were excellent or good in only 60.8 per cent, while the control group treated by adequate operative technique showed excellent or good results in 92.8 per cent.

8.Hemodynamic Effect of Prazosin in Congestive Heart Failure.

The hemodynamic effects of Prazosin were evaluated in 31 cases with congestive heart failure (mitral stenosis; 15 cases, mitral insufficiency; 7 cases, hypertensive heart disease;4 cases, congestive cardiomyopathy; 5 cases) by means of echocardiographic method. Before and 90 min. after the administration of prazosin(3mg p.o.), M-mode echocardiograms of left ventricle were recorded, from which end-diastolic and end-systolic dimensions of left ventricle and left ventricular ejection time were measured. Hemodynamic parameters of left ventricular performance, i.e., mean veloity of circumferential fiber shortening, fractional fiber shortening, ejection fraction, stroke volume, and cardiac output were calculated from the measurements. Left ventricular end-diastolic and end-systolic dimensions were decreased significantly after prazosin administration and left ventricular performance was significantly increased after prazosin administration, whereas stroke volume was not. Total systemic peripheral resistance and mean arterial pressure showed no significant changes. All cases, except one of the patients showed marked symptomatic relief after prazosin administration evaluated by NYHA functional classification. There was no considerable untoward effect in the patients of congestive heart failure to whom the prazosin was administered.
