1.Analysis on Rational Use of Cephalosporins in 583 Cases of a Large General Hospital

OBJECTIVE To study the rational use of antibiotics in a comprehensive general hospital.METHODS The clinical data of 583 cases used the five kinds of most frequently antibiotics during Nov 20-26,2006 in a comprehensive general hospital were collected and analyzed.RESULTS The total five kinds of most frequently antibiotics used in this hospital were the third generation of cephalosporins,cefoperazone/tazobactam(Kaisi),flomoxef(Fuomaning),cefoperazone/tazobactam(Kaishute),cefpiramide(Xibaiao) and cefoperazone/tazobactam(Xianbita).The daily frequency of these antibiotics was rational to the drug metabolic dynamics characteristics.Some patients received the long time therapy of antibiotic,the longest one was 22 days.The frequency of over-doses in these patients was 19.9%.CONCLUSIONS Antibiotics use is apparently rational in this hospital and the training and education for drug use should be enhanced in the future.
2.Congenital Syphilis: Hematologic Findings of Early Congenital Syphilis.

A retrospective study was made of the previous medical records of 47 infants with congenital syphilis born from mothers with untreated syphilis at Severance Hospital, Yonsei University College of Medicine during the years 1972 through 1983 The VDRL titer of these infants was higher than 1: 8 with reactive FT.A-ABS test. They were subjectd to hematologic examinations and the results were as follow s, 1. Of 46 infants with congenital syphilis, anemia was observed in thirty-one (68%) Reticulocytosis occurred in seventeen(89%) of nineteen infants with congenital syphilis having the mean value of 6. 0%. 3, Of 4.7 infants with congenital syphilis, leucocytosis was noted in seveenteen 4, Thrombocytopenia was observed in twenty (64%) of thirty-one infants with congenital syphilis.

3.Risk Assessment of Combined Detection of Lipoprotein Associated Phospholipase A2 and Cystatin C for Lupus Nephritis in Patients with Systemic Lupus Erythematosus

Objective To investigate the value of combinedevaluation of serum lipoprotein associated phospholipase A2 (LP-PLA2) and cystatin C (CysC) on systemic lupus erythematosus (SLE) patients with lupus erythematosus.Methods A total of 177 patients with SLE were enrolled in the General Hospital of People's Liberation Army from June 2000 to February 2017,87 cases in lupus group (SLE group),90 cases in lupus nephritis group (LN group),60 cases in healthy control group.The concentration of LP-PLA2 and CysC were measured.The ROC curves was established for estimation of the cutoff values of two indexes in SLE patients with LN,and the diagnostic efficacy of different diagnostic criteria was compared.Results The LP-PLA2 and CysC concentration in the LN group were higher than those in the SLE group (Z=-2.512,-5.688,all P<0.05).The cutoff value of LP-PLA2 was 245.5 U/L and the cutoff value of CysC was 1.235 mg/L on the risk assessment of in SLE patients with LN.When using the parallel method for combined LP-PLA2 and CysC to evaluate the risk of LN,the sensitivity and negative predictive value would be significantly improved (x2 =9.461,4.377,all P< 0.05),the efficiency was better.Conclusion Parallel assessment of serum LP-PLA2 and CysC have clinical value on assessment of the risk of LN in SLE patients.

5.Two Case Reports of Bone-invading Basal Cell Carcinoma.

Basal cell carcinoma is the most common skin cancer, especially on the eyelid and nose. As it rarely invades to a underlying bone or metastasizes an distant site, and is usually found at an early stage, it is regarded as a curative disease. However, basal cell carcinoma on the eyelid and nose may be resected incompletely due to efforts to preserve important structures or as a result of esthetic considerations. We experienced two cases of basal cell carcinoma with local invasion to underlying bone. One was a recurred case on the nasal area extending to the nasal bone arts medial wall of the ethmoid sinus. The lesion was widely resected and covered with a radial forearm free flap. The other was on the eyelid extending to the orbit. It was treated with orbital exenteration and resection of the involved eyelid. The defect was reconstructed with the temporalis muscle flap with split-thickness skin graft. These patients were followed up for 7 months with no evidence of recurrence. Since basal cell carcinoma can invade to the bone and metastasize to a distance site, it should be resected radically in the regions of the eyelid and nose.

7.MR Imaging Findings of Spinal Subarachnoid Hemorrhage: A Case Report.

We report magnetic resonance imaging findings of massive spinal subarachnoid hemorrhage (SAH) caused by repeated lumbar punctures during spinal anesthesia in a 36-year-old man. The signal intensities of spinal SAH were similar to those of the conus medullaris on both T1 and T2- weighted spin-echo images. Although spinal SAH is hardly recognized on MR, spinal SAH of sufficient amount may cause alteration of the cerebrospinal fluid signal.

9.Non-Convulsive Status with Myoclonic-Astatic Epilepsy: A Case Repot.

Myoclonic-astatic epilepsy is an epileptic syndrome characterized bv the presence of myoclonic and astatic seizures. It is a rare generalized epilepsy of childhood and represents 1 to 2% of epilepsis in children up to the age of 9 years. In addition to myoclonic and astatic seizures, generalized tonic-clonic seizure, tonic seizure and absence status (non-convulsive status) can be combined. Non-convulsive status can produce apathetic and stuporous appearance. We represent 4-year-old boy who showed nonconvulsive status of myoclonic-astatic epilepsy with typical clinical and eleckoencephalographic characteristics.

Objective To explore the effect of reduced glutathione and venous systemic oxygen perfusion on apoptosis and ultrastructure of hepatocytes in rat steatotic liver grafts. Methods Before liver transplantation grade Ⅱ steatotic liver model was established by a diet consisting of 79% standard diet,20% lard and 1% cholesterol for 6 weeks. In pretreatment group, the donor received intraperitoneal injection of reduced glutathione at a dosage of 500 mg/kg/body weight 3 times a day for 2 days, and intrahepatic venous oxygen perfusion for 6 hours while kept in cold preservation.
