1.Cutaneous Plasmacytoma Associated with Multiple Myeloma.

A 52-year-old man with multiple myeloma developed cutaneous nodules while being treated with melphalan and prednisolone. A biopsy specimen showed dermal infiltration by well differentiated plasma cells similar to those found on bone marrow biopsy. The use of peroxidase anti-peroxidase to demonstrate the monoclonality or polyclonality of the cytomplasmic immunoglohulins in the tumor cells revealed a positivity for IgG and 1 chain (ie, monotypic staining). Ultrastructurally, each plasmacytoma cell contained varyting amounts of rough endoplasmic reticulum and Golgi-apparatus. The cutnneous nodules completely disappeared after radiotherapy

2.Seborrheic Keratosis: A Clinical and Histopathological Study.

A total of 127 cases of seborrheic keratosis examind at the Dept. of Pathology, Yonsei University School of Medicine during 1980~1986 was reviewed. The results were as follow; 1) Sex distribution showed without preponderance with male; Female ratio of 1.05:1. 2) Age distrubution were 10~19 years 0.8%, 20~29 years 4.7%, 30~39 years 14.2%, 40-49 years 25.2%, 50~59 years 29.1%, 60~69 years 17.3%, 70~79 years 6.3% and neck (3.7%). 3) The most common site was face (44.8%), followed by trunk (29.8), lower extremities (10.4%), scalp (6.7%) and neck (3.7%). 4) The clinical diagnoses were sebrorrheic keratosis (47%), nevus (15.4%), verruca (12%), malignant melanoma (7.7%), actinic keratosis (3.4%), pyogenic granuloma (2.6%), mass (2.6%), basal cell epithelioma (0.9%), corn (0.9%), leproy (0.9%), and angiokeratoma (0.9%). 5) The distributions of the light microscopic diagnosis were acanthotic 52 cases (42.9%), hyperkeratotic 33 cases (30%), irritated 17 cases (13.4%), adenoid 15 cases (11.8%), clonal 3 cases (2.4%), melanoacanthoma 1 case (0.8%) and mixed 6 cases (4.7%). 6) The lesions measured from 0.2 cm to 3 cm in maximum diameter and its color was light brown to black with sharply demarcated papule, pladque or nodule.

4.Anterior Tibial Muscle Hernia Treated with Local Periosteal Rotational Flap: A Case Report.

Tibialis anterior muscle hernia is the most common hernia among lower extremity muscles. This condition can be diagnosed by physical examination and radiologic findings, especially by dynamic ultrasonography. There are surgical methods of treatment for muscle hernia, including direct repair, fasciotomy, fascial patch grafting using autologous fascia lata or synthetic mesh. We report a case of tibialis anterior muscle hernia treated with local periosteal rotational flap. Because there are several advantages to the local periosteal rotational flap, such as lack of donor site morbidity, lack of skin irritation, low cost, simplicity, and an easy approach, this technique could be an option for tibialis anterior muscle hernia.

7.A Rare Hyperextension Injury in Thoracic Spine Presenting with Delayed Paraplegia.

Hyperextension injury in the thoracic spine is uncommon with only a few cases documented in the literature. The mechanism of these injuries is hyperextension combined with axial or shearing force. These types of injuries are associated with a high risk of dural tears and paraplegia. A 91-year-old female presented with acute back pain from a hyperextension injury in thoracic spine with no neurological deficit. Lumbar magnetic resonance imaging showed a intervertebral disc rupture. On day 20 of hospitalization, the herniated intervertebral disc compressed the spinal cord with incomplete paraplegia. Hyperextension injuries involving the three columns are very unstable and we recommend surgical treatment as soon as possible, not only because of the initial trauma, but a ruptured disc herniation can damage the spinal cord.

8.Two Cases of Topless Optic Disc Syndrome.

Topless optic disc syndrome is considered as congenital optic nerve hypoplasia.The risk factors of this syndrome are female sex, prematurity and history of maternal diabetes.Normal central visual acuity, thinning of superior peripapillary nerve fiber layer and corresponding inferior visual field defects are the characteristic features of this syndrome.Relative superior entrance of central retinal vessels and superior scleral halo can be found in patients with this syndrome. We experienced two cases of topless optic disc syndrome :one was unilateral and the other, bilateral. They had no history of maternal diabetes. Diffuse superior retinal nerve fiber layer atrophy and inferior visual field defects were observed.Pallor of superior optic neural rim, relative superior entrance of central retinal vessels and superior scleral halo were also observed. It will be necessary to differentiate these findings from glaucomatous optic disc damage and visual field defects.

9.Results of Surgical Treatment of Complicated Pars Planitis.

We performed pars plana vitrectomy on 19 patients with pars planitis with its complications like vitreous opacity and retinal detachment.There were 16 males and 3 females with a mean age of 42.8 years.The follow-up ranged from 6 to 122 months with a mean of 25 months.Following surgery, final visual acuity was improved or unchanged in the 78.9 percent of patients.Eight cases (42.1%)showed improved vision, seven patients (36.8%) showed no change and four cases (21.1%)worse vision.Retinal detachment was successfully repaired in 17 patients and two eyes failed anatomically due to the proliferative vitreoretinopathy.The main factor favoring functional success was the short duration from the onset to the time of vitrectomy.It might be concluded that vitrectomy should be performed in the vision threatening stages on the patients with complicated pars planitis.

10.A Case of Giant Primary Liposarcoma of the Mediastinum.

The primary mediastinal liposarcoma is very rare and only a few cases have been reported. We report a 25 year-old man who have had coughing and mild fever for a month. The simple chest X-ray and computed tomography of the chest revealed a huge mass in the right thoracic cavity that originated in the anterior mediastinum. Cytologic examination of the needle aspiration biopsy confirmed the diagnosis of a primary mediastinal liposarcoma. Through an exploratory right thoracotomy and a median sternotomy, the huge mass, lobulated and about 2,500 gm in weight, was extirpated. Mass was attached to a portion of the pericardium and diaphragm, but there was no invasion or metastasis to pleura and lung. The patient was discharged on the 12th postoperative day. He is continuously receiving radiotherapy.
