1.A Case of Granuloma Gluteale Infantum.

Granuloma gluteale infantum is a granulomatous lesion in the diaper area of infants. Athough usually seen in infants, it has been described in incontinent elderly patients. The causes of granuloma gluteale infantum are not clear, although child care equipment(ditergents1, diapers2, and starch3), corticosteroids2,4,5,6, candidial infection3 urine and feces7 have been postulated for its etiology. We report a case of granuloma gluteale infantum in a 5-month-old boy. He was healthy except for an asymtomatic 1.5×1.5cm sized, round, Rat-topped brownish and firm nodule on the perianal area, and two, 1×3cm and 1×6cm sized brownish plaques on the intertriginous area of the neck. The lesions were resistant to the treatment of systemic antibiotics. These nodules completely resolved in the three months after we tried to keep the diaper area clean without using any corticosteroids or other ointments and after changing from plastic diapers to cotton o A histological examination of the nodule on the perianal area showed mild hyperkeratosis, spongiosis, and focal infiltration of neutrophils in the epidermis. 3.A Case of Mycosis Fungoides following Multiple Open Fractures.

We report a case of mycosis fungoides in a 30 year-old man who had been treated for multiple open fractures after a traffic accident. Histopathologic findings showed epidermotrophism wiih hyperchromatic haloed lymphocytes and atypical lymphocytic infiltration in the upper dermis. The infiltrated cells were positively stained with CD45Ro, but not with CD20. He had no evidence of extracutaneous involvement by laboratory studies, Skin lesions improved markedly after treatment with systemic PUVA therapy combined with topical corticosteroids for 3 months.

4.Fracture-separation of the distal humeral epiphysis in children.

We have reviewed seven cases of fracture-separation of the distal humeral epiphysis, one of which was initially misdiagnosed as a fracture of the lateral condyle. There were difficulties in making the diagnosis. The injury must be distinguished from an elbow dislocation and a fracture of the lateral humeral condyle. All seven patients revealed posteromedial displacement of the distal humeral epiphysis on initial x-rays. Three patients were treated by closed reduction and cast immobilization, and four patients by open reduction and internal fixation. All three patients with conservative treatment had slight cubitus varus (under 5 degrees). Two patients with operative treatment had significant deformities of the elbow, one 25 degrees of valgus and one 20 degrees of varus. In treatment of these injuries, accurate evaluation of the state of reduction is most important. We got acceptable results with a conservative treatment.

5.A Case of Lichen Nitidus Coexisted with Molluscum Contagiosum.

Lichen nitidus is an uncommon chronic dermatosis characterized by its typical clinical and histopathologic findings and has rarely been described in association with other diseases. We observed a patient with an association of lichen nitidus and molluscum contagiosum, which to the best of our knowledge has not been previously reported in the literature. It remains to be further studied about a possible pathogenesis of lichen nitidus secondary to some viral infections such as molluscum contagiosum, etc.

7.Pain on Injection of Propofol: Comparison of the Methods of Alleviation.

BACKGROUND: Various kinds of methods had been tried to reduce pain on injection of propofol. In this study, the effect of lidocaine pretreatment and that of temperature controlled injections were compared and evaluated its clinical utility. METHODS: One hundred and twenty patients were randomly allocated into 4 groups after permission. Room temperature propofol was used as induction agent in group P,. In group C, cooling (4 degrees C) propofol was used and warming propofol (37 degrees C) in group H. And room temperature propofol following lidocaine (1 mg/kg) was used in group L. Injection dosage of propofol was 2 mg/kg and injection speed was 2 ml/sec in all groups. In each patients, pain score and visual analog scale were measured and tested by ANOVA or Kruskal-Wallis test. RESULTS: There were no statistical significant difference in pain score among the 4 groups. But in visual analog scale analysis, group L markedly reduced values than the other groups by statistically significant manner (in Duncan grouping). CONCLUSION: The alleviating effect of lidocaine pretreatment on painful injection was better than that of changing temperature of propofol itself. More over effectiveness, in view of simplicity, we recommend lidocaine pretreatment.
