Sjogren-Larsson Syndrome is a rare disorder chracterized by congenital icthyosis, spastic diplegia or tetraplegia and mental retardation. The inheritance is autosomal recessive, due to deficiency of alcohol dehydrogenase activity of fatty acid matabolism. We have experienced a case of Sjogren-Larsson Syndrome in a 16 month-old male children who have dried thick skin, developmental delay, and spastic diplegia. He also has glistening spot on macula area with irregular high voltage slow wave (2-3Hz) on electroencephalography. From Biopsy finding, Lamella ictyosis with acanthosis and hyperkeratosis in epithelium is shown. He was much improved clinically for skin lesion and developmental status with etretinate supplement. We report the case with brief review of related literatures.

The radial head forms articulation with radial head fossa of proximal ulna and capitellum, and it ditectly contributes pronation and supination of forearm and also flexion and extension of elbow. There were debates in treatment of radial head fracture especially in displaced or communited fracture. From January 1982 to February 1992, we experienced and analysed 29 cases of radial head fracture. They were treated with conservative treatment or operative treatment according to type The results were as follows; 1. We could get better results with open reduction and internal fixation with miniscrew than radial head excision in type 11 radial head fracture. 2. There were unfavorable results of valgus instability, weakness in all cases of radial head excision.

The development of the arthroscopic surgery technique presented a great method in the reconstruction of anterior cruciate ligament. There were many debates about the timing of reconstruction in the acute anterior cruciate ligament injury. However, it was regarded true that primary reconstruction in the acute phase have higher complication rate of arthrofibrosis than in the chronic phase. Many authors recommended delay of reconstruction at least 3 weeks in acute tear of anterior cruciate ligament. In contrast, some surgeons have tried to suture the ruptured anterior cruciate ligament accompanied by 'over-the-top' augmentation with the hamstring tendon or the artificial ligament arthroscopically. Since Jan. 1992, the authors have implanted the synthetic polyester ligament(ABC ligament; Surgicraft, U.K.) into the substance of ruptured ligament proper and fixed at the 'over-the-top' position without any procedure for repair of the ruptured anterior cruciate ligament. Our indication for this surgery is only acute rupture of anterior cruciate ligament with good stump condition. We report the clinical results of 22 cases at 20.3 months follow-up(mean) preliminarily. l. On measurement of pre-operative and post-operative differences between the normal and the affected side by Telos stress X-ray study and KT-2000 arthrometer. Differences were markedly improved from 5.6mm(mean: pre-op) to 1.9mm(mean: post-op) on Telos stress X-ray study(on 15 Kp stress) and both knee showed minimal differences on KT-2000 study(2.2mm on 9Kp, 2.5mm on maximum stress) at the time of follow-up. 2. The average Lysholm score was 82. By Clancy criteria, the 20 cases(91%) showed good and excellent results. 3. Second look arthroscopy was available in three cases. In two cases, there were marked fibrous proliferation around the ligament. The stability felt good on probing. We could observe healing-like appearance of ruptured ACL with minimal fibrous tissue proliferation in one another case.

In the treatment of proximal tibial condyle fracture, it should be considered that it can often accompany injuries of the collateral ligament, both cruciate ligament, and the menisci of the knee. Moreover it is necessary to restore anatomical congruity of the articular surface accurately. We performed reduction of the depressed articular fragment under monitoring of arthroscopy and fixed with cannulated cancellous screws and accompanied by bone graft under the control of image intensifier in 2 cases of Type I(Pure cleavage) and 3 cases of type II(cleavage with depression) fractures. The results were very satisfactory. Arthroscopy assisted reduction and fixation are very useful method in the treatment of proximal tibial condyle fracture because of (1) the minimized operative morbidity, (2) the ability to evaluate and manage associated intraarticular pathology simultaneously, (3) the rapid rehabilitation, (4) the achievement of good reduction comparable to open reduction.

OBJECTIVE: The purpose of this study is to compare the effects of haloperidol and risperidone on the quality of life in chronic schizophrenic patients and to identify how the quality of life is related with the severity of psychopathology and drug side effects. METHODS: The study sample was composed of 38 ambulatory patients on haloperidol 1.5-4.5mg/day and 33 patients on risperidone 1-4mg/day for more than 2 years. They were physically healthy and had diagnoses of chronic undifferentiated or residual type of schizophrenia according to the criteria of DSM-IV. Those who had other comorbid mental disorders were excluded. For the assessment of the quality of life(QOL), the Korean version of WHOQOL was used. The severity of psychotic symptoms was evaluated by BPRS, and the side effects of the medication was evaluated by UKU Side Effects Rating Scales. RESULTS: The results showed that sex, age, education level, marital states, duration of antipsychotic medication and BPRS scores were not significantly different between the haloperidol group and risperidone group. But, the UKU Side Effects Rating Scales scores in the haloperidol group were significantly higher than those in the risperidone group. The mean score of the WHOQOL-BREF in the risperidone group was a little higher than that in the haloperidol group, and in the same way the 3 domain scores of the WHOQOL-BREF(physical, psychological, environmental domains) in the risperidone group was a little higher than those in the haloperidol group. However, the differences were not statistically significant between the two groups. There was a negative correlation between total QOL score and total BPRS scores, and both between BPRS item of tension and the total QOL score and between depressive mood and total QOL score. But, there was no correlation between total QOL score and UKU side effect scores. CONCLUSIONS: This study suggests that, if schizophrenia has once become chronic, the quality of life is not significantly influenced by the kind of antipsychotic drugs in the maintenance dose, and that the quality of life is correlated with a severity of the symptoms, especially tension and depressive mood.

OBJECTIVE: This study is to test the validity and reliability of the Korean version of the World Health Organization Quality of Life Assessment Instrument(WHOQOL). METHODS: According to the instructions of the WHOQOL group, THe WHOQOL was translated into Korean. Words for anchor points were selected based on a preliminary survey. Words or sentences were examined and modified by layman focus groups and a group of psychiatrists and psychologists so that the scale would be expressed in a more culturallyadequate and easily-understandable way. A back-translated Korean version into English was sent to the WHO group for examination. A group of 583 Koreans consisting of healthy people, medical patients and college students rated the Korean version of WHOQOL. For test-retest reliability, 113 college student rated it again after a 3-week interval. Statistical analysis was done with the collected data. RESULTS: Correlation coefficients were regarded to have good test-retest reliabilily. Cronbach alpha value of total score, and scores of domains and facets were high. Total score and score of domains and facets were significantly correlated with overall QOL score and QOL-general impression score showing a significant criterion validity. Path analysis for content validity showed a certain pattern of relations among domains. There were significant differences in some domains and facets between sexes and age groups, as well as between healthy group and patient groups showing a significant discriminative validity. CONCLUSION: The Korean version of WHOQOL was proved to be a reliable and valid instrument for quality of life.
