2.Nerve Regeneration After Autogenous Nerve Graft Using Perfabricated Adiponeural and Myoneural Flap: An Experimental Study.

Most of the peripheral nerve injuries from crushing or compressive forces are accompanied by surrounding soft tissue injuries. As a result, poor vascularity due to fibrosis and sacr formation compromises regeneration of the grafted nerve. Vascularized nerve graft shows superior regeneration to that of a non-vascularized one. However, the human body provides few donor sites of vascularized nerve graft clinically. We presumed that the prefabricated myoneural or adiponeural flap, which include fabricated nerves wrapped with surrounding vascularized muscle or adipose tissue flap, influences superiorly on the regeneration of grafted nerve because that surrounding vasculatity indirectly enhances the vascularity of the grafted nerve itself. Thirty adult male Sprague-Dawley rats were divided into three groups: 1) conventional reversed autogenous graft of the femoral nerve alone(n=10); 2) nerve graft entubulated with abdominal adipose tissue flap with a pedicle of inferior epigastric artery(n=10); 3) nerve graft entubulated with adductor muscle flap with a pedicle of the first muscular branch of the femoral artery(n=10). At three months postoperatively, grafted nerves were examined by electrophysiologic study to check amplitudes and motor nerve conduction velocities, as well as histopathologic study for evaluation of regenerated nerve cells, fibrosis and neo-vascularization. Consquently, nerve regeneration was found in all three groups. Both the myoneural and adiponeural flap groups had better improved results of nerve regeneration compared to that of the conventional nerve graft group. The result of myoneural flap group was superior to that of the adiponeural flap group. The myoneural flap group showed minimal fibrosis and less prominent neovascularization around moderately regenerated nerves. The adiponeural flap group showed more severe perineural and endoneural fibrosis, as well as vascular proliferation around focal regenerated nerves. The results of myoneural flap group proved to be statistically significant. We concluded that it is possible to use nerve graft entubulated with a vascularized muscle flap (myoneural flap) as a substitute for vascularized nerve graft.

3.Design and Implementation of Pulse-Diagnosis Ontology in Ubiquitous Computing Environment.

OBJECTIVE: This paper proposes an ontology plan for oriental-medical service in ubiquitous computing environment. With popularization of ubiquitous computing technology, recently oriental medicine field becomes interested in oriental medicine knowledge management system and needs more systematic-scientific knowledge management and information communication. But oriental medical scientists have various standards which classify a pulse, so the information about pulse is massive. METHOD: In this paper, for more systematic-scientific knowledge management and information communication, we designed and implemented ontology using the pulse which is used as diagnosis basis in oriental medicine. by using the pulse which is classified into twenty eight division we implemented ontology based on characteristic of each pulse. We represented the pulse with OWL language and used Racer as inference engine to check errors of implemented ontology. RESULT / CONCLUSION: Given the information of the pulse-diagnosis ontology, systematic-scientific knowledge management and information communication become possible so that oriental doctors are able to provide faster and effective medical service. Furthermore, self-diagnosis and medical service will be possible at anytime and anywhere regardless of time and place. In the future research, we will implement an integrated ontology with the whole of diagnosis basis in oriental medicine based on pulse ontology implemented in this paper.

Apert syndrome is a rare disorder characterized by malformation of the skull, (usually oxyce-phaly or acrocephaly), in association with sys temic syndactyly It is due to congenital disturbance in the growth of bone and soft tissue affecting principally the head, the hands and the feet. The original description was presented by Troquart in 1886. Since then, more than 200cases have been reported in the world up to 1970. Recently we have seena patient with typical Apert Syndrome and report it here with a brief eviewr of the relevant literature.

6.A Case of Male Turner Syndrome.

We prsent here a 6 year old who has abnormalities of his genitalia, as well as other characteristic features typical of Turner syndrome. He has a small penis with hypospadias, hypolastic testes, and cryptorchi dism. He has short stature, an abnormal looking face, a wide, short webbed neck, and cubitus valgus. He was found to have hydronephrosis and hydroureter on I. V. P. Chromosomal analysis revealed anormal male karyotype with no apparent aberrations.

7.Serum Interleukin-6 Concentration in Carcinoma of the Cervix.

Interleukin-6(IL-6) is a multifunctional cytokine, and deregulated IL-6 gene expression has been implicated as being involved in the pathogenesis of a number of diseaaes. So we measured the serum level of IL-6 to find a relationahip between uterine cervical cancer and serum IL-6 level. The serum levels of IL-6 were meesured m 54 patients adrnitted to the Severance hospital, from August, 1992 to February, 1993, of whom 27 patients were diagnosed as invasive cancer of cervix, 17 as CIN, and also utilizing 10 patiente diagnosed as uterine myoma as a control group for matched age and parity. The average serum level of IL-6 in invasive cancer, CIN, and uterine myoma were respectively 30.33pg/ml, 37.27pg/ml, and 12.45pg Jml. In 22% af cervical cancer patients (6 of 27), and 27% of CIN patients (4 of 17) studied blood levels of II-6 were detected whereas only 10% of uterine myoma (1 of 10) shorved the same result. However, the difference does not seem to be statistically significant, There was no statistically significant difference among any of the groups, and age and stage did not show any significant cmrelation, and the results need furt,ber investigations.

8.Clinical Observation of Pediatric Empyema.

Seventy cases of empyema treated on the pediatric Service of Kwangju Christian Hospital during 9 years 6 months from Jan. 1970 to Jun. 1979 were analyzed and summerized as follows : 1. Males outnumbered females 38 to 32. No seasonal difference in prevalence was notes. 2. About 60% of empyema(41 cases) were on the right side and 36% on the left side. In 3 cases both sides were affected. The most common concurrent disease was pneumonia, which was suspected as th etiology of empyema. 3. Common chief complaints were dyspnea(53%), fever(47%) and cough(43%). 4. Upon culture of pus, 21 cases among 63 cases(33%) showed no bacterial growth, indicating that had under gone antibacterical treatment before admission. 5. The most common causative organism was Staphylococcus aureus(41.2%) followed vy Pseudomonas(7.9%) Streptococcus(4.8%) E.Coli(4.8%) and Pneumococcus(4.8%). 6. Upon sensitivity test, no resistance was observed to Methicillin and Amikacin, but most of the causative agents were sensitive to Gentamicin, Cephalothin and Novobiocin, while they were least sensitive to Penicillin(17%) and colimycin(9%). 7. Average hospital days were 18, and two cases(2.9%) expired, both one year of age. Average duration of closed drainage was two weeks.
