3.A Case of AIDS-related Kaposi's Sarcoma.

We herein report a case of AIDS-related Kaposi's sarcoma (KS) in a 36-year-old male patient, who had a solitary nodular skin lesion on the left supraclavicular area. The histopathologic findings showed typical features of KS as spindle cell proliferation and vascular spaces lined with endothelial cells. The patient's serum was positive for antibodies to the human immunodeficiency virus (HIV) and he had opportunistic infection of Pneumocystis carinii pneumonia and pulmonary tuberculosis. The WBC count was 1,200/mm3 and CD4 count was 50/mm3 with decreased CD4/CD8 ratio to 0.06. He died due to an aggravated respiratory infection.

4.The fitness of adjustable dental impression trays on the Koreans.

Purpose: this study was to evaluate the fitness of adjustable dental impression trays on the Koreans : the trays used in the previous study by Kim et al. as part of the dental adjustable tray development project were improved and modified. Material and method: The patterns of tray were made through CAD-CAM process, and a simple silicone-base molds were made from them. The trial products were reproduced by pouring polyurethane into these molds. 30 male students(Wonkwang University, Dental College) and 30 female students (Wonkwang Health Science College, Department of Dental Hygiene) were selected and Reversible hydrocolloid impression materials were used for this study. The fitness of the trays was evaluated by measuring the width and length of impression materials of each measurement sites. Results and conclusion: 1. In adapting the trays inside the mouth, a uniform width of impression material(3.6mm) was obtained in most sites due to the tooth stops and the inclined planes accommodating the width of the tray. 2. The thickness of impression material in the central part of the palate was a mean 9.8 mm, which turned out to be somewhat thick. 3. In the mandible, the thickness of the impression material in the lingual side inferior to the contact point of the 1st and 2nd molars was 2.7mm, and the thickness of the material in the lingual side of the rearmost margin was 2.5 mm. The thickness of the impression material of these areas was relatively thin.

6.A Case of Erythropoietic Protoporphyria.

Erythropoietic protoporphyria(EPP) is an inherited inborn error of porphyrin metabolism caused by decreased activity of the enzyme ferrochelatase. EPP is characterized clinically by photosensitivity to visible light commencing in childhood, and biochemically by elevated red cell free protoporphyrin levels. We report herein a case of EPP which occurred in a 44-year-old man and his family. He had suffered from immediate photosensitivity since he was 4 years old. He was presented with burning, erythema, scars and waxy thickening of the sun-exposed skin. Red cell free protoporphyrin level was elevated and urinary porphyrins were normal. Histopathologically, homogeneous eosinophilic materials that stained with PAS were deposited in perivascular area of upper dermis. He was managed with light restriction and sunscreen.

7.A Case of Cutaneous Leishmaniasis.

Cutaneous leishmaniasis is usually a self-limited infection of skin caused by the genus Leishmania, transmitted by a sandfly. This disease is endemic in tropical and subtropical zone, but recently several cases of cutaneous leishmaniasis were reported in Korean who had worked in Middle East. We report a case of cutaneous leishmaniasis in a 24-year-old male patient, who had travelled in Africa.

8.Success rate and marginal bone loss of Osstem USII plus implants; Short term clinical study.

PURPOSE: The aim of this study was to evaluate the clinical value of Osstem(R) USII plus system implants. Clinical and radiographic data were analyzed for 88 implants placed and functionally loaded for a 12 month period at the Yonsei University Dental Hospital. MATERIALS AND METHOD: Based on the patient's medical records, clinical factors and their effects on implant marginal bone resorption, distribution and survival rate were analyzed. The marginal bone loss was evaluated at implant placement and during a 6 to 12 months functional loading period. The independent sample t-test was used to evaluate the interrelationship between the factors (alpha=0.05), and one way repeated measures ANOVA was used to compare the amount of marginal bone resorption. RESULTS: The cumulative survival rate for 88 implants was 100%. The marginal bone resorption from implant placement to prosthetic delivery was 0.24 mm and the average marginal bone resorption from prosthetic delivery to 12 months of functional loading was 0.19 mm. The total average bone resorption from implant placement to 12 months of functional loading was 0.43 mm. There were no statistically differences in the amount of marginal bone resorption when implants were placed in the maxilla or the mandible (P>.05), however, implants placed in the posterior areas showed significantly more marginal bone loss than those placed in the anterior areas (P<.05). CONCLUSION: Based on these results, the short term clinical success rate of RBM surface treated external connection domestic implants showed satisfactory results and the marginal bone loss was in accord with the success criteria of dental implants.

9.A Case of Successful Pregnancy in a Woman with Anti-M Isoimmunization after Intravenous Immunoglobulin Therapy.

Although severe hemolytic diseases of the newborn triggered by anti-M are very rare, anti-M alloantibodies have been known to be associated with a cause of multipie intrauterine death. Serological and hematological investigations have been reported on a woman who experienced four multiple intrauterine deaths due to anti-M. The mothers blood type was of group A, NN and the husbands cells were of group B, MN. In the serological examination at 9th week's gestation of the fifth pregnancy, anti-M antibodies were identified in her serum. The antibodies comprised IgM saline agglutinin at a titer of 16 at 4 degrees C and IgG agglutinin reacted in an indirect antiglobulin technique at a titer of 4 at 37 degrees C. She underwent high-dose immunoglobulin infusion therapy on a monthly program from 3rd month gestation and a total of 6 times of intravenous immunoglobulin was given. The anti-M titer did not rise during the pregnancy. She delivered a live girl by cesarean section at the 37th week because of a failure of induction. The childs blood type was of group O, MN. The child was discharged and developed normally.

10.Clinical Efficacy of Intravenous Immunoglobulin for the Prevention of Recurrent Spontaneous Abortion.

The aim of this study was to evaluate the efficacy of intravenous immunoglobulin treatment for recurrent spontaneous abortion. Immunologic causes in either alloimmune or autoimmune type have been suggested for more than 80% of these patients. Various immunotherapy including paternal leukocyte transfusion has been used, but there is controversy on its efficacy and side-effects. The proposed immunomodulatory mechanism of intravenous immunoglobulin includes passive transfer of blocking or anti-idiotype antibody, blockade of Fc receptors, enhancement of supressor T-cell function, down regulation of B cell function. In this study, we used intravenous immunoglobulin for the prevention of spontaneous abortion. Five patients with a history of two or more spontaneous abortions were enrolled in this study. Other etiologic factors such as anatomical, chromosomal, hormonal factors were excluded. Three of them were positive for anti-cardiolipin antibody (ACA). When the pregnancy was diagnosed at about five weeks of gestation, 30 g intravenous immunoglobulin was administered and continued up to 28 weeks with three weeks. Ongoing pregnancy beyond 20 weeks was considered successful. Four among five patients (80%) was successful in maintaining pregnancy now ongoing 20, 31, 33, 39 weeks. One patient with ACA positive had abartion due to anembryonic pregnancy. No adverse reaction was observed during the treatment. From these preliminary data, it is suggested that intravenous immunoglobulin treatment may be effective in maintaining pregnancy in patients with unexplained recurrent spontaneous abortion, Further studies are needed to clarify the its immunomodulatory mechanism and establish a more simplified protocol limiting the use at certain critical period of time.
