2.A Case of Cutaneous Larva Migrans.

A 34-year-old woman presented with several intensely pruritic erythematous serpiginous thread-like skin lesions which began as a small papule on the epigastrium 4 months prior to her visit and was migrating to the left chest area. The laboratory examination showed eosinophilia and the total serum IgE level was slightly increased. Histopathologic examination revealed a pustular burrow in the epidermis with a larva-like cystic lesion apart from the burrow in the epidermis. The patient was treated with topical 10 % albendazole cream 3 times daily for 1 week without recurrence for 22 months up to now. We thought this was a rare case which was confirmed by a skin biopsy showing larva in the epidermis.

3.Norm-referenced standard of health-related physical fitness of ROKAF pilots.

BACKGROUND: The present study is undertaken to acquire normative data of health-related physical fitness(HRPF) of Republic of Korea Air Force(ROKAF) aircrews and to provide norm-referenced standard of HRPF. METHODS: From 15 May 1998 through 14 December 1999, 1284 KAF pilots are checked muscular strength, muscular endurance, flexibility, and body fat percentage. RESULTS: The grip strength was relatively maintained, but flexibility, back strength, and muscular endurance were declined according to promotion and aging. And the incidence of obesity determined by bioelectrical impedence analysis was higher than that assessed on the basis of height and weight. CONCLUSION: It is suggested that many pilots who were normal in BMI have abdominal or visceral obesity. Further studies must be made on the health promotion program for the pilots, on the changes in physical fitness, and on the criterion-referenced standard of HRPF of the pilots.

4.Change in Expression of Survivin Caused by Using Oxaliplatin in HCT116 Colon Cancer Cells.

PURPOSE: Oxaliplatin is a third-generation platinum compound, and it has no nephrotoxicity and has reduced bone marrow toxicity. Cancer cells that are resistant to cisplatin are sensitive to oxaliplatin. Oxaliplatin is used widely for the treatment of colon cancers. Recently, oxaliplatin was reported to inhibit the expression of survivin, which protects cell apoptosis. However, there are no reports on the expressions of survivin variants and the changes in intracellular localization of survivin in cancer cells. We studied the expression of survivin caused by oxaliplatin in HCT116 colon cancer cells, and we observed the localization of survivin in the mitotic phase. METHODS: We treated the HCT116 colon cancer cells with 2.0 micrometer of oxaliplatin, and we studied the expressions of survivin protein, and survivin mRNA variants, as well as the changes in intracellular localization, by using the Western blot method, RT-PCR, immunocytochemistry, and flowcytometry. RESULTS: Oxaliplatin inhibits the expression of the survivin protein and survivin mRNA in HCT116 colon cancer cells. The expression of the survivin-2B variants, which have no antiapoptotic activity but control cell mitosis by localization on a microtubule, is reduced continuously 2 days after treatment with oxaliplatin. In immunocytochemistry, expression of survivin in the cytoplasm is reduced and especially is not expressed in microtubules and contractile rings. CONCLUSION: One of the mechanisms of oxaliplatin is to inhibit the expression of and to change the localization of survivin. Based on these results, we suggest that changes in the expression of survivin variants and in their localization are two effects of oxaliplatin.

5.Clinical Study on Chronic Granulomatous Disease(CGD) -Occurrence in Siblings-.

To analyse the age of onset, etiologic microorganisms, clinical manifestations, managements and genetic variation of chromic granulomatous disease, the authors reviewed four patients who were diagnosed as CGD at Department of Pediatrics, Seoul National Univesity Children's Hospital. They were siblings in relationship-two of them were brothers, the others were brother-sister. @ES The results were as follows; 1) Initial manifestations developed within 1 year old, and lymphadenopathy associated with BCG vaccination was the most common. 2) In culture study of micro-organism, catalase positive microorganisms such as Staphylococcus aureus, Serratia marcescens, Coagulase (-) Staphylococcus, Enterococcus, Proleus vulgaris, Klebsiella pneumoniae, E. coli and fungus such as Candida albicans were isolated. In 2 cases, culture study revealed no growth. 3) Fever, lymphadenopathy, hepatomegaly, suppurative dermatitis and pneumonia were the most common manifestations. In most of cases, antituberculous medications were administered under the impression of tuberculosis without response. 4) Immunologic screening tests including B-cell system, T-cell system, and complement system were within normal limit except NBT test. 5) In spite of severe infections, NBT scores of all the cases were less than those of controls. Liver biopsies of 3 cases showed granuloma formation with characteristic yellow brown pigment-laden macrophages.

6.3 Cases of Mobius Syndrome.

Mobius syndrome is a rare congenital disease characterized by unilateral or bilateral limitation of abduction and facial palsy due to the 6th and 7th cranial nerve paralysis. In addition, it may be associated with limb anomalies and other cranial nerve paralysis. We experienced three cases of Mobius syndrome characterized by esodeviation associated with bilateral limitation of abduction, bilateral facial atroph, tongue atrophy, congenital amputation of limb, and congenital clubfoot. We performed a large amount of bilateral medial rectus recession and unilateral lateral rectus resection, and obtained cosmetically successful results that orthophoric or less than 10 prism diopter of esotropia in primary postition, but mild limitation of adduction was observed as a complication.

8.Congenital Fibrous Papule of the Face.

A 2-month-old Korean boy presented with a solitary papule on the cheek which was noted at birth. Histopathologic findings were consistent with angiofibroma of fibrous papule of the face (FPF). FPF is known to affect adults, and congenital occurrence has not been reported to the best of our knowledge. We report a case of congenital FPF which showed a facial papule clinically and an angiofibroma histologically.

9.A Case of Pigmented Fungiform Papillae of the Tongue.

Pigmented fungiform papillae of the tongue is a rare disorder in Korea. It is characterized clinically by pigmentation confined to the fungiform papillae and histopathologically by the presence of melanophages in the upper dermis. We report a case of pigmented fungiform papillae of the tongue in a 21-year-old woman who had brown to violaceous dots on the fungiform papillae and pigmented patches on the right lateral dorsum of the tongue. Histological findings showed increased melanophages in the upper dermis.

10.Urinary Cytology in the Diagnosis of Urothelial Carcinoma.

The cytologic examination of the urine is very simple and inexpensive procedure for the detection and follow up the genitourinary tumors, and worthy as a mass screening test. The voided urines of 20 patients who were admitted to the department of urology, Seoul city hospital from October 1983 to September 1985 with the diagnosis of bladder carcinoma were analysed as to variations in PH and osmolality values and quality of cytologic smears. The same analyses were repeated after addition of trichloracetic acid (TCA) and after diuresis was obtained by Furosemide medication. Standardization of values of PH (average 2) and of osmolality (average 300 mosm/kg) was obtained by adding TCA to furosemide urine. The cellular smears of this TCA treated furosemide urine exhibited the finest cytological details when compared to the smears obtained by routine method.
